No one knows how big the impact is"},"content":{"rendered":"<p>                Facebook<\/p>\n<p>                Tweet<\/p>\n<p>        <a class=\"social-share_labelled-list__share\" href=\"mailto:?subject=CNN%20content%20share&amp;body=Check%20out%20this%20article%3A%0Ahttps%3A%2F%2Fwww.cnn.com%2F2025%2F09%2F13%2Fafrica%2Fhiv-aids-program-cuts-trump-pepfar-intl\" data-type=\"email\" target=\"_blank\" rel=\"noopener noreferrer\" aria-label=\"share with email\" title=\"Share with email\"><\/p>\n<p>                Email<br \/>\n        <\/a><\/p>\n<p>                Link<\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmfe7icqk002l28p2adv2fvtf@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The United States has suspended some funding for its flagship AIDS relief program, according to international organizations and members of Congress who warn the cuts are already hurting patients and halting critical projects globally.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5w000e3b6n1usu34pp@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The full extent of the budget cuts related to US-funded HIV\/AIDS relief work is highly unclear, and Congress is still battling the White House\u2019s proposed budget clawbacks and withholding of billions of dollars in funding.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5w000f3b6nx0ezk5sk@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            PEPFAR, formally called the US President\u2019s Emergency Plan for AIDS Relief, is credited with saving more than 26 million lives over the past two decades and preventing millions of HIV infections, particularly in Africa.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5w000g3b6nec8zekxz@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Last year alone, government figures show that PEPFAR provided 20.6 million people with life-saving HIV medicines, called antiretroviral therapy. It also supported more than 342,000 health workers to deliver HIV treatments, prevention care and support services, bolstering healthcare systems in <a href=\"https:\/\/www.hiv.gov\/federal-response\/pepfar-global-aids\/pepfar\" target=\"_blank\" rel=\"nofollow noopener\">more than 50 countries<\/a>.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5w000h3b6npti4mkmz@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Funding for the landmark program \u2013 established in 2003 by the Bush administration \u2013 was primarily implemented by USAID, which US President Donald Trump dismantled earlier this year after a freeze on foreign aid. The US State Department later issued a waiver to exempt life-saving services from that freeze, including those of PEPFAR, and moved operations to the State Department\u2019s remit.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000i3b6nfmh68hki@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            But nonprofits are sounding the alarm that HIV\/AIDS projects have been terminated regardless. And without USAID, implementation of many planned initiatives has also stalled. Such cuts are severely disrupting patients\u2019 medical care across Africa, Asia and Latin America, threatening the global fight against the disease, they say.\n    <\/p>\n<p>       <img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/uk\/wp-content\/uploads\/2025\/09\/1757767268_997_002-002-gettyimages-2212122508.jpg\" alt=\"An aerial view of Migosi Sub-county Hospital in Kisumu, Kenya, where the continuity of services for thousands of HIV patients has been threatened.\" class=\"image_large__dam-img image_large__dam-img--loading\" onload=\"this.classList.remove('image_large__dam-img--loading')\" onerror=\"imageLoadError(this)\" height=\"1875\" width=\"2500\" loading=\"lazy\"\/><\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000k3b6n7qdzi1as@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            UNAIDS, the United Nations\u2019 agency working to end AIDS, has cited examples around the world of medicine stockouts, staffing cuts at HIV\/AIDS clinics, suspended community outreach services and \u201crising levels of stigma, discrimination and mortality rates\u201d in the wake of the <a href=\"https:\/\/www.unaids.org\/en\/resources\/presscentre\/featurestories\/2025\/may\/20250512_community-led-services\" target=\"_blank\" rel=\"nofollow noopener\">US funding cuts<\/a>.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000l3b6nlskqzkv3@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The suspensions have caused major disruptions to HIV responses in dozens of countries, including <a href=\"https:\/\/www.unaids.org\/en\/resources\/presscentre\/featurestories\/2025\/july\/20250718_uganda\" target=\"_blank\" rel=\"nofollow noopener\">Uganda<\/a>, <a href=\"https:\/\/www.unaids.org\/en\/resources\/presscentre\/featurestories\/2025\/july\/20250721_philippines\" target=\"_blank\" rel=\"nofollow noopener\">the Philippines<\/a> and <a href=\"https:\/\/www.unaids.org\/en\/resources\/presscentre\/featurestories\/2025\/may\/20250505_people-who-use-drugs\" target=\"_blank\" rel=\"nofollow noopener\">Tanzania,<\/a> the UN agency said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000m3b6niqmep49e@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            People living with HIV are skipping and rationing doses of antiretroviral medications, creating conditions for drug-resistant HIV strains to emerge, according to the nonprofit Physicians for Human Rights, which compiled <a href=\"https:\/\/phr.org\/news\/trump-administration-disruptions-to-global-hiv-efforts-have-caused-severe-harms-but-congress-can-still-preserve-live-saving-programs-new-phr-research\/\" target=\"_blank\" rel=\"nofollow noopener\">testimonies<\/a> of healthcare disruptions from people in Tanzania and Uganda.