Sacramento County health inspectors closed six eateries, including a Sacramento Japanese restaurant after they found a dead mouse and 389 rodent droppings in the facility.

Meanwhile, 19 eateries received yellow placards for health code violations ranging from dying cockroaches to pink slime in the soda gun nozzle.

What do these inspections mean?

While a yellow placard signals two or more major violations. These are typically corrected or mitigated during the inspection, according to the Sacramento County Retail Food Inspection Guide.

A red placard, however, signals “imminent danger to public health and safety” and suspends the health permit until violations are corrected. This could include, but is not limited to, major vermin contamination.

In contrast, a green placard means a restaurant passed inspection. The county performs about 14,000 inspections annually and about 97% of establishments pass, spokesman Ken Casparis previously told The Sacramento Bee.

Approximately 1% of inspections result in a closure, he said.

These were the results of Sacramento County food facility inspections for Thursday, Feb. 19, through Wednesday, Feb. 25, as of Thursday, Feb. 26. If an inspection listed below needs clarification, business owners can email The Sacramento Bee at servicejournalists@sacbee.com.

7245 East Parkway, Sacramento

Date: Thursday, Feb. 19

Sacramento County health inspectors shut down the Parkway international grocery store after discovering 173 rodent droppings throughout the store including in contact with Ghana yam on the retail floor, near boxes of canned goods and by the back door.Health inspectors found a dented can of baby corn and two dented cans of green peas at the retail shelves.The facility’s self-service bags of raw peanuts were found without proper labeling.An old cardboard was used to line shelves inside the walk-in cooler.A gap of more than ¼ inch was observed under the back door.

Red Star International Grocery was reinspected on Friday, Feb. 20 and passed with a green placard. The Sacramento Bee reached out to the grocery store on Thursday, Feb. 26 and the owners declined to comment.

Koreana Plaza Market

10971 Olson Drive, Rancho Cordova

Date: Thursday, Feb. 19

Health inspectors gave a red placard to the Rancho Cordova Korean market after discovering rodent gnaw marks on one bulk package of flour and 32 rodent droppings throughout the market including on top of a bulk Oreo box and below a shelf storing the bulk flour.One carton of eggs was stored directly on one pack of bulk sugar.15 bottles of fresh squeezed orange juice and six bottles of fresh squeezed watermelon juice was stored without proper labels.The facility had old flour and bread crumbs throughout the back food preparation area. An employee sweater was stored above bulk flour and sugar by the handwashing station.Three cases of eggs were stored above proper holding temperature. A cleaning bucket next to the deep fryer had no detectable sanitizer.

Otoro Sushi & Shabu

2131 J St., Sacramento

Health inspectors shut down to the Boulevard Park Japanese restaurant after finding a dead mouse in a trap in the dry storage room, one gnawed bag of rice, a gnawed styrofoam box and gnawed wall material. Inspectors also found 389 rodent droppings throughout the facility, including next to the end of the sushi bar, on canned oyster sauce and underneath the dish machine.Tempura batter was above proper holding temperature.The facility had several holes and openings including around the plumbing above the water heater, around the basement plumbing and in the corner of the office ceiling.

Otoro Sushi & Shabu was reinspected on Monday, Feb. 23 and passed with a green placard. The Bee reached out to the restaurant on Thursday, Feb. 26 but could not reach a representative of the business.

Royal Cuisine

4132 Manzanita Ave., Suite 400, Carmichael

Health inspectors gave a red placard to the Afghan restaurant after finding eight live cockroaches in various stages of life and 10 dead cockroaches.A large covered pot of cooked beans was below proper hot holding temperature.Employees did not demonstrate acceptable food safety knowledge on the proper food storage, proper handwashing and glove use.The facility’s food safety certificate was not available and only one staff member had a food handler card.

Royal Cuisine was reinspected on Wednesday, Feb 25 and passed with a green placard. The Bee reached out to the restaurant on Thursday, Feb. 26 but could not reach a representative of the business.

