<\/p>\nA landmark study published in the BMJ has revealed that 574 individuals in England received a tuberculosis diagnosis only after they had already died, spanning the decade from 2010 to 2020.<\/p>\n
While this figure constituted a relatively small proportion \u2014 just 0.8 per cent \u2014 of all TB diagnoses during that period, it represented a far more alarming share of fatalities. <\/p>\n
Close to one in five deaths recorded among TB patients involved cases where the disease had gone entirely undetected during the person’s lifetime.<\/p>\n
The researchers described this pattern as “the ultimate diagnostic delay,” with the findings equating to roughly one such death every week over the 10-year period.<\/p>\n

The study has raised fresh concerns that post-mortem TB diagnoses remain disturbingly common in England.<\/p>\n
These findings arrive amid a worrying upward trend in tuberculosis across England. <\/p>\n
According to the most recent UK Health Security Agency data, the country recorded a rate of 9.4 cases per 100,000 people in 2024 \u2014 the highest level in a decade.<\/p>\n
That figure places England perilously close to the World Health Organisation’s threshold of 10 per 100,000, below which a nation qualifies as a “low incidence country.”<\/p>\n
A key obstacle to timely detection lies in the nature of TB symptoms themselves. <\/p>\n
The disease tends to develop gradually, presenting as a persistent cough lasting beyond three weeks, fatigue, night sweats, appetite loss and weight reduction.<\/p>\n
Tom Wingfield, deputy director of the Centre for Tuberculosis Research and a study author, noted that these early signs are easily overlooked or attributed to other ailments.<\/p>\n
Clinicians may, for instance, assume a patient has a “smoker’s cough” or emphysema, conditions whose symptoms closely mirror those of tuberculosis.<\/p>\n
A separate analysis examining confirmed post-mortem TB cases \u2014 where samples had been taken during the patient’s lifetime but results returned too late \u2014 found that the risk of diagnosis after death increased dramatically with advancing age.<\/p>\n
Men, UK-born individuals, those with documented harmful drug or alcohol use, and patients whose TB had developed outside the lungs were also disproportionately represented.<\/p>\n
The pattern among older British-born patients may reflect childhood exposure to the bacterium during an era when infection was far more prevalent. <\/p>\n
TB can remain dormant within the body for decades, held in check by the immune system in a state known as latent infection.<\/p>\n
However, as immunity declines through ageing, illness or certain medications, the disease can reactivate and become dangerous.<\/p>\n
The World Health Organisation classifies tuberculosis as the leading global killer among diseases caused by a single infectious agent.<\/p>\n
In 2024 alone, an estimated 10.7 million people fell ill with the disease worldwide, with 1.23 million deaths recorded.<\/p>\n
Experts universally emphasise that early detection remains the most critical weapon against TB.<\/p>\n
In Britain, the BCG vaccine \u2014 the most widely administered TB immunisation in the world \u2014 is no longer routinely given to secondary school pupils.<\/p>\n

Instead, the NHS targets it at high-risk young children, including those living in areas with elevated TB rates or with family connections to countries where the disease is more widespread.<\/p>\n