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000n3b6nvih6xm03@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            <a href=\"https:\/\/www.doctorswithoutborders.org\/latest\/how-hiv-funding-cuts-are-undermining-years-progress-zimbabwe\" target=\"_blank\" rel=\"nofollow noopener\">Doctors Without Borders<\/a> has warned that \u201cPEPFAR faces an uncertain future\u201d and that project cancellations following the dismantling of USAID have already impacted humanitarian work.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000o3b6nfk87uj9q@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cPEPFAR\u2019s scope of work has already been dramatically reduced since January when the State Department restricted its work on key areas of HIV prevention, treatment, care, and support,\u201d the Doctors Without Borders statement said, contradicting the US government\u2019s repeated claims that lifesaving aid work is being preserved.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000p3b6n4r9gr3rn@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cCuts are not just hitting program activities and medical stocks; they are crippling the logistical backbone of HIV care. Transport for distributing supplies has all but vanished,\u201d said the organization\u2019s representative in Zimbabwe, Zahra Zeggani-Bec.\n    <\/p>\n<p>       <img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/uk\/wp-content\/uploads\/2025\/09\/1757767269_191_003-004-2025-03-11t072002z-1555113881-rc20tcact4o1-rtrmadp-3-usa-trump-kenya.jpg\" alt=\"Children play on a trampoline at the Nyumbani Children's Home in Kenya, which cares for more than 100 children with HIV, whose parents died of the disease, while providing them with US-funded supplies of antiretroviral medicines.\" class=\"image_large__dam-img image_large__dam-img--loading\" onload=\"this.classList.remove('image_large__dam-img--loading')\" onerror=\"imageLoadError(this)\" height=\"1652\" width=\"2500\" loading=\"lazy\"\/><\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000q3b6nl4pdxqzh@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            World Vision, an international Christian organization focusing on poverty and development, told CNN it had a large PEPFAR program in Kenya that had been terminated.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000r3b6nj6zaq2am@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThat (program) had focused primarily on orphans and vulnerable children and prevention activities,\u201d said Margaret Schuler, World Vision\u2019s chief impact officer. Shuler said it was surprising that \u201cwhat would have been considered \u2018lifesaving programs\u2019 were terminated,\u201d including other World Vision programs related to health care and disease control.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000t3b6n6qumoog0@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            PEPFAR reporting data has been publicly unavailable for months, meaning there\u2019s little clarity on the program\u2019s ongoing activities. A <a href=\"https:\/\/data.pepfar.gov\/?_gl=1*s1iv85*_gcl_au*NDcyMjk2NTA1LjE3NTY0ODEzMTM.\" target=\"_blank\" rel=\"nofollow noopener\">message<\/a> on the government website says it is \u201cundergoing updates.\u201d The release dates for 2025 PEPFAR data reports are all listed as \u201cTBD.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000u3b6n4k1yweom@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            A State Department spokesperson told CNN that \u201cdata collection is ongoing to capture recent updates to programming.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000v3b6nt6b1zv30@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            UNAIDS said in a report in April that, among 70 of its country offices, 28 (40%) had witnessed an end to community-led services due to the US funding cuts. Meanwhile, 21 (30%) reported that services by international NGOs had been stopped.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000w3b6n29uqc0ux@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Some of that work may have resumed, but it\u2019s impossible to determine how much, given the lack of data on the budget, contracted services and \u201cwhat has actually been delivered,\u201d said Charles Kenny, a senior fellow at the Center for Global Development, a think tank based in the US and UK.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000x3b6nnlzh3cpm@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            He wrote in an <a href=\"https:\/\/www.cgdev.org\/blog\/theres-lot-we-dont-know-about-status-us-assistance-delivery-should-worry-congress\" target=\"_blank\" rel=\"nofollow noopener\">analysis of the status of US aid operations<\/a> that \u201cright now, Congress and taxpayers have no visibility on what the foreign assistance system is trying to accomplish, let alone if that is happening.\u201d\n    <\/p>\n<p>       <img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/uk\/wp-content\/uploads\/2025\/09\/1757767269_600_004-gettyimages-2200213177.jpg\" alt=\"Antiretroviral medication is dispensed at TASO Mulago service center in Kampala, Uganda, on February 17, 2025.