At a Board of Supervisors Committee hearing Thursday, PG&E officials were questioned over apparent lapses before a massive fire and blackout at a San Francisco substation last year.
That incident left 130,000 people in the dark on Dec. 20. Last week, an independent review suggested it was sparked by condensation and dust build-up on equipment inside the mission substation, leading to short circuiting.
