(Supplied:\u00a0Royal Commission into Violence, Abuse, Neglect, and Exploitation of People with Disability)<\/p>\n<p class=\"paragraph_paragraph___QITb\">The ombudsman&#8217;s report found Kaleb and Jonathan \u2014 who were diagnosed with &#8220;significant global development delay&#8221; and intellectual disability \u2014 missed multiple appointments with Queensland Health specialist services between 2005 and 2020.<\/p>\n<p class=\"paragraph_paragraph___QITb\">However, health workers did not always follow up on their non-attendance.<\/p>\n<p class=\"paragraph_paragraph___QITb\">Despite this, and reports to child protection services between 2000 and 2005, Queensland Health did not make any other referrals to Child Safety.<\/p>\n<p class=\"paragraph_paragraph___QITb\">The shocking case was examined by the disability royal commission to determine how and why they experienced violence, abuse, neglect, and a deprivation of human rights during their childhood and adolescence.<\/p>\n<p class=\"paragraph_paragraph___QITb\">The commission recommended that the Queensland government should apologise for omissions in preventing the harm they experienced.<\/p>\n<p class=\"paragraph_paragraph___QITb\">A formal apology was delivered in September 2023.<\/p>\n<p><img decoding=\"async\" alt=\"a squalid bedroom with a sheet over the window\" class=\"Image_image__5tFYM ContentImage_image__DQ_cq\"  src=\"https:\/\/www.newsbeep.com\/uk\/wp-content\/uploads\/2026\/04\/dd0d1d0d9447dda811b2dd9aa599f9b8.png\" loading=\"lazy\" data-component=\"Image\" data-lazy=\"true\"\/><\/p>\n<p class=\"Typography_base__sj2RP FigureCaption_text__zDxQ5 Typography_sizeMobile12__w_FPC Typography_lineHeightMobile20___U7Vr Typography_regular__WeIG6 Typography_colourInherit__dfnUx\" data-component=\"Typography\">The royal commission recommended the Queensland government should apologise for omissions in preventing the harm the boys experienced. (Supplied:\u00a0Royal Commission into Violence, Abuse, Neglect, and Exploitation of People with Disability)<\/p>\n<p class=\"paragraph_paragraph___QITb\">The Queensland ombudsman has conducted an independent review into the powers and responsibilities of all departments and agencies that engaged with Kaleb, Jonathon, and their father.<\/p>\n<p class=\"paragraph_paragraph___QITb\">That has resulted in three reports, including one into Queensland Health, which was tabled on Tuesday.<\/p>\n<p class=\"paragraph_paragraph___QITb\">His first report in April 2025 to Education Queensland found special school staff had &#8220;almost daily interaction&#8221; with the boys during their school years.<\/p>\n<p class=\"paragraph_paragraph___QITb\">Staff noticed Kaleb and Jonathon did not always have adequate clothing or food, sometimes needed bathing because they smelled of urine and dog odour, and at times <a class=\"Link_link__kR0xA Link_link__5eL5m ScreenReaderOnly_srLinkHint__OysWz Link_showVisited__C1Fea Link_showFocus__ALyv2\" href=\"https:\/\/www.abc.net.au\/news\/2025-04-09\/queensland-education-department-improvements-child-abuse\/105151536\" data-component=\"Link\" data-uri=\"coremedia:\/\/article\/105151536\" rel=\"nofollow noopener\" target=\"_blank\">passed &#8220;rocks and pebbles&#8221; in their bowel motions.<\/a><\/p>\n<p class=\"paragraph_paragraph___QITb\">The staff made notes in the education department&#8217;s information system about the brothers, but only one student protection report was submitted to the department of child safety.<\/p>\n<p class=\"paragraph_paragraph___QITb\">Ombudsman Anthony Reilly&#8217;s third report found the health department wanting across a range of areas, citing limited guidance and information to staff about recognising that missed health appointments could be a sign of neglect.