but that doesn\u2019t necessarily mean everything is working as it should beneath the surface.<\/p>\n<p class=\"wp-block-paragraph\">In fact, according to a recent <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10369123\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">UK Biobank Cohort study<\/a>, 1 in 15 people within a \u2018normal\u2019 BMI are living with undiagnosed cardiovascular disease or high-risk metabolic phenotypes.<\/p>\n<p class=\"wp-block-paragraph\">Another found that more than half of people with a normal BMI also had at least one metabolic abnormality, despite being within the supposedly idealweight range.<\/p>\n<p class=\"wp-block-paragraph\">Basically, you can\u2019t judge a book by its cover <a data-ico=\"hyperlink-article\" href=\"https:\/\/metro.co.uk\/2026\/08\/23\/7-surprising-signs-show-pretty-healthy-actually-29414607\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">when it comes to health<\/a>, which is why regular check-ups are so important in early diagnosis. <\/p>\n<p class=\"wp-block-paragraph\">Here are the signs to look out for that suggest all may not be as it seems on the outside.<\/p>\n<p>Your blood pressure is high<\/p>\n<p class=\"wp-block-paragraph\">\u2018I regularly see people who are surprised when they are told their blood pressure is high because they exercise, eat reasonably well, have a healthy BMI and feel perfectly normal,\u2019 Dr Suzanne Wylie, GP and medical adviser for <a href=\"https:\/\/www.iqdoctor.co.uk\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">IQdoctor<\/a>, tells Metro.<\/p>\n<p class=\"wp-block-paragraph\">\u2018But blood pressure is not something you can reliably judge from appearance or how you feel.\u2019<\/p>\n<p>\t\t<img width=\"646\" height=\"431\" src=\"https:\/\/www.newsbeep.com\/uk\/wp-content\/uploads\/2026\/09\/GettyImages-1700252021-f18e.jpg\" class=\"article-image Getty Images)<\/p>\n<p class=\"wp-block-paragraph\">While the NHS notes that high blood pressure often causes no symptoms, over time, untreated hypertension can increase the risk of heart disease, stroke, heart failure and kidney disease. <\/p>\n<p class=\"wp-block-paragraph\">That\u2019s why regularly checking is vital, even if you feel just fine.<\/p>\n<p>Your cholesterol is class=\"wp-block-paragraph\">You might associate high cholesterol with an unhealthy diet or being overweight, but neither your appearance nor your fitness level can tell you what your cholesterol levels are doing.<\/p>\n<p class=\"wp-block-paragraph\">\u2018Cholesterol is something we measure rather than something we can identify reliably from how someone looks or feels,\u2019 says Dr Wylie.<\/p>\n<p class=\"wp-block-paragraph\">In particular, raised LDL cholesterol can contribute to fatty deposits building up in the arteries, increasing cardiovascular risk. <\/p>\n<p class=\"wp-block-paragraph\">Dr Wylie adds: \u201dThere are also people with genetically high cholesterol who can be relatively young and outwardly very healthy, which is why I would not regard body weight as a substitute for checking cholesterol, particularly if there is a family history of heart attacks or cardiovascular disease at a younger age.\u2019<\/p>\n<p>You\u2019re constantly exhausted<\/p>\n<p class=\"wp-block-paragraph\">Although Dr Wylie stresses that fatigue is \u2018not specific enough to diagnose anything on its own\u2019, she highlights ongoing exhaustion can be a warning sign for all sorts of issues, from poor sleep and stress to anaemia and thyroid problems.<\/p>\n<p class=\"wp-block-paragraph\">She explains: \u2018If tiredness is persistent, disproportionate to the amount of activity someone is doing or accompanied by symptoms such as increased thirst, passing urine more frequently, breathlessness or changes in weight, it\u2019s worth discussing with a GP.<\/p>\n<p class=\"wp-block-paragraph\">\u2018Sometimes, it\u2019s because the body is signalling a problem before there\u2019s anything visibly wrong.\u2019<\/p>\n<p>\t\t<img width=\"646\" class=\"wp-block-paragraph\">Being a little out of breath after running for a bus is normal, but a noticeable change in how easily you manage everyday activity could be concerning.<\/p>\n<p class=\"wp-block-paragraph\">Dr Wylie says that while there are a number of explanations for a reduction in exercise tolerance, it can sometimes be an early sign of a cardiovascular or respiratory problem.<\/p>\n<p class=\"wp-block-paragraph\">As such, if you suddenly find yourself struggling with stairs or a walk that you previously managed comfortably \u2014 particularly alongside chest discomfort, palpitations, dizziness or swollen ankles \u2014 it shouldn\u2019t automatically be put down to getting older or being out of shape.