FORT MYERS, Fla.—Lee Health has launched a community health survey targeting residents of Southwest Florida to improve care and address patient needs in Charlotte, Collier, Glades, Hendry, and Lee counties.

The survey is open to everyone in the community, including non-patients, staff, local businesses, and the public. Lee Health is sharing the survey link through community outreach, asking local media for support, and using social media to reach as many community members as possible.

Under the 2010 Patient Protection and Affordable Care Act, not-for-profit hospitals must conduct Community Health Needs Assessments every three years to identify the primary health needs and challenges of the communities they serve.