<\/p>\n<p><a href=\"https:\/\/www.abc.net.au\/news\/2023-09-05\/qld-disability-royal-commission-abuse-autism-child-safety\/102813354\" data-component=\"FullBleedLink\" class=\"RelatedCard_link__rsgR9 FullBleedLink_root__lTw_U interactive_focusContext__yRhc_ interactive_defaults__AKxUU FullBleedLink_showVisited__g3Xvz\" rel=\"nofollow noopener\" target=\"_blank\">Queensland government should compensate teens with autism found living in filth, royal commission finds<\/a><\/p>\n<p class=\"Typography_base__sj2RP RelatedCard_synopsis__cFwMW Typography_sizeMobile14__u7TGe Typography_lineHeightMobile20___U7Vr Typography_regular__WeIG6 Typography_colourInherit__dfnUx\" data-component=\"Typography\">Brothers\u00a0Kaleb and Jonathan*, then aged 19 and 17,\u00a0were\u00a0found in May 2020 after their father was discovered dead from a terminal illness at their Brisbane home.<\/p>\n<p class=\"paragraph_paragraph___QITb\">It said Queensland Health also did not ensure staff received &#8220;consistent and regular&#8221; child protection training.<\/p>\n<p class=\"paragraph_paragraph___QITb\">&#8220;Queensland Health told us that one hospital and health service does not offer any child protection training,&#8221; the ombudsman noted.<\/p>\n<p class=\"paragraph_paragraph___QITb\">His investigation also found the department allowed hospital and health services to store child protection information in paper-based or electronic forms, or a combination of both.<\/p>\n<p class=\"paragraph_paragraph___QITb\">&#8220;If child protection information is stored in paper-based records, it is confined to one hospital and health service,&#8221; the report stated.<\/p>\n<p class=\"paragraph_paragraph___QITb\">&#8220;This could limit a staff member&#8217;s ability to properly assess a child protection concern or to identify potential patterns of harm or cumulative harm.&#8221;<\/p>\n<p class=\"paragraph_paragraph___QITb\">In his written response to the ombudsman, Queensland Health director-general David Rosengren said the department &#8220;continues to face challenges associated with information systems across a geographically dispersed and decentralised health system&#8221;.<\/p>\n<p class=\"paragraph_paragraph___QITb\">&#8220;Queensland Health will continue efforts to progress the Digital Hospital Program, with the intent of transitioning all hospital and health services to a world-class, digitally enabled health system,&#8221; Dr Rosengren said.<\/p>\n<p class=\"paragraph_paragraph___QITb\">The department told the ombudsman that in recent years, &#8220;almost one child in 10 has not attended an outpatient appointment&#8221;.<\/p>\n<p class=\"paragraph_paragraph___QITb\">&#8220;This is higher than the rate of non-attendance for the general population, regardless of age,&#8221; the report said.<\/p>\n<p class=\"paragraph_paragraph___QITb\">&#8220;We found this concerning.&#8221;<\/p>\n<p class=\"paragraph_paragraph___QITb\">After their discovery in 2020, Kaleb and Jonathon \u2014 both now aged in their 20s \u2014 were treated in hospital for two weeks for severe malnutrition.<\/p>\n<p class=\"paragraph_paragraph___QITb\">They were then released into state care with supports funded through the National Disability Insurance Scheme (NDIS).<\/p>\n<p class=\"paragraph_paragraph___QITb\">The ombudsman&#8217;s office visited the brothers when preparing the latest report, which made 13 recommendations.<\/p>\n<p class=\"paragraph_paragraph___QITb\">Among them were that Queensland Health introduce regular audits of a sample of outpatient appointments that children fail to attend.<\/p>\n<p class=\"paragraph_paragraph___QITb\">The ombudsman also recommended the department assists hospital and health services to implement more contemporary information management systems.<\/p>\n<p class=\"paragraph_paragraph___QITb\">Another recommendation calls for changes to procedures so that when staff identify a child protection concern, they consult with &#8220;their child protection unit if they have queries and it is practical to do so&#8221;.<\/p>\n<p class=\"paragraph_paragraph___QITb\">&#8220;This should be promoted as best practice,&#8221;