Vegan Deadly Sins/E Tea

900 15th St., Sacramento

Health inspectors shut down the downtown Sacramento vegan restaurant after finding 39 dead German cockroaches and four living cockroaches in the facility, including on a bottle of bleach and next to the water heater.A butcher knife was wedged between a wall and the prep table. Red chili peppers in a can on the bottom shelf of the prep table were left uncleaned. Two open trays of roasted peanuts were left uncovered and two bags of rice were left open in the employee break room.The facility had old food debris on catch pans and under the wok.Health inspectors found puddling water in the two-door prep cooler.A wiping cloth bucket had no detectable sanitizer. The facility had a leak at the water filter system.

Health inspectors gave a red placard to the Arden Manor sandwich chain after finding 22 German cockroaches in various stages of life throughout the facility including at the three compartment sink and inside a glue trap.Slice tomatoes measured above proper cold holding temperatures.The fountain drink machine had slimy debris inside the ice chute. The facility had boxes of food on the floor in the walk-in freezer.Discarded boxes were found on the floor near the prep table.

Subway had yet to be reinspected as of Thursday, Feb. 26. The Bee reached out to the restaurant on Thursday, Feb. 26 but could not reach a representative of the business.

Pink slime, dead cockroaches and a missing sneeze guardCarnitas El Rincon

2201 Northgate Blvd., Suite C, Sacramento

Sacramento County health inspectors cited the Northgate Mexican restaurant after finding 8 pounds of carnitas, 12 pounds of buche, and 7 pounds of costillas and cuero improperly cooling.Rice, refried beans and carnitas were improperly reheated and below proper hot holding temperatures.The facility did not have a food safety manager certificate or food handler cards available for review.The facility’s prep cooler was in disrepair and not functioning properly.

Carnitas El Rincon was reinspected on Friday, Feb. 20 and passed with a green placard.

Health inspectors cited the Arden Fair Asian eatery after discovering an employee handling raw chicken then steamed rice without proper handwashing with hand soap.Peas, carrots and a bus bin of raw chicken were above proper hot holding temperature. Staff washed utensils, cutting boards and the three-compartment sink without sanitizer. The facility’s certified food manager certificate was not available.

Seoul Express was reinspected on Friday, Feb. 20 and passed with a green placard.

Teriyaki Time

9141 East Stockton Blvd., Suite 260, Elk Grove

Health inspectors cited the Elk Grove Asian eatery after discovering raw chicken measured above proper cold holding temperature.The facility’s California food handler cards were not available for review.A personal drink in an open cup was observed on the metal storage above the prep sink. The automatic paper towel dispenser was only dispensing two inches of paper towel at a time.

Teriyaki Time was reinspected on Monday, Feb. 23 and passed with a green placard.

Taqueria El Burrito

4730 Natomas Blvd., Suite 170, Sacramento

Date: Friday, Feb. 20

Health inspectors cited the Natomas Park restaurant after finding the employees lacked food reheating knowledge.The handwashing sink inside the men’s restroom did not have a cold water supply. The facility’s California food handler cards were not available for review.Two dead cockroaches and one old German cockroach eggsack were found in the facility.

Taqueria El Burrito was reinspected on Monday, Feb. 23 and passed with a green placard.

Mr. Perry’s Coffee Shop

7820 Alta Valley Drive, Sacramento

Health inspectors cited the North Laguna cafe after discovering pancake batter, egg wash, raw liquid egg, cooked sausage and raw beef patties above proper holding temperatures.The facility’s food safety certification could not be located upon request. And the California food handler cards were not available for review.A personal beverage was observed on top of the cutting board in the cook’s line.

Mr. Perry’s Coffee Shop was reinspected on Feb. 25 and received an additional yellow placard due to food found outside of proper holding temperatures. The cafe was reinspected on Thursday, Feb. 26 and passed with a green placard.