\" class=\"image_large__dam-img image_large__dam-img--loading\" onload=\"this.classList.remove('image_large__dam-img--loading')\" onerror=\"imageLoadError(this)\" height=\"1667\" width=\"2500\" loading=\"lazy\"\/><\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000y3b6nsnvccrw5@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Secretary of State Marco Rubio has stated PEPFAR is an important and life-saving program that will continue, a State Department spokesperson told CNN, adding that Rubio has \u201calso said that PEPFAR, like all assistance programs, should be reduced over time as they are impactful in achieving their mission.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x000z3b6nf4k5jiev@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Last week, the State Department announced a joint commitment with the Global Fund to purchase the drug lenacapavir, an HIV prevention injection that only has to be taken twice a year, from the American biopharmaceutical company Gilead Sciences. PEPFAR will distribute the drug in eight to 12 high-burden HIV countries in 2026, with a focus on reducing the number of new HIV infections in pregnant and breastfeeding mothers, according to a State Department <a href=\"https:\/\/www.state.gov\/releases\/the-united-states-presidents-emergency-plan-for-aids-relief\/2025\/09\/pepfar-commits-to-distributing-breakthrough-hiv-drug-lenacapavir-demonstrating-american-excellence-in-science-and-american-leadership-in-hiv-prevention\/\" target=\"_blank\" rel=\"nofollow noopener\">statement<\/a>.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x00113b6nft5c07lk@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The White House Office of Management and Budget (OMB) has released only about half ($2.9 billion) of the $6 billion appropriated by Congress for PEPFAR\u2019s 2025 funding, according to <a href=\"https:\/\/openomb.org\/file\/11440522#tafs_11440522--019-1031-2025-2029--3--2025\" target=\"_blank\" rel=\"nofollow noopener\">budget documents<\/a> on a nonprofit website that tracks the OMB.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x00123b6n97w6jv3g@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            A congressional aide raised concerns that the budget document listed billions of dollars of this year\u2019s funds as part of fiscal year 2026, which the aide described as atypical.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x00133b6nmucz1miz@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            A budget expert told CNN that the 2025 PEPFAR apportionment looks \u201cweird\u201d compared to <a href=\"https:\/\/openomb.org\/file\/11396527#tafs_11396527--019-1031-2024-2028--2--2025\" target=\"_blank\" rel=\"nofollow noopener\">previous years\u2019 documents<\/a>, which typically specify how much money is going to each government department involved in PEPFAR. For this year, the released funds are listed as \u201cunallocated\u201d and conditional on a spending plan that has to be agreed upon by OMB and the State Department. Those spending plans are not publicly available.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x00143b6nunmmnm7l@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The source also said that placing the funds into fiscal year 2026 is at least an indication that OMB is not willing to spend the PEPFAR money now, and it could be an attempt to \u201cslow walk\u201d the funding.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x00153b6nze3xz453@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Another congressional aide told CNN that Congress doesn\u2019t have a clear picture because of a lag time in the information the White House must report to the public. It\u2019s possible that 2025 funding could still be released in the coming weeks and months.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x00163b6n29312e6p@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The picture of funding flows remains fuzzy, and it comes after the Trump administration already tried to claw back $400 million for PEPFAR. That proposal was canceled after bipartisan opposition in the Senate. Yet not all of those funds had been released, top Senate appropriator Susan Collins, a Republican from Maine, told CNN in a statement last week.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x00173b6nmnkuuhxn@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cOMB is blocking funding for PEPFAR, one of the most successful global health programs in history,\u201d Collins said. \u201cPEPFAR funds are simply not reaching those in need, as confirmed by those in the field.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x00183b6n3geqjfcr@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The ranking Democrat on the Senate Appropriations Committee, Senator Patty Murray of Washington, has called out OMB Director Russell Vought directly, saying that \u201ceven after promising Republican lawmakers that the program would be protected, he\u2019s choked off a huge chunk of funding provided by Congress for PEPFAR.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x00193b6nrf7fak55@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe law is clear as day: the full funding Congress provided must be used for the work PEPFAR does day in and out. The more these funds are obstructed and delayed, the more people will die needlessly,\u201d Murray told CNN in a statement.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x001a3b6ndtsteon1@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            CNN has reached out to OMB for comment but has not received a response.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmff38e5x001c3b6ne64x9fdw@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Separately, last week, Trump notified Congress he was moving to cancel $4.9 billion in foreign aid already approved for this year, drawing criticism from lawmakers from both parties who questioned the legality of the move.
