For nearly 110 years, Lee Health has been our community’s trusted healthcare destination. We have grown and evolved to provide leading-edge services and programs—investing in modern facilities and lifesaving technology and collaborating with local community partners to improve the overall health of Southwest Florida residents. Throughout our history, we have remained focused on our mission and steadfast in our commitment to serving as the safety-net health system for Lee County.

When Lee Health converted from a public entity to a private, community-focused nonprofit health system, we made a commitment to continue providing care and fulfilling our safety-net mission for Lee County residents in perpetuity. To ensure we are meeting the needs of our communities, we must listen and learn—and the Community Health Needs Assessment Survey is one of the most important ways we do that.

The survey helps identify health disparities, barriers to care, and priority areas for action. Results guide Lee Health and our community partners in how we invest resources, design programs, and deliver services that make a meaningful difference in the communities we serve.

We need your feedback and insights to help shape how local healthcare needs are addressed. We’re asking Southwest Florida residents ages 18 and older to take about 10 minutes to complete the Community Health Needs Assessment Survey.
