The Fresno County Department of Public Health released its 2024 – 2028 Community Health Improvement Plan (CHIP), drawing a strategic roadmap to address health challenges and improve overall wellbeing of Fresno County residents.

The CHIP, which was developed in collaboration with community partners, residents and local organizations, identifies priority areas for action based on findings from the 2023 Community Health Assessment. The CHIP outlines specific goals and strategies guiding public health programs, policies and partnerships through 2028.

Three top priority areas include physical activity and nutrition, equitable access to health care and behavioral health.

The plan aims to reduce obesity and chronic disease by promoting physical activity and access to nutritious foods. Strategies outlined by the CHIP include expanding healthy food options in schools and workplaces, as well as improving neighborhood walkability and safety.

Its second priority, improving behavioral health, seeks to improve access to mental health and substance use services, reduce stigma and strengthen prevention and treatment efforts, with goals including integrating behavioral health into primary care, supporting trauma-informed care and increasing community awareness.

The CHIP’s third goal is to improve equitable access to health care through mobile health services, culturally competent care and improved referral systems.
