by the Care Quality Commission (CQC) following an inspection in December.\n<\/p>\n<p>\n  The CQC raised serious concerns about the leadership and management of the practice, identifying multiple breaches of legal regulations, including failures in providing safe care and treatment, staffing, and the employment of fit and proper persons.\n<\/p>\n<p>\n  Victoria Oecken, deputy director of primary care and community services in the South West, said: &#8220;When we inspected Cricklade Surgery, we found people were being put at risk by poor leadership and ineffective systems.\n<\/p>\n<p>\n  &#8220;Although people were positive about the care and treatment they received, our inspection identified multiple areas where the service wasn\u2019t meeting required standards.&#8221;\n<\/p>\n<p>\n  Inspectors found that the service wasn\u2019t always following national guidance when it came to managing long-term conditions.\n<\/p>\n<p>\n  In one example, 70 patients may have had a missed diagnosis of chronic kidney disease.\n<\/p>\n<p>\n  Reception staff were responsible for triaging appointment requests, but leaders had not provided them with training or triage tools to support them to do this.\n<\/p>\n<p>\n  Ms Oecken added: &#8220;It was clear the service wasn\u2019t providing staff with enough training or support to deliver safe, effective care.\n<\/p>\n<p>\n  &#8220;For example, there were times when there weren\u2019t any clinicians present at the surgery, but leaders hadn\u2019t ensured staff had the required skills or training to respond if there was a clinical emergency during those times.&#8221;\n<\/p>\n<p>\n  The report also revealed a lack of adequate supervision and support for staff.\n<\/p>\n<p>\n  Appraisals were not being completed, and regular one-to-one supervision was not in place.\n<\/p>\n<p>\n  This meant that staff performance and safety were not being properly monitored.\n<\/p>\n<p>\n  The surgery was found to be clean and well-maintained, but there were gaps in health and safety arrangements.\n<\/p>\n<p>\n  There were no trained fire marshals on site, putting patients and staff at risk in the event of a fire.\n<\/p>\n<p>\n  Ms Oecken said: &#8220;We found significant issues with how leaders were managing medicines at the surgery.\n<\/p>\n<p>\n  &#8220;Some people were recorded as having had a medication review when they hadn\u2019t, and staff told us they helped dispense prescribed medicines despite having no training to do so.\n<\/p>\n<p>\n  &#8220;This means people may not have been receiving their medicines safely, putting them at risk of harm.&#8221;\n<\/p>\n<p>\n  Inspectors also identified a backlog of vital test results waiting for review, some dating back several months.\n<\/p>\n<p>\n  Ms Oecken said: &#8220;Leaders didn\u2019t have effective systems and processes in place to manage medicines safely, monitor long-term conditions, or deal with test results appropriately.\n<\/p>\n<p>\n  &#8220;They\u2019d allowed a backlog to build up of test results waiting to be reviewed, some dating back months.\n<\/p>\n<p>\n  &#8220;For example, there were 232 pathology test results awaiting review, including both normal and abnormal results.\n<\/p>\n<p>\n  &#8220;This means people may not have received timely diagnoses or treatment.&#8221;\n<\/p>\n<p>\n  The CQC has issued three warning notices to the surgery and requested an action plan.\n<\/p>\n<p>\n  Following the inspection, the surgery&#8217;s ratings for safe and well-led were downgraded to inadequate, while its effectiveness dropped from good to requires improvement.\n<\/p>\n<p>\n  Caring and responsive were not assessed during this inspection and remain rated as good from a previous visit.\n<\/p>\n<p>\n  The CQC has placed the service in special measures, which means it will be kept under close review while improvements are made.\n<\/p>\n<p>\n  Ms Oecken said: &#8220;We have told leaders exactly where we expect to see rapid improvements and we will continue to monitor the service closely to keep people safe during this time.&#8221;\n<\/p>\n<p>\n  Special measures also set out clear timeframes for improvement and what further action will be taken if necessary changes are not made.\n<\/p>\n<p>\n  The inspection was prompted by concerns raised about the quality of care at the surgery.<\/p>\n","protected":false},"excerpt":{"rendered":"Cricklade Surgery in Wiltshire, which serves around 3,400 patients, was rated &#8220;inadequate&#8221;
