STUART, Fla. (CBS12) \u2014 Tiger Woods <\/a>has been formally ordered to appear in Martin County court next month in connection with misdemeanor charges stemming from a suspected DUI crash<\/a>, according to a notice filed this week by the county clerk.<\/p>\n

Court records show Eldrick Tont “Tiger” Woods is scheduled to appear at 8:30 a.m. May 5 at the Martin County Courthouse in Stuart.<\/p>\n

The notice, issued April 6, lists two charges:<\/p>\n

Driving under the influence causing damage to property or a personRefusal to submit to a breath, blood or urine test<\/a><\/p>\n

The notice to appear is the most recent filing in the case, which remains pending in the county court. Woods\u2019 attorney, Jason F. Fagnano of Jupiter, is listed as counsel of record.<\/p>\n

The charges stem from a March 27 rollover crash on South Beach Road on Jupiter Island. Investigators say Woods attempted to pass a pickup truck towing a trailer, crossed over double solid lines, and struck the trailer, causing his SUV to flip onto its side. No serious injuries were reported.<\/p>\n

Earlier this month, the Martin County Sheriff\u2019s Office released body-worn camera footage<\/a> from deputies who responded to the crash, offering additional detail about the moments after impact and the subsequent DUI investigation. In the footage, Woods tells deputies he was looking down at his phone and adjusting the radio when the crash occurred. <\/p>\n

\u201cI looked down at my phone and all of a sudden, boom,\u201d he can be heard saying.<\/p>\n

According to a probable cause affidavit <\/a>referenced alongside the video, deputies reported signs of impairment, including bloodshot and glassy eyes, dilated pupils, and lethargic movements. Two pills later identified as hydrocodone were found loose in Woods\u2019 pocket during a search, the affidavit states. Woods told deputies he had not consumed alcohol and cited previous back and leg surgeries while attempting field sobriety exercises.<\/p>\n

Woods pleaded not guilty <\/a>to the charges and waived his arraignment. He remains free on bond. A judge has also granted Woods permission to travel out of the country<\/a> for inpatient treatment while the case proceeds, provided he returns for future court appearances as ordered.<\/p>\n
