Meanwhile, the conductor and the train operator have been removed from service with the investigation underway.

MTA NYC Transit President Demetrius Crichlow said the agency examined the train itself and found it was functioning properly, including the doors. What remains unclear is whether the conductor — who had two and a half years on the job — followed proper protocol.

“The conductor has the responsibility of observing the platform. They observe as they are departing the station. Whether or not that was done is under investigation,” said Crichlow, who insisted the incident that has chilled riders is “a very, very rare instance.”

The man, identified by police as Yaw Boahene, died at the scene of Monday’s rush-hour accident as a Manhattan-bound A train was leaving the Utica Avenue station. According to investigators, Boahene either stumbled or got stuck as he was exiting the train car around 5:30 p.m.

Witnesses say he had been holding both a cane and a blue plastic bag before he got dragged by the train. The man might have refused to let the plastic bag go, a law enforcement source previously told NBC New York, adding that video shows him appearing to still clutch it as the train drags him down the platform and into a wall.

Investigators are also looking into the possibility that the bag was caught around Boahene’s wrist and he couldn’t break free, the source says.

The 57-year-old fell and rolled onto the tracks after he hit the wall, but the source says the train had already left the station and he wasn’t struck.

In addition to the bag possibly stuck in the door, a law enforcement source said investigators are also looking into whether the victim’s foot was stuck between the platform and the train car as it pulled away from the station. Detectives said it could be a possibility, given how mangled the man’s leg and foot were, the source told NBC New York.

MTA Chairman Janno Lieber said it was not clear if the victim stumbled and fell, or if the bag got snagged somehow.

“There was nothing attached to this man — he was carrying a blue plastic bag. May have gotten entangled but so far we don’t have any proof of that,” said Lieber.

Others on the train did not appear to react or notice what was happening as the train pulled away, according to the source, because Boahene had fallen and was dragged below the line of sight of the riders.

Police have not released video from inside the train station.

It wasn’t clear if subway door sensors would have sensed such a thin bag. The investigation is ongoing. It was not clear when the NTSB might have a preliminary report on the incident.

Federal investigators are now looking into the the horrifying death of a man who was dragged by a…

[36
98,100,99,141,9,24,63,849
post-361712","post","type-post","status-publish","format-standard","has-post-thumbnail","category-brooklyn","tag-brooklyn","tag-brooklyn-headlines","tag-brooklyn-news","tag-mta","tag-new-york","tag-new-york-city","tag-nyc","tag-nyc-subway
["GET
