Research suggests that <a href=\"https:\/\/www.sph.umn.edu\/news\/study-suggests-body-dissatisfaction-begins-before-adolescence-remains-constant-into-adulthood\/\" rel=\"nofollow noopener\" target=\"_blank\">almost half of American adolescents<\/a> are dissatisfied with their body. By adulthood, it\u2019s more common for women to be dissatisfied with the way they look than satisfied (<a href=\"https:\/\/www.nationaleatingdisorders.org\/body-image-and-eating-disorders\/\" rel=\"nofollow noopener\" target=\"_blank\">National Eating Disorders Association<\/a>). <\/p>\n<p>Weight Loss and Substance Use<\/p>\n<p>In addition to being a key feature of many eating disorders, <a href=\"https:\/\/www.nationaleatingdisorders.org\/body-image-and-eating-disorders\/\" rel=\"nofollow noopener\" target=\"_blank\">body image disturbance<\/a> and a desire to lose weight can motivate substance use\u2014both legal and illegal\u2014as women strive to attain a culturally valued low body weight (<a href=\"https:\/\/www.tandfonline.com\/doi\/abs\/10.1080\/16066359.2022.2127692\" rel=\"nofollow noopener\" target=\"_blank\">Robinson, Walter, Deane, &amp; Larance, 2023<\/a>). For example, in a nationally representative sample of more than 12,000 young adults in the US, losing weight was one of the most common motivations for non-medical use of stimulants for women (<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7410496\/\" rel=\"nofollow noopener\" target=\"_blank\">Drazdowski et al., 2020<\/a>). Similarly, in a sample of 131 college students who used substances, 15.3 percent of women reported using drugs for weight control purposes; and those who did mostly used illicit stimulants known for their weight-loss and <a href=\"https:\/\/www.psychologytoday.com\/gb\/basics\/appetite\" title=\"Psychology Today looks at appetite\" class=\"basics-link\" hreflang=\"en\" rel=\"nofollow noopener\" target=\"_blank\">appetite<\/a>-suppressing side effects (including <a href=\"https:\/\/www.psychologytoday.com\/gb\/basics\/nootropics\" title=\"Psychology Today looks at amphetamine\" class=\"basics-link\" hreflang=\"en\" rel=\"nofollow noopener\" target=\"_blank\">amphetamine<\/a>, cocaine and methamphetamine; <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29886378\/\" rel=\"nofollow noopener\" target=\"_blank\">Bruening et al., 2018<\/a>). <\/p>\n<p>In fact, women in treatment for substance use treatment often report weight-loss as a primary <a href=\"https:\/\/www.psychologytoday.com\/gb\/basics\/motivation\" title=\"Psychology Today looks at motivation\" class=\"basics-link\" hreflang=\"en\" rel=\"nofollow noopener\" target=\"_blank\">motivation<\/a> to start and continue using drugs (<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/23107389\/\" rel=\"nofollow noopener\" target=\"_blank\">Warren et al., 2013<\/a>, <a href=\"https:\/\/www.tandfonline.com\/doi\/abs\/10.1080\/10640266.2026.2680102\" rel=\"nofollow noopener\" target=\"_blank\">2026)<\/a>. In a recent study of 1,533 women in drug treatment centers in Nevada, my colleagues and I found that 12.3 percent had a lifetime eating disorder diagnosis and 21 percent reported symptoms of eating pathology without a formal diagnosis (<a href=\"https:\/\/www.tandfonline.com\/doi\/abs\/10.1080\/10640266.2026.2680102\" rel=\"nofollow noopener\" target=\"_blank\">Warren et al., 2026<\/a>). Additionally, about 44 percent strongly agreed that they started using substances for weight loss purposes, and more than half were concerned about weight gain in recovery (57 percent). Strikingly, women who endorsed using substances for weight loss also reported significantly higher mean levels of body dissatisfaction, binge eating behavior, and general eating pathology than those who did not.<\/p>\n<p>Implications for Drug Treatment<\/p>\n<p>A 2026 advisory by the Substance Use and Mental Health Administration (<a href=\"https:\/\/library.samhsa.gov\/product\/advisory-evidence-based-care-clients-co-occurring-substance-use-disorders-eating-disorders\/pep25-02-010\" rel=\"nofollow noopener\" target=\"_blank\">SAMHSA, 2026<\/a>) described the need for evidence-based care for people struggling with drug use, <a href=\"https:\/\/www.psychologytoday.com\/gb\/basics\/body-image\" title=\"Psychology Today looks at body image\" class=\"basics-link\" hreflang=\"en\" rel=\"nofollow noopener\" target=\"_blank\">body image<\/a> concerns, and eating disorder symptoms. Specifically, assessing how body image and weight concerns relate to substance use, treating overlapping symptoms simultaneously, and offering supplemental care in a supportive, nonjudgmental environment with experts who understand drug <a href=\"https:\/\/www.psychologytoday.com\/gb\/basics\/addiction\" title=\"Psychology Today looks at addiction\" class=\"basics-link\" hreflang=\"en\" rel=\"nofollow noopener\" target=\"_blank\">addiction<\/a> as well as eating pathology is key to recovery success. <\/p>\n<p>One evidence-based program for women in drug treatment is the Healthy Steps to Freedom (HSF; <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/22154034\/\" rel=\"nofollow noopener\" target=\"_blank\">Lindsay et al., 2012<\/a>, <a href=\"https:\/\/www.jneb.org\/article\/S1499-4046(26)00037-0\/pdf\" rel=\"nofollow noopener\" target=\"_blank\">2026<\/a>) program. HSF is a 10- to 12-week supplemental program that provides participants in drug treatment information about weight, <a href=\"https:\/\/www.psychologytoday.com\/gb\/basics\/diet\" title=\"Psychology Today looks at nutrition\" class=\"basics-link\" hreflang=\"en\" rel=\"nofollow noopener\" target=\"_blank\">nutrition<\/a>, physical activity, body image, and eating behavior. In a recent study of 607 adult women in drug treatment (<a href=\"https:\/\/www.jneb.org\/article\/S1499-4046(26)00037-0\/pdf\" rel=\"nofollow noopener\" target=\"_blank\">Lindsay et al., 2026<\/a>), participation in HSF improved healthy eating and physical activity while thin-ideal internalization, body dissatisfaction, and <a href=\"https:\/\/www.psychologytoday.com\/gb\/basics\/eating-disorders\" title=\"Psychology Today looks at disordered eating\" class=\"basics-link\" hreflang=\"en\" rel=\"nofollow noopener\" target=\"_blank\">disordered eating<\/a> symptoms significantly decreased. These findings suggest that programs like HSF can support women in drug treatment to address weight-related concerns related to nutrition, physical activity, body image, and eating pathology as they relate to substance use. <\/p>\n<p>The Naked Truth<\/p>\n<p>In Western cultural contexts, women have been using drugs\u2014both legal and illegal\u2014to lose weight for decades. In fact, research increasingly highlights the link between weight dissatisfaction, disordered eating behavior, and drug use. Given that over-the-counter and prescription drugs are increasingly used to treat obesity in an accessible and culturally sanctioned way, addressing body concerns in a culture that increasingly promotes drug use to manage weight is particularly timely.<\/p>\n<p>For anyone struggling with body image and substance use, the National Eating Disorder Association <a href=\"https:\/\/www.nationaleatingdisorders.org\/\" rel=\"nofollow noopener\" target=\"_blank\">website<\/a> has a host of helpful tools and information on treatment options.
