The Northwest Ohio Pathways HUB has recognized Kelsey Strickfaden of the Erie County Health Department for her outstanding contributions to community health and patient advocacy, according to a news release.

During a formal presentation, Strickfaden received a Certificate of Recognition for enrolling 76 funded clients in 2025, the highest number of enrollments for the year, the release said.

The Northwest Ohio Pathways HUB, managed by the Hospital Council of Northwest Ohio, is a regional care coordination system designed to connect at-risk and low-income residents with vital medical and social services.

By utilizing Community Health Workers, the program addresses social determinants of health such as housing, transportation and food security to ensure that all community members can achieve optimal health outcomes, according to the release.

“We are incredibly proud of Kelsey’s dedication to the residents of Erie County,” said Nicole Ziegler, chief health services officer for the Erie County Health Department, in the release. “Enrolling 76 clients in the Pathways HUB, represents 76 individuals and families who now have a direct line to the resources they need to thrive.

“As a Gold Award recipient from the Health Resources Services Administration, (the Erie County Health Department/Erie County Community Health Center) is the premiere provider in the area, and Kelsey’s commitment to excellence is a perfect reflection of the high standard of care we provide to our community.”

Strickfaden’s achievement highlights the growing impact of the HUB model in Erie County, where care coordinators work one-on-one with clients to navigate complex systems and remove barriers to care, the release said.

Her record-breaking enrollment numbers underscore the high demand for these services and the effectiveness of the Erie County Health Department’s outreach efforts, according to the release.
