On September 27, 2026, at approximately 3:57 p.m., State Police responded to County Route 46 in the town of Le Ray for a two-vehicle crash.
A preliminary investigation determined that a 2002 Honda CR-V, operated by Nathan H. Fifield, age 32, of Gouverneur, was traveling south on County Route 46 when it attempted to pass an uninvolved vehicle and collided head on with a 2023 Buick Encore traveling north operated by a Gary A. Neilsen, age 65, of Theresa.
The Honda was occupied by a passenger, Cassandra M. Thomas, age 31, of Deferiet.
All three occupants were transported to Samaritan Medical Center.
Cassandra M. Thomas, was later pronounced deceased at the hospital.
Both drivers have serious, non-life-threatening injuries.
The New York State Police Collision Reconstruction Unit responded and is assisting with the investigation.
The investigation remains ongoing.
