{"id":283756,"date":"2026-04-24T13:22:59","date_gmt":"2026-04-24T13:22:59","guid":{"rendered":"https:\/\/www.newsbeep.com\/us-ca\/283756\/"},"modified":"2026-04-24T13:22:59","modified_gmt":"2026-04-24T13:22:59","slug":"s-f-hospital-stabbing-analysis-confirms-reporting-on-security-lapses","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/us-ca\/283756\/","title":{"rendered":"S.F. hospital stabbing analysis confirms reporting on security lapses"},"content":{"rendered":"<p>A 13-page <a href=\"https:\/\/media.api.sf.gov\/documents\/04.21.26_SFDPH_Safety_and_Security_Assessment.cleaned.pdf\" rel=\"nofollow noopener\" target=\"_blank\">assessment<\/a> released today by the San Francisco Department of Public Health confirms Mission Local reporting last month that <a href=\"https:\/\/missionlocal.org\/2026\/03\/sf-public-health-protocol-breakdown-hospital-stabbing\/\" rel=\"nofollow noopener\" target=\"_blank\">protocol failures<\/a> contributed to a social worker\u2019s fatal stabbing in December, and that hospital workers, not a sheriff\u2019s deputy, were <a href=\"https:\/\/missionlocal.org\/2025\/12\/sf-witness-deputy-petting-dog-hospital-stabbing\/\" rel=\"nofollow noopener\" target=\"_blank\">first to intervene<\/a> in the attack.\u00a0<\/p>\n<p>The DPH has hired four additional staff members to its security team to ensure round-the-clock threat-management coverage, and committed an additional $15 million a year to \u201csupport a fundamentally strengthened and modernized approach to safety and security\u201d across its facilities.\u00a0<\/p>\n<p>After a period of increasingly threatening behavior toward his doctor at Zuckerberg San Francisco General Hospital\u2019s Ward 86 HIV clinic, Wilfredo Tortolero Arriechi, 35, arrived on Dec. 4 and was intercepted by his social worker, Alberto Rangel. He stabbed Rangel, 51, to death in the hallway.\u00a0<\/p>\n<p>According to today\u2019s report, the DPH immediately took action after the attack: Installing a weapons detection system at Buildings 80-90 where the attack occurred, launching a 24\/7 threat-management team to triage and respond to concerns and establishing a formal threat-escalation protocol that \u201cbalances safety measures with trauma-informed, patient-centered approaches.\u201d\u00a0\u00a0<\/p>\n<p>The report also identified a need for better processes to respond to emergencies that occur within the DPH system. <\/p>\n<p>Although Rangel was stabbed at Ward 86, a clinic on the grounds of San Francisco General Hospital, and witnesses on the scene called 911 immediately, EMS workers did not arrive to take over Rangel\u2019s care until 11 minutes after his stabbing. <\/p>\n<p>A full 26 minutes elapsed between the 911 call and Rangel\u2019s arrival in the emergency room, only a block away.\u00a0<\/p>\n<p>Today\u2019s report also confirmed Mission Local reporting that a Ward 86 employee first intervened in the attack on Dec. 4 \u2014\u00a0a direct contradiction to claims from the sheriff\u2019s union that a sheriff\u2019s deputy assigned to the site had \u201csaved Ward 86 from a rapid mass casualty stabbing.\u201d\u00a0<\/p>\n<p>The deputy had been assigned to the area that day after Tortolero Arriechi had made threats against his doctor, who worked there. According to today\u2019s report, the doctor was in a different hallway at the time of the stabbing.\u00a0<\/p>\n<p>Hospital staff had repeatedly raised alarm bells with DPH security specifically about Tortolero Arriechi\u2019s threatening behavior, but today\u2019s assessment confirmed that no additional safety measures were taken until the day of the incident.\u00a0<\/p>\n<p>Mission Local reported that Tortolero Arriechi posted increasingly <a href=\"https:\/\/missionlocal.org\/2026\/01\/sf-social-media-hospital-worker-stabbing-suspect\/\" rel=\"nofollow noopener\" target=\"_blank\">erratic messages<\/a> on his social media in the weeks leading up to the stabbing, including a photo of his doctor\u2019s note pinned to a wall with a knife.