Gary Carter, a former mental health nurse, who was part of the Early Intervention in Psychosis team which cared for Calocane, left the trust before an internal investigation into him had concluded, the inquiry heard previously.<\/p>\n

In his evidence to the inquiry<\/a>, he claimed Hull told him during a meeting “there’s no way we could have predicted [the attacks] and there’s no way we could have prevented it”. <\/p>\n

Hull said that she did not recall saying that. <\/p>\n

She told the inquiry: “I don’t remember those exact words. I know that what I would’ve said is that there is seldom a single point of failure, this will be systemic and there will be huge amounts of learning. <\/p>\n

“And we accepted all of the learning from all of those reviews.” <\/p>\n

Langdale asked Hull: “Well do you think it was predictable or preventable?”<\/p>\n

The chief nurse said: “I think it would be difficult, and I don’t think I could say that the terrible events of June 13 2023 could have been prevented. <\/p>\n

“But what I can say with absolutely clarity is that we failed to exhaust all of the options and all of the interventions available to us. <\/p>\n

“And given the kind of things we’ve heard particularly through the inquiry, it does lead me to think that had we \u2013 at different points during his care and our contact points \u2013 had we and others taken different action it might have impacted the outcome.”<\/p>\n

Langdale said: “Well, very likely impacted the outcome, not ‘might’.”<\/p>\n