<\/p>\n<p>Your sleep is consistently poor<\/p>\n<p class=\"wp-block-paragraph\">Loud snoring, repeated waking, morning headaches and feeling groggy despite apparently getting enough sleep can sometimes point towards obstructive sleep apnoea, where breathing repeatedly stops and starts during sleep.<\/p>\n<p>\t\t\t\t\tDo you feel regular health check-ups are necessary even if you look fine?<\/p>\n<p>Yes, to catch hidden issues \u2018have wider effects on health, including its relationship with blood pressure and cardiovascular risk.\u2019 <\/p>\n<p class=\"wp-block-paragraph\">So, regardless of whether you fit the \u2018stereotypical image\u2019 of sleep apnoea, if someone has noticed pauses in your breathing at night or you\u2019re excessively sleepy during the day despite a full night\u2019s sleep, it\u2019s worth discussing with your GP. <\/p>\n<p>You\u2019re suddenly much thirstier or peeing more<\/p>\n<p class=\"wp-block-paragraph\">Changes in how often you need the toilet can be easy to dismiss, particularly if they\u2019re gradual. But repeatedly waking during the night to urinate or becoming much thirstier than usual can sometimes point to diabetes or kidney disease.<\/p>\n<p>\t\t<img width=\"646\" height=\"430\" src=\"https:\/\/www.newsbeep.com\/uk\/wp-content\/uploads\/2026\/09\/GettyImages-2266425042-f848.jpg\" class=\"article-image wp-image-29661468\" alt=\"\" decoding=\"async\" loading=\"lazy\"\/><br \/>\n\t\tNoticed a sudden change in your thirst? (Picture: Getty Images)<\/p>\n<p class=\"wp-block-paragraph\">Again, Dr Wylie stresses that these symptoms \u2018are not diagnostic and there are many less serious explanations,\u2019 says Dr Wylie. <\/p>\n<p class=\"wp-block-paragraph\">However, she adds: \u2018Early kidney disease is especially important here because it can be remarkably quiet; the NHS notes that many people with chronic kidney disease have no symptoms in its early stages and that it is often discovered through routine blood or urine testing rather than because the person feels unwell.\u2019<\/p>\n<p>Your resting heart rate has changed<\/p>\n<p class=\"wp-block-paragraph\">Your heart rate naturally varies depending on exercise, stress, caffeine, illness and medication, so one unusually high reading isn\u2019t necessarily cause for concern.<\/p>\n<p class=\"wp-block-paragraph\">However, repeatedly noticing that your resting pulse is faster than usual \u2014 particularly when you\u2019re relaxed \u2014 is something worth mentioning to a GP, as it can be a signal of abnormal heart rhythm, anaemia or thyroid problems.<\/p>\n<p class=\"wp-block-paragraph\">Dr Wylie advises: \u2018If someone repeatedly finds that their resting pulse is unusually fast, particularly when they are otherwise relaxed, or they experience frequent palpitations, dizziness, fainting or breathlessness, it is sensible to have it assessed.\u2019<\/p>\n<p>Your family history tells a different story<\/p>\n<p class=\"wp-block-paragraph\">Finally, one of the biggest things you can\u2019t see in the mirror is your genetics.<\/p>\n<p class=\"wp-block-paragraph\">\u2018Someone can do everything they are supposed to do, maintain a healthy weight and exercise regularly and still inherit a higher susceptibility to cardiovascular disease, high cholesterol, diabetes or other conditions,\u2019 says Dr Wylie.<\/p>\n<p class=\"wp-block-paragraph\">\u2018A close relative developing cardiovascular disease unusually early, for example, is something I would want to know about when assessing somebody\u2019s overall risk, because it can change how we interpret otherwise reassuring numbers.\u2019<\/p>\n<p class=\"wp-block-paragraph\">Essentially, while looks can be deceiving, as long as you keep your GP abreast of family history, get any concerning symptoms looked at, and attend your scheduled screenings, you\u2019re on the right track. <\/p>\n<p class=\"wp-block-paragraph\">And if you need some extra reassurance and are aged 40 to 74 without certain existing conditions, you can get an <a href=\"https:\/\/www.nhs.uk\/tests-and-treatments\/nhs-health-check\/\" target=\"_blank\" rel=\"noreferrer noopener nofollow\">NHS Health Check<\/a> (covering cardiovascular disease, diabetes, kidney disease and stroke risk, as well as blood pressure, cholesterol, waist circumference) totally free.<\/p>\n<p class=\"has-text-align-center wp-block-paragraph\">Do you have a story you\u2019d like to share?
