Image credit: Helen Rushbrook\/StocksyAbout one in every nine adults around the world lives with diabetes, with 90% of them having type 2 diabetes.Past studies show that certain lifestyle factors can help lower a person\u2019s risk for type 2 diabetes, such as exercising regularly and eating a healthy diet like the Mediterranean diet. A new study has found that a combination of following a calorie-restricted Mediterranean-style diet, plus moderate exercise and following a weight-loss program, is a more effective way to lower type 2 diabetes risk compared to only following the Mediterranean diet. <\/p>\n<p>\u201cType 2 diabetes mellitus is a major public health problem worldwide,\u201d <a href=\"https:\/\/www.bioquimica.urv.cat\/en\/about\/directory\/full-time-faculty\/salas-salvado-jordi\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-90fpmc\">Jordi Salas-Salvad\u00f3<\/a>, MD, PhD, professor of nutrition at the University of Rovira i Virgili in Spain and general coordinator of the <a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0939475317300984\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-90fpmc\">PREDIMED trial<\/a>, told Medical News Today.<\/p>\n<p>\u201cThe prevalence of type 2 diabetes is increasing in parallel to the obesity epidemics, and type 2 diabetes is a potent risk factor for <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9559793\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-90fpmc\">cardiovascular disease<\/a>, <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9924922\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-90fpmc\">blindness<\/a>, <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC9780388\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-90fpmc\">renal failure<\/a>, and <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC8328738\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-90fpmc\">lower limb amputation<\/a>, decreasing quality of life, and increasing the healthcare expenditures. Through such complications, diabetes kills 5 million people annually worldwide and most of them prematurely,\u201d explained Salas-Salvad\u00f3.<\/p>\n<p>Salas-Salvad\u00f3 is the corresponding and senior author of a new study recently published in the journal <a href=\"https:\/\/www.acpjournals.org\/doi\/10.7326\/ANNALS-25-00388\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-90fpmc\">Annals of Internal Medicine<\/a>, which adds to the evidence suggesting the Mediterranean diet may help lower type 2 diabetes risk. <\/p>\n<p>In this study, researchers report that a combination of following a Mediterranean-style diet with fewer calories, plus moderate exercise and following a weight loss program, is a more effective way to lower type 2 diabetes risk compared to only following the Mediterranean diet. <\/p>\n<p>This study is part of <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/34474192\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-90fpmc\">PREDIMED-Plus<\/a> trial \u2014 the largest European nutrition trial first started in 2013. Researchers followed more than 4,700 adults ages 55 to 75 from 23 health centers around Spain for 6 years. At the start of the study, participants had either overweight or obesity, had <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/263834\" class=\"content-link css-90fpmc\" rel=\"nofollow noopener\" target=\"_blank\">metabolic syndrome<\/a>, and no history of heart disease or diabetes. <\/p>\n<p>Study participants were divided into two groups. The first group followed a calorie-reduced Mediterranean diet, a moderate exercise plan such as <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/brisk-walking\" class=\"content-link css-90fpmc\" rel=\"nofollow noopener\" target=\"_blank\">brisk walking<\/a> and <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/functional-strength-training\" class=\"content-link css-90fpmc\" rel=\"nofollow noopener\" target=\"_blank\">strength training<\/a>, and were provided guidance through a professional weight loss program.<\/p>\n<p>The control group only followed the Mediterranean diet without any calorie restriction, exercise advice, or weight loss plan. <\/p>\n<p>Of the trial, Salas-Salvad\u00f3 told us:<\/p>\n<p>\u201cSome years ago, in the context of the PREDIMED trial we have demonstrated that an ad libitum traditional MedDiet supplemented with either <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/318397\" class=\"content-link css-90fpmc\" rel=\"nofollow noopener\" target=\"_blank\">extra-virgin olive oil<\/a> or mixed nuts \u2014 typically Mediterranean food rich in vegetable fat \u2014 reduced diabetes incidence by 30%, compared with a low-fat diet. However, this reduction occurred without parallel changes in body weight. Building on this finding, the PREDIMED-Plus trial was designed to test whether adding caloric restriction and physical activity to the MedDiet would provide additional benefit beyond the effect observed only with the MedDiet \u2014 the same recommendations that that we tested in the PREDIMED study a few years ago.