{"id":203749,"date":"2026-04-20T20:07:43","date_gmt":"2026-04-20T20:07:43","guid":{"rendered":"https:\/\/www.newsbeep.com\/us-ny\/203749\/"},"modified":"2026-04-20T20:07:43","modified_gmt":"2026-04-20T20:07:43","slug":"opinion-the-trump-administration-is-coming-after-birth-control-access-in-a-terrifying-new-way","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/us-ny\/203749\/","title":{"rendered":"Opinion | The Trump Administration Is Coming After Birth Control Access in a Terrifying New Way"},"content":{"rendered":"<p class=\"css-ac37hb evys1bk0\">Some 60 years ago, American legislators set out to tackle a problem that was driving employment and education rates down, driving health care and welfare costs up and making American family life significantly less stable: Many American women, and particularly poor women and teenagers, were having more children than they wanted or could afford. Close to <a class=\"css-yywogo\" href=\"https:\/\/jenni.uchicago.edu\/Fertility\/Hayford_Benjamin-Guzzo_2016_PopDevRev_v42_n1.pdf\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">half<\/a> of births were to women who had not intended to get pregnant.<\/p>\n<p class=\"css-ac37hb evys1bk0\">Decreasing the unintended pregnancy rate was a bipartisan wish. In 1969, President Richard Nixon <a class=\"css-yywogo\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC4206087\/\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">recognized<\/a> that \u201cunwanted or untimely childbearing is one of several forces which are driving many families into poverty.\u201d A year later, Congress passed Title X: the first federal program entirely dedicated to family planning and reproductive health care.<\/p>\n<p class=\"css-ac37hb evys1bk0\">It would go on to become one of the most successful federal programs of the last century, with <a class=\"css-yywogo\" href=\"https:\/\/www.guttmacher.org\/gpr\/2001\/02\/title-x-three-decades-accomplishment\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">one study<\/a> finding it prevented some 20 million unintended pregnancies in just 20 of its 50 years by providing women with free and low-cost birth control. It has significantly reduced child poverty. In 1957, nearly one in 10 teenage girls gave birth. Today, the rate is closer to <a class=\"css-yywogo\" href=\"https:\/\/www.cdc.gov\/nchs\/data\/vsrr\/vsrr043.pdf\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">one in 100<\/a>. For every dollar spent on family planning funds, the government saves $7 in Medicaid costs.<\/p>\n<p class=\"css-ac37hb evys1bk0\">But President Trump seems intent on killing Title X. This month, the Department of Health and Human Services quietly issued new <a class=\"css-yywogo\" href=\"https:\/\/files.simpler.grants.gov\/opportunities\/770eae58-b245-4431-a4b8-7b1aca9e917f\/attachments\/5e3ac609-8998-466a-a8b6-c3d7d49a2e6c\/2027_Title_X_Services_NOFO_PA-FPH-27-001_PDF.pdf\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">funding guidelines<\/a> that have effectively subverted the program\u2019s entire purpose. Instead of getting highly effective contraception methods to the country\u2019s poorest women so that they may decide if and when to have children, Title X under Mr. Trump seems aimed at getting more women pregnant, whether they want to be or not. And it appears to cater to three influential parts of the Trump coalition: The anti-abortion movement; the MAHA, or Make America Healthy Again, movement; and pronatalists who want to see birthrates rise at nearly any cost.<\/p>\n<p class=\"css-ac37hb evys1bk0\">More than half of patients at Title X clinics use modern contraceptive methods to prevent pregnancy. But the word \u201ccontraception\u201d comes up just once in the Title X funding document, and only in a section on \u201creducing overmedicalization in health care.\u201d Instead, in a change pulled directly from <a class=\"css-yywogo\" href=\"https:\/\/static.heritage.org\/project2025\/2025_MandateForLeadership_FULL.pdf\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">Project 2025<\/a>, H.H.S. tells Title X clinics to emphasize \u201cfertility-awareness-based methods,\u201d a broad category that includes things like tracking your periods or your body temperature to estimate which days you might be fertile. These methods can be <a class=\"css-yywogo\" href=\"https:\/\/www.ox.ac.uk\/news\/2023-09-15-when-it-comes-starting-family-timing-everything\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">helpful<\/a> for getting pregnant, but are generally <a class=\"css-yywogo\" href=\"https:\/\/www.acog.org\/-\/media\/project\/acog\/acogorg\/womens-health\/files\/infographics\/effectiveness-of-birth-control-methods.pdf?rev=45f328a5217f44888270ca793cc5195c&amp;hash=AFA8F75E09303C80FD71AAE766524C03\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">far less so<\/a> for preventing pregnancy. Fertility awareness methods have typical-use <a class=\"css-yywogo\" href=\"https:\/\/www.acog.org\/womens-health\/faqs\/fertility-awareness-based-methods-of-family-planning\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">failure rates<\/a> between 12 and 24 percent in the first year, according to the American College of Obstetricians and Gynecologists. The intrauterine device, by contrast, has a failure rate of less than 1 percent.<\/p>\n<p class=\"css-ac37hb evys1bk0\">The health department seems to want to shift taxpayer dollars away from reliable contraception and toward counseling men on erectile dysfunction, testosterone levels and sperm motility, each of which merits three mentions in the new guidance, while IUDs and birth control pills earn none. The document is a mishmash of Make America Healthy Again talking points on lifestyle changes, conservative bromides on marriage before babies and pronatalist nods to fertility.<\/p>\n<p class=\"css-ac37hb evys1bk0\">Some of the guidance sounds sensible on its face. H.H.S. cribs from MAHA when it says it will focus on chronic disease in order to promote \u201chealthy pregnancies and family formation.