Former child actor Blake Garrett, best known for his role in the 2006 film “How to Eat Fried Worms,” has died, according to a report from TMZ.

Blake Garrett, 33, died on Sunday, Feb. 8, the outlet says.

The former child actor’s mother, Carol Garrett, confirmed his death, adding that it came after a recent emergency room visit and shingles diagnosis, according to TMZ.

While Carol Garrett said that her family would wait for a medical examiner’s report, she told the outlet that her son’s death could have been an accident.

Blake Garrett had been living in Tulsa, Oklahoma, and had gotten sober, his mother said to TMZ.

Born in Austin, Texas, Blake Garrett appeared in two productions as a child actor, according to the outlet.

In 2004, he was featured in the special “Barney’s Colorful World, Live!” before playing the character Plug from “How to Eat Fried Worms” in 2006.
