The idea is deeply rooted in American culture, especially in the US Midwestern states that helped bring Donald Trump to power. His \u201cbig, beautiful bill\u201d \u2014 which includes one of the largest cuts ever in America\u2019s social safety net \u2014 will force many recipients of public health insurance to prove they have a job to qualify.<\/p>\n<p>So who are they, and how hard will it be for them to keep <a href=\"https:\/\/www.ft.com\/us-healthcare-reform\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">healthcare<\/a> coverage in a country where a single accident or operation can bankrupt someone? They are nearly all of us: almost 70 per cent of those polled recently by KFF, the health policy research group, said <a href=\"https:\/\/www.kff.org\/medicaid\/poll-finding\/7-charts-about-public-opinion-on-medicaid\/\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">someone close to them had received help from Medicaid<\/a> (the taxpayer-funded health insurance programme for low-income and disabled Americans). <\/p>\n<p>Even among Republicans, 51 per cent had been on Medicaid themselves or had a family member covered.\u00a0In the Midwest in 2023, 25 to 30 per cent of the population was covered by <a href=\"https:\/\/www.medicaid.gov\/state-overviews\/scorecard\/measure\/Percentage-of-Population-Enrolled-in-Medicaid-or-CHIP?pillar=3&amp;dataAvailabilities=%5B%221%22%5D&amp;measure=EE.9&amp;measureView=state&amp;dataView=pointInTime&amp;chart=map&amp;timePeriods=%5B%222023%22%5D\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">either Medicaid or the children\u2019s health insurance programme (Chip<\/a>) \u2014 including several members of my family. Now, <a href=\"https:\/\/www.kff.org\/medicaid\/issue-brief\/a-closer-look-at-the-medicaid-work-requirement-provisions-in-the-big-beautiful-bill\/\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">nearly a quarter<\/a> of them will have to prove they work, study or volunteer for 80 hours a month, or are exempt from doing so.\u00a0<\/p>\n<p>Brooke Rollins, the US agriculture secretary, recently suggested that <a href=\"https:\/\/www.reuters.com\/legal\/government\/us-farm-secretary-says-no-amnesty-farmworkers-deportation-2025-07-08\/\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">Medicaid recipients could replace deported farm workers<\/a>. But many recipients affected by the new law \u2014 mostly childless able-bodied adults \u2014 are already working. KFF estimates that 64 per cent of working age Medicaid recipients <a href=\"https:\/\/www.kff.org\/medicaid\/issue-brief\/understanding-the-intersection-of-medicaid-and-work-an-update\/\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">already have a full or part-time job<\/a>. Another 28 per cent can\u2019t work due to caregiving, illness, disability or school attendance. The American Enterprise Institute uses different data and finds that <a href=\"https:\/\/www.aei.org\/opportunity-social-mobility\/less-than-half-of-medicaid-recipients-work-enough-to-comply-with-a-work-requirement\/\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">less than half of those affected<\/a> already work.<\/p>\n<p>Either way, many Medicaid recipients are clearly already working. So what impact will the new law have? When Arkansas experimented with work requirements, <a href=\"https:\/\/www.kff.org\/medicaid\/issue-brief\/state-data-for-medicaid-work-requirements-in-arkansas\/\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">thousands lost Medicaid<\/a> cover. And the Congressional Budget Office estimates the new requirements will increase the number of Americans without health insurance by <a href=\"https:\/\/www.cbo.gov\/system\/files\/2025-06\/Arrington-Guthrie-Letter-Medicaid.pdf\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">4.8mn<\/a>. This will be achieved mostly by \u201cdriving people off Medicaid who can\u2019t navigate the red tape\u201d, Drew Altman, president and chief executive of KFF, told me. <\/p>\n<p>I\u2019ve interviewed Medicaid recipients (who didn\u2019t want to be identified) in Ohio, a Republican state whose government believes Medicaid work requirements \u201cgive Ohioans the opportunity to live up to their God-given potential\u201d. Most of them are working, but fear they will lose coverage because of bureaucratic hurdles. \u201cEvery new administrative requirement is another barrier [to coverage],\u201d <a href=\"https:\/\/www.theohiocouncil.org\/staff\" data-trackable=\"link\" rel=\"nofollow noopener\" target=\"_blank\">Teresa Lampl<\/a>, an Ohio Medicaid expert, told me. \u201cNotification comes through snail mail,\u201d she adds, and if it\u2019s a text message recipients may wonder if it\u2019s a scam.<\/p>\n<p>Melanie is an entrepreneur who has been on and off Medicaid for years. \u201cI\u2019m a libertarian, I don\u2019t believe in big government and it\u2019s hard for me philosophically to be on Medicaid,\u201d she tells me. But she has to rely on it while she builds a new business. She\u2019s worried that being self-employed will make it difficult to prove how many hours she\u2019s working. If she can\u2019t, she could lose coverage again. <\/p>\n<p>Traci\u2019s 28-year-old son works as a delivery driver. Medicaid pays for treatment for his bipolar disorder that would otherwise bankrupt the family. As a gig worker, she fears, he may struggle to document his work hours \u2014 or prove that he qualifies for an exemption due to mental illness.<\/p>\n<p>\u201cEveryone in America salutes the flag of work,\u201d Altman says. That\u2019s why Republicans in Congress were able to push through the new requirements. \u201cBut it makes no sense for states to mount Herculean efforts\u2009.\u2009.\u2009.\u2009that might result in just a tiny, tiny percentage [more] people working. The juice isn\u2019t worth the squeeze.\u201d\u00a0<\/p>\n<p>The Medicaid recipients I interviewed don\u2019t need a push from Congress to get jobs \u2014 they already have them. But they say they need Medicaid to keep working. Tying them up in red tape could yet backfire for Republicans. Even in the pull-yourself-up-by-your-bootstraps Midwest, millions can\u2019t get healthcare without Medicaid.