\u201d But contraception use is a significant part of how women ensure they have healthy pregnancies and form the families they desire, and it\u2019s also a common treatment for chronic diseases, <a class=\"css-yywogo\" href=\"https:\/\/mcpress.mayoclinic.org\/women-health\/beyond-contraception-other-uses-for-birth-control\/\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">including<\/a> endometriosis and polycystic ovarian syndrome. Yet it doesn\u2019t come up in the section spelling out the department\u2019s top Title X spending priorities. What does? Addressing \u201cexposure to harmful chemical and environmental toxins,\u201d low sperm count, and pornography use.<\/p>\n<p class=\"css-ac37hb evys1bk0\">The new Title X guidance also includes many mentions of infertility. It\u2019s true that men and women desire more support in having wanted pregnancies, but H.H.S.\u2019s prescription, which includes \u201csleep\u201d and counseling on \u201cmarriage prior to childbearing,\u201d is unsatisfying. On the campaign trail, Mr. Trump <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2024\/10\/16\/us\/politics\/trump-ivf-abortion.html\" title=\"\" rel=\"nofollow noopener\" target=\"_blank\">declared<\/a> himself the \u201cfather\u201d of in vitro fertilization, yet I.V.F. is absent from his administration\u2019s family planning funding goals.<\/p>\n<p class=\"css-ac37hb evys1bk0\">The good news is that, at least for now, Title X funds are still <a class=\"css-yywogo\" href=\"https:\/\/www.federalregister.gov\/documents\/2021\/04\/15\/2021-07762\/ensuring-access-to-equitable-affordable-client-centered-quality-family-planning-services\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">required<\/a> to go to clinics that provide or refer out for a range of modern contraceptive options. The bad news is that this is only because of a Biden-era rule that <a class=\"css-yywogo\" href=\"https:\/\/www.kff.org\/quick-take\/navigating-uncertainty-the-latest-challenge-to-the-title-x-family-planning-safety-net\/\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">the administration<\/a> has already signaled it might try to rescind. The threat to the program is all the more concerning because, according to the most recent data available, it <a class=\"css-yywogo\" href=\"https:\/\/www.kff.org\/quick-take\/the-federal-family-planning-program-has-not-recovered-from-trump-era-restrictions\/\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">still hasn\u2019t fully recovered<\/a> from draconian regulations put in place during Mr. Trump\u2019s first term, which led to an exodus of clinics and cut the number of patients served by half.<\/p>\n<p class=\"css-ac37hb evys1bk0\">An H.H.S. employee <a class=\"css-yywogo\" href=\"https:\/\/www.politico.com\/news\/2026\/04\/03\/trump-admin-moves-title-x-family-planning-program-away-from-contraception-towards-conception-00858913\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">told Politico<\/a> that the new guidance was catering to the anti-abortion wing of the G.O.P. There\u2019s a terrible irony here \u2014 by reducing unintended pregnancy, Title X has prevented more than 9 million abortions \u2014 but it\u2019s not surprising: Most of the major \u201cpro-life\u201d groups in the United States either oppose contraception or stay mum on the topic. The old anti-abortion movement strategy was to attack contraception as immoral, though few Americans share that view. The <a class=\"css-yywogo\" href=\"https:\/\/www.nytimes.com\/2025\/11\/09\/us\/politics\/maha-natural-family-planning.html\" title=\"\" rel=\"nofollow noopener\" target=\"_blank\">new tactic<\/a> is more MAHA-coded, and with a pronatalist twist: Sow fear that modern contraceptives are unnatural, and push holistic alternatives instead; generate alarm about declining birthrates and blame the dip on working women (in reality, it largely comes from fewer teen pregnancies).<\/p>\n<p class=\"css-ac37hb evys1bk0\">Women who are able to plan their pregnancies wind up in better <a class=\"css-yywogo\" href=\"https:\/\/medicalxpress.com\/news\/2024-11-health-decline-childbirth-greater-women.html\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">physical<\/a> and <a class=\"css-yywogo\" href=\"https:\/\/odphp.health.gov\/healthypeople\/objectives-and-data\/browse-objectives\/family-planning\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">psychological<\/a> health, birth <a class=\"css-yywogo\" href=\"https:\/\/odphp.health.gov\/healthypeople\/objectives-and-data\/browse-objectives\/family-planning\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">healthier<\/a> <a class=\"css-yywogo\" href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10759294\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">infants<\/a>, make more <a class=\"css-yywogo\" href=\"https:\/\/www.guttmacher.org\/sites\/default\/files\/report_pdf\/social-economic-benefits.pdf\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">money<\/a>, are less likely to get <a class=\"css-yywogo\" href=\"https:\/\/www.guttmacher.org\/sites\/default\/files\/report_pdf\/social-economic-benefits.pdf\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">divorced<\/a>, are less likely to rely on public assistance and <a class=\"css-yywogo\" href=\"https:\/\/www.guttmacher.org\/sites\/default\/files\/report_pdf\/social-economic-benefits.pdf\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">invest<\/a> more in their children, who, in turn, do better educationally and behaviorally. Modern contraception is nothing short of a medical miracle \u2014 one that has saved the lives of millions of <a class=\"css-yywogo\" href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/22784531\/\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">women<\/a> and <a class=\"css-yywogo\" href=\"https:\/\/obgyn.onlinelibrary.wiley.com\/doi\/10.1002\/ijgo.15127\" title=\"\" rel=\"noopener noreferrer nofollow\" target=\"_blank\">babies<\/a> worldwide.<\/p>\n<p class=\"css-ac37hb evys1bk0\">Not satisfied with the end of legal abortion in America, the anti-abortion movement seems poised to end the era of affordable contraception. The result isn\u2019t just the end of Title X as we knew it.