\u201d <\/p>\n<p>At the study\u2019s conclusion, researchers found that participants following the combination approach of a calorie-reduced Mediterranean-style diet, more moderate exercise, and professional support for weight loss, reduced their risk of developing type 2 diabetes by 31%, when compared to the control group. <\/p>\n<p>\u201cIn adults aged 55 to 75 with overweight or obesity and other cardiometabolic risk factors, including <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/311240\" class=\"content-link css-90fpmc\" rel=\"nofollow noopener\" target=\"_blank\">prediabetes<\/a>, even a relatively modest reduction in calorie intake combined with moderate physical activity further reduces the likelihood of developing type 2 diabetes,\u201d <a href=\"https:\/\/mediterraneanhealth.org\/miguel-ruiz-canela\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-90fpmc\">Miguel Ruiz-Canela<\/a>, MD, PhD, professor and chair of the Department of Preventive Medicine and Public Health at the University of Navarra in Spain and first author of this study, explained to MNT.<\/p>\n<p>\u201cClinicians should consider recommending this multi-domain intervention approach for patients with overweight or obesity, particularly when conventional Mediterranean dietary advice alone has proven insufficient. We think that this multi-component lifestyle modification represents a practical and sustainable strategy that could be incorporated into routine clinical practice for diabetes prevention,\u201d Ruiz-Canela told us.<\/p>\n<p>In addition, scientists discovered that studyparticipants in the combination group lost an average of 3.3 kilograms (kg) and reduced their <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/319439\" class=\"content-link css-90fpmc\" rel=\"nofollow noopener\" target=\"_blank\">waist circumference<\/a> by 3.6 centimeters (cm), compared to just 0.6 kg and 0.3 cm in the control group. <\/p>\n<p>\u201cA reduction of 3.3 kg in weight and 3.6 cm in waist circumference is clinically relevant, as even modest weight loss can significantly lower the risk of chronic conditions such as type 2 diabetes and cardiovascular disease,\u201d <a href=\"https:\/\/orcid.org\/0000-0002-1817-483X\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-90fpmc\">Montse Fit\u00f3<\/a>, MD, PHD, coordinator of the Cardiovascular Risk and Nutrition Research Group at the Hospital del Mar Research Institute and corresponding author on this study, told MNT.<\/p>\n<p>\u201cAdditionally, maintaining these effects over the long term is crucial for ensuring the applicability and translation of these findings into practical recommendations. In this sense, the lifestyle intervention of the study has demonstrated a good adherence long-term,\u201d said Fit\u00f3.<\/p>\n<p>MNT had the opportunity to speak with <a href=\"https:\/\/www.memorialcare.org\/providers\/mir-b-ali\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-90fpmc\">Mir Ali<\/a>, MD, a board-certified general surgeon, bariatric surgeon and medical director of MemorialCare Surgical Weight Loss Center at Orange Coast Medical Center in Fountain Valley, CA, about this study. <\/p>\n<p>\u201c[The findings are] not surprising to me because there have been other studies that show any kind of calorie deficit is beneficial for patients who are trying to lose weight,\u201d Ali, who was not involved in this research, commented. \u201cAnd also the Mediterranean diet, and diets based on that, they\u2019ve also been shown to help people lose weight and reduce symptoms for many conditions like diabetes, <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/150109\" class=\"content-link css-90fpmc\" rel=\"nofollow noopener\" target=\"_blank\">hypertension<\/a>, and other issues.\u201d <\/p>\n<p>\u201cDiabetes and obesity in general are on the rise in this country, and anything that can show improvement in that is going to be helpful,\u201d he continued. \u201cThe problem [is that] no one thing works for every person. So as long as there\u2019s research going on showing different options for people, better ways to improve their health, I think it\u2019s going to be helpful.\u201d <\/p>\n<p>MNT also spoke to <a href=\"https:\/\/eatrightrx.com\/about-me-2\/\" target=\"_blank\" rel=\"noopener noreferrer nofollow\" class=\"content-link css-90fpmc\">Monique Richard<\/a>, MS, RDN, LDN, a registered dietitian nutritionist and owner of Nutrition-In-Sight, likewise not involved in the study, who commented she was not surprised by its results, as we already know the Mediterranean diet is associated with improved <a href=\"https:\/\/www.medicalnewstoday.com\/articles\/323027\" class=\"content-link css-90fpmc\" rel=\"nofollow noopener\" target=\"_blank\">insulin sensitivity<\/a>, reduced inflammation, and better metabolic outcomes.<\/p>\n<p>\u201cThis study highlights the added power of pairing dietary quality with moderation in calories, movement, and support \u2014 a trifecta that\u2019s realistic and sustainable for those willing to learn, explore and embrace change,\u201d Richard told us.<\/p>\n<p>\u201cThe study reinforces that lifestyle patterns \u2014 not one single food \u2014 has the ability to drive prevention of chronic disease.
