The horror of the Bondi Junction killings has been unpicked in forensic detail.

But beyond the need to bear witness to what happened, the purpose of last year’s inquest into the death of six people was about ways and means to prevent a reoccurrence.

That question can be simply put. How should we care for patients with treatment-resistant schizophrenia to ensure they do not find themselves like Joel Cauchi: unmedicated, psychotic, homeless and lost to any kind of support system?

The coroner’s clinical recommendations have been subject to far less analysis than the events leading up to the killings.
