Here\u2019s what to know about the hurdles."},"content":{"rendered":"<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xrpuk000x29qj7yb067ck@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Connecting people with clinical trials is complicated \u2014 even if people identify a promising match, there\u2019s a slew of potentially thorny factors, including geographic incompatibility, and financial and time considerations.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczo00053b6w2bkj97yy@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Simply finding an appropriate trial can present an enormous hurdle. In a recent <a href=\"https:\/\/clinicaltrials.panfoundation.org\/wp-content\/uploads\/2026\/05\/Clinical-Trials-Patient-Interest-and-Access-2026-National-Polling.pdf\" target=\"_blank\" rel=\"nofollow noopener\">online survey<\/a> of more than 2,000 adults, 71% of patients with chronic conditions said they would be likely to participate in a clinical trial if given the chance. But two-thirds said that their healthcare provider had never discussed clinical trials with them. According to <a href=\"https:\/\/hints.cancer.gov\/docs\/Briefs\/HINTS_Brief_48.pdf\" target=\"_blank\" rel=\"nofollow noopener\">one study<\/a> using data from 2020, just 9% of adults reported ever being invited to participate in a clinical trial.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczo00063b6wo6zdryv4@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Clinical trials are essential to the development of new and effective medical treatments. But gathering the real-world human data necessary to win Food and Drug Administration approval for drugs, devices, and other interventions can be an arduous task. By <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC11721763\/\" target=\"_blank\" rel=\"nofollow noopener\">one estimate<\/a>, up to 86% of clinical trials don\u2019t meet their recruitment targets during the trial time frame.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczo00073b6wkg9n3157@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Getting people signed up isn\u2019t the only challenge.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczo00083b6wtgocv9jn@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cRecruitment is one thing \u2014 retention is another,\u201d said Alan Balch, executive board chair at the Patient Advocate Foundation, which has a <a href=\"https:\/\/trialfinder.panfoundation.org\/en-US\/trials?x_st=1\" target=\"_blank\" rel=\"nofollow noopener\">clinical trial search tool<\/a> and maintains extensive educational materials online. \u201cEvery touchpoint is an opportunity for access and affordability to be a problem.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczo00093b6wfge9qben@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The need to improve patient participation in clinical trials is not a new concern, but it\u2019s attracting new interest.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczo000a3b6wls3ia15k@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In June, the Department of Health and Human Services <a href=\"https:\/\/www.hhs.gov\/press-room\/hhs-launches-clinical-trials-reform-initiative.html\" target=\"_blank\" rel=\"nofollow noopener\">announced an effort<\/a> to streamline and enhance clinical research in the United States. It included a public <a href=\"https:\/\/oig.hhs.gov\/documents\/root\/11720\/OIG_Clinical_Trials_RFI.pdf\" target=\"_blank\" rel=\"nofollow noopener\">request for information<\/a> about whether to modify federal rules that currently deter some trial sponsors from paying clinical trial participants for expenses such as travel and lodging.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczp000b3b6wf3q9pyik@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In July, a group of nearly 200 patient advocacy and public health groups <a href=\"https:\/\/www.fightcancer.org\/sites\/default\/files\/119th_senate_ctma_support_letter_final.pdf\" target=\"_blank\" rel=\"nofollow noopener\">sent a letter<\/a> to the Senate sponsors of the Clinical Trial Modernization Act, urging its passage. The bill would allow trial sponsors to cover trial participants\u2019 medical costs, such as insurance deductibles and copays, and nonmedical expenses like travel and childcare. It would also exclude up to $2,000 in financial support for clinical trial participation from federal taxes, so people wouldn\u2019t risk losing their eligibility for Medicaid or other income-based programs if they signed on.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczp000c3b6whunzkxmc@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            While these efforts to improve clinical trials and patient participation are ongoing, here are answers to some questions about how the system works now and what patients can do if they want to take part.\n    <\/p>\n<p>        Why be a guinea pig? Understand the facts<\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczp000e3b6w6mffhn8a@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In some trials, some participants are given a new drug or therapy that\u2019s being investigated while others receive a placebo with no physical effect.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczp000f3b6wu5iai4cs@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            But there are many <a href=\"https:\/\/clinicaltrials.panfoundation.org\/how-clinical-trials-work\/#types-of-clinical-trials\" target=\"_blank\" rel=\"nofollow noopener\">types of clinical trials<\/a>. Some test different drug combinations, for example. They can test medical devices, preventive measures such as vaccines, or lifestyle changes. Others test ways to screen for or diagnose medical conditions.