Pho Viet 2

1589 West El Camino Ave., Suite 105, Sacramento

Health inspectors cited the South Natomas Vietnamese restaurant after discovering a large strainer of just-cooked noodles blocking the kitchen hand sink.Some of the facility’s food handler cards were missing.The ice machine’s interior panel edge had pink residue. Several small insects were inside and interspersed with rice in the bulk bin.

Pho Viet 2 was reinspected on Wednesday, Feb. 25 and passed with a green placard.

Station 16

9625 East Stockton Blvd., Elk Grove

Date: Monday, Feb. 23

Health inspectors cited the Elk Grove seafood restaurant after discovering black slime accumulation in the ice machine.Pink slime accumulation was observed in the soda gun nozzles at the bar.Unapproved scoops were observed in the bulk containers for rice, Cajun seasoning, sugar and brown sugar.The tallow bin was open, and grease accumulation was present on the ground around the bin and dumpster area.

Station 16 was reinspected on Tuesday, Feb. 24 and passed with a green placard.

Rockstarr Kitchens

4140 Manzanita Ave., Carmichael

Date: Monday, Feb. 23

Health inspectors cited the Carmichael American eatery after finding the facility selling various desserts from an unapproved food source “Cookiesbyuriah.” The seller had a seller’s permit, but lacked a health permit.The laundry restroom handwash station lacked hot water.The ceiling light’s shatterproof cover was loose.The facility had no sanitizer test strips and a wiping bucket had no measurable sanitizer.

Rockstarr Kitchens was reinspected on Wednesday, Feb. 25 and passed with a green placard.

Old Soul at the Weatherstone

812 21st. St., Sacramento

Date: Monday, Feb. 23

Health inspectors cited the Boulevard Park cafe after discovering pesto in a squeeze bottle above proper cold holding temperature.The facility was lacking a copy of a food safety certificate.An employee touched a faucet handle to turn off when handwashing was complete instead of using a paper towel to turn it off. The interior surface of the ice machine had pink slime build up.

Health inspectors cited the Sky Parkway Chinese restaurant after discovering fresh garlic-in-oil left at room temperature without time or temperature control.Frozen meat was left unattended at the prep sink drain board.The facility’s California food handler cards were not available for review.Several personal cups were stored above the equipment in the storage rack.

Fremont Market

2330 N St., Sacramento

Health inspectors cited the Midtown Sacramento market after finding deli sandwiches in a glass door cooler above proper cold holding temperatures.The maximum temperature of water at the three compartment sink was below proper hot water temperatures.The facility’s deli sandwiches lacked labels for sandwiches and macaroni salads.The facility had an “unapproved electrical unit” at the sandwich prep station to cook eggs.

Fremont Market was reinspected on Thursday, Feb. 26 and passed with a green placard.

Cilantro’s Mexican Restaurant

4160 Northgate Blvd., Suite 1, Sacramento

Date: Tuesday, Feb. 24

Health inspectors cited the Northgate Mexican restaurant after finding chicken held below proper hot holding temperature. Refried beans and cheese sauce were also below proper hot holding temperature. Inspectors determined the steam table was not working properly.The facility had no sanitizer available. The facility had overfilled cooked steak improperly cooling in a five-inch depth pan in the walk-in cooler.The self-service chip station was missing a sneeze guard.

Raw pork was stored on top of a shelf of plantain in the two-door freezer.

Health inspectors cited the downtown Sacramento hotel after finding roasted garlic held above proper cold holding temperatures.An employee was observed drinking out of a water bottle with a cap and did not wash their hands before returning to work. An employee was observed using their bare hands to contact ready-eat-garnish.The soda gun holster was unclean.

The Citizen Hotel had yet to be reinspected as of Thursday, Feb. 26.

Matsuyama

10420 Twin Cities Road, Galt

Date: Wednesday, Feb. 25

Health inspectors cited the Galt Japanese restaurant after discovering two of three handwash sinks had no soap and one of the three sinks was blocked with a sanitizer bucket.Unagi was measured below proper hot holding temperature.The facility was missing sanitizer test strips.The facility’s probe thermometer was not available for review.