\u00a0<\/p>\n<p>The DPH assessment includes a timeline, which shows that Tortolero Arriechi had to be escorted out of City Clinic in SoMa as early as Nov. 13 after he appeared seeking out his Ward 86 doctor, who also worked there.\u00a0<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" width=\"361\" height=\"640\" src=\"https:\/\/www.newsbeep.com\/us-ca\/wp-content\/uploads\/2026\/01\/note-stabbing-361x640.jpeg\" alt=\"A stack of official documents with redacted text, an envelope labeled &quot;SI ENTRA SIN PERMIS,&quot; and a kitchen knife placed on top, all resting on a door hinge.\" class=\"wp-image-830958\"  \/>A doctor\u2019s note with a knife through it, posted by Tortolero Arriechi in October 2025. Image from Instagram Threads.<\/p>\n<p>A week later, on Nov. 20 and 21, Tortolero Arriechi exhibited \u201celevated behaviors\u201d at an appointment with the doctor, who reported his behavior to DPH security. The next week, between Nov. 24 and 26, security \u201cattempted multiple times\u201d to reach Tortolero Arriechi by phone, with no success. Security leadership at General Hospital \u201cdiscussed\u201d the case, but apparently took no further action.\u00a0<\/p>\n<p>On Dec. 4, the morning of the stabbing, Tortolero Arriechi went to both the City Clinic and Ward 86.\u00a0<\/p>\n<p>The doctor again reported to security that Tortolero Arriechi was seeking him out at City Clinic, and that Tortolero Arriechi had allegedly insisted that he would return daily until he could see the doctor. According to the report, DPH security then assigned a \u201csafety ambassador\u201d to the clinic.\u00a0<\/p>\n<p>That same morning at Ward 86, staff contacted DPH head of security, Basil Price, and informed him that Tortolero Arriechi had once again shown up at the clinic looking for the doctor, and told them that he would be returning that afternoon.\u00a0<\/p>\n<p>The DPH requested a \u201ccriminal history check\u201d by the sheriff\u2019s department that day, which surfaced no warrants for Tortolero Arriechi. After a sheriff\u2019s lieutenant conducted a \u201cthreat assessment\u201d on the situation, the sheriff\u2019s department assigned a deputy to be \u201cposted at Ward 86.\u201d <\/p>\n<p>Staff at Ward 86 interviewed by Mission Local were under the <a href=\"https:\/\/missionlocal.org\/2025\/12\/sf-hospital-killing-timeline\/\" rel=\"nofollow noopener\" target=\"_blank\">impression<\/a> that the deputy was keeping an eye out for Tortolero Arriechi, but the DPH report confirms the sheriff\u2019s department\u2019s assertion that the deputy was directed only to station near the specific physician that Tortolero Arriechi had threatened.\u00a0<\/p>\n<p>Later in the afternoon of Dec. 4, 2025, Tortolero Arriechi again went to Ward 86 looking for the doctor, where he was directed to speak with his social worker, Rangel. <\/p>\n<p>Moments later, Tortolero Arriechi stabbed Rangel, who later died, despite efforts by his colleagues to resuscitate him.\u00a0<\/p>\n<p>Tortolero Arriechi is currently facing murder charges, and his public defender has said that he was suffering a <a href=\"https:\/\/missionlocal.org\/2026\/01\/sf-hospital-stabbing-pleads-not-guilty\/\" rel=\"nofollow noopener\" target=\"_blank\">mental health crisis<\/a>.\u00a0<\/p>\n<p>\u201cNo actions can undo the events of Dec. 4, 2025,\u201d the report said. \u201cHowever, through an expertly informed re-evaluation of our current safety and security measures, we can ensure an improved approach to workplace safety and security going forward.\u201d\u00a0<\/p>\n<p>Ward 86 employee Alex Alvarez said he was frustrated at the lack of funding for mental-health care and support for traumatized employees who have not yet returned to work.\u00a0<\/p>\n<p>Due to the lack of protocols in place, he said, \u201cwe have to create this whole ecosystem of services, of safety protocols \u2026 why do the employees have to pay for this?