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczp000g3b6wuqxav1ji@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            For people with very serious illnesses, a clinical trial may offer the best hope for extending their life or improving their quality of life.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczp000h3b6w8dckxsxj@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cCancer is often a fatal disease, and clinical trials offer an opportunity to try something that may or may not be better,\u201d said Mark Fleury, the policy principal for emerging science at the American Cancer Society Cancer Action Network. \u201cIf you know the existing standard of care has an average survival of eight months, you want something with a better opportunity.\u201d\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczp000i3b6w01dgr5on@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In addition, even if patients don\u2019t receive the therapy being tested in the clinical trial, they are monitored closely throughout and receive the gold standard of care, which they might not receive elsewhere, patient advocates said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczp000j3b6w1dtuq0w1@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Some people decide to participate in trials to aid in advancing science.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczp000k3b6wa2972s7a@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Jim Taylor\u2019s wife, Geri, died of Alzheimer\u2019s disease two years ago, more than a decade after her diagnosis in 2012. The couple <a href=\"https:\/\/www.nytimes.com\/interactive\/2016\/05\/01\/nyregion\/living-with-alzheimers.html\" target=\"_blank\" rel=\"nofollow noopener\">became advocates<\/a> for people with the disease, and Taylor is continuing that effort. He\u2019s currently participating in three observational Alzheimer\u2019s trials that are employing cognitive tests and scans to track how his brain is changing compared with the brains of people who\u2019ve been diagnosed with the disease.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczp000l3b6wyotypt2q@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe reason I\u2019ve done it is so I can explain to people, with some authenticness and experience, what a trial is like,\u201d he said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczp000n3b6weoolo4lo@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Despite widespread interest in clinical trial participation, most patients don\u2019t know how to find one.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczp000o3b6wo3pee7k4@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            They can\u2019t necessarily count on their doctors for help. According to an <a href=\"https:\/\/uniting.patientadvocate.org\/news\/survey-reveals-gap-primary-care-provider-support-clinical-trials\/\" target=\"_blank\" rel=\"nofollow noopener\">online survey<\/a> of just over 500 primary care physicians in March, sponsored by the Patient Advocate Foundation, even though 86% of respondents said they were somewhat or very likely to refer their patients to a trial, only 37% had ever done so. When doctors did discuss clinical trials with their patients, it was usually because they had asked about them (67%), they weren\u2019t responding to standard treatment (65%), or their disease was progressing (55%).\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq000p3b6w8dyp48ky@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            But for time-strapped doctors, identifying clinical trials for which patients might be eligible isn\u2019t a simple task. A community oncologist, for example, would typically have to conduct a search using one of the available clinical trial search engines (<a href=\"http:\/\/clinicaltrials.gov\" target=\"_blank\" rel=\"nofollow noopener\">clinicaltrials.gov<\/a> is the most comprehensive), type in all the patient\u2019s characteristics, look at the trials that might be appropriate, and call the site to ask whether the trial is still open, Fleury said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq000q3b6w0daxcrlh@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cAnd if they\u2019re successful, what happens? They lose their patient,\u201d he said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq000r3b6w012d4rxu@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Patients may have an even tougher time searching for trials on their own. Some patient advocacy groups have in-person or online navigators that can help people identify trials they might be eligible for.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq000s3b6wedizya5t@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            The American Cancer Society has a <a href=\"https:\/\/www.cancer.org\/cancer\/making-treatment-decisions\/clinical-trials\/acs-acts.html\" target=\"_blank\" rel=\"nofollow noopener\">clinical trial matching service<\/a>, for example. Organizations such as the <a href=\"https:\/\/www.arthritis.org\/search\/results?query=clinical+trials\" target=\"_blank\" rel=\"nofollow noopener\">Arthritis Foundation<\/a> and the <a href=\"https:\/\/www.nationalmssociety.org\/search#f-nmss_categories=Study%20Listing\" target=\"_blank\" rel=\"nofollow noopener\">National Multiple Sclerosis Society<\/a> have information about disease-specific trials on their websites.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq000t3b6wvbgiuiwx@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            If a hospital or health facility is part of a clinical trial, patients there are often best positioned to enroll. Patients can ask their doctor or the facility for more information.