Observed the walk-in cooler shelving to be rusting and to have an apparent mold build up. Observed multiple areas throughout the facility where spray foam has been sued to make repairs. This is not an easily cleanable or non-porous surface. Observed the door handle on the walk-in cooler to be broken. The floor / wall juncture in multiple area is not covered and closed to 1/32 inch. Area where the is coving the material is peeling away. Floors throughout the facility are made of tiles that are cracked, roughened, and are not a smooth, easily cleanable surface. Walk-in cooler floor is made of metal and it broken and buckling. Food facility is not removing dead or trapped pests from control devices frequently, as evidenced by observed old / decaying mouse in pest control device in the front milkshake area. Corrected. Ceiling tiles in the back area by the exit door are damaged and need replaced. Wall in multiple area throughout the facility has holes, or is broken and in need of repair. Food facility inspection indicates evidence of rodent activity throughout the kitchen area. Facility has a pest control service. Grease trap area of the food facility is extremely dirty, dusty, and in need of cleaning.
