Media Advisory: Notice of Data Incident

Chicago, IL – Erie Family Health Centers (“Erie”) is a Federally Qualified Health Center (FQHC) that provides healthcare services to patients in the greater Chicago area.

On January 27, 2026, Erie learned