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq000u3b6www73hrri@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cMost recruitment for a trial happens at the site where the trial is happening,\u201d Balch said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq000w3b6wx9m4r2gi@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Much of the clinical research in the U.S. is conducted at large, often urban, academic medical centers. It can be tough for patients to enroll in a trial at a site unless they live nearby or are already being treated there, according to clinical trial experts.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq000x3b6wq3nqvg03@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            To participate in a trial, people generally have to meet periodically with the researchers conducting it. They may also need to get regular blood draws or imaging, or to answer questionnaires to monitor their progress.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq000y3b6wmifph3f5@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThe number one barrier keeping patients out of trials is a lack of onsite clinical trials,\u201d Fleury said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq000z3b6wtxb6w9ri@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            A <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6410951\/\" target=\"_blank\" rel=\"nofollow noopener\">2019 study<\/a> that examined 8,893 cancer patients\u2019 participation in clinical trials found that more than half (55.6%) didn\u2019t have an available trial for their type and stage of cancer at the medical facility where they were being treated. An additional 21.5% didn\u2019t meet the eligibility criteria for an available trial.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq00103b6wkfm7h9yw@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            If a patient identifies a clinical trial at a viable location and wants to be considered, the patient should contact the trial recruiters directly and ask them, Balch said.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq00113b6wxwgf0f5i@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThat\u2019s just the beginning,\u201d he said. Patients also need to find out whether they meet a trial\u2019s eligibility requirements and whether it\u2019s covered by insurance, and to consider how they\u2019re going to pay for any medical or nonmedical costs.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq00123b6wc3ss0k6d@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            Recently there\u2019s been a lot of interest in decentralized access to clinical trials, so patients could do at least some of the trial tasks at home or at their local cancer center, for example.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq00133b6wacq9hxx8@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cIt\u2019s not common yet,\u201d Balch said. But if decentralization grows, he said, it will open up the opportunity to more patients \u2014 and more representative groups of patients.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq00153b6wf7mrw3dy@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            If someone participates in a clinical trial, the trial sponsor picks up the tab for costs stemming directly from the trial, including the drug or device being investigated.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq00163b6wxozsvyhc@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            In addition, under the Affordable Care Act, most commercial health plans are required to cover <a href=\"https:\/\/mypolicyhub.com\/aca-entry\/phsa-section-2709-coverage-for-individuals-participating-in-approved-clinical-trials\/#statute\" target=\"_blank\" rel=\"nofollow noopener\">routine patient costs<\/a> associated with participating in a clinical trial.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczq00173b6wtu2twwin@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            But that doesn\u2019t mean members won\u2019t owe anything. They are generally still responsible for any deductibles, copays, or coinsurance amounts for the routine care that they receive during a clinical trial. And the ACA doesn\u2019t require plans to have out-of-network benefits. That means if a clinical trial is sponsored by a provider that is out of someone\u2019s provider network, the plan might not cover those costs.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczr00183b6wddktl73q@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            <a href=\"https:\/\/www.cms.gov\/medicare\/coverage\/clinicaltrialpolicies\/downloads\/finalnationalcoverage.pdf\" target=\"_blank\" rel=\"nofollow noopener\">Medicare<\/a> and <a href=\"https:\/\/aaci-library.org\/legislation-tracker\/the-clinical-treatment-act\" target=\"_blank\" rel=\"nofollow noopener\">Medicaid<\/a> have similar requirements for coverage of routine clinical trial costs.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczr00193b6wz25wtljp@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            For some patients, incidental expenses can put participation in a clinical trial out of financial reach. Participants may face costs for travel to the trial site, parking, lodging, childcare, or taking time off work.\n    <\/p>\n<p class=\"paragraph-elevate inline-placeholder vossi-paragraph_elevate\" data-uri=\"cms.cnn.com\/_components\/paragraph\/instances\/cmt1xuczr001a3b6wsyh1udpy@published\" data-editable=\"text\" data-component-name=\"paragraph\" data-article-gutter=\"true\">\n            \u201cThere shouldn\u2019t be an added set of concerns and disincentives around costs and financial toxicity,\u201d said <a href=\"https:\/\/www.wscollaborative.com\/\" target=\"_blank\" rel=\"nofollow noopener\">Wendy Selig<\/a>, the founder and CEO of WSCollaborative, a healthcare consultancy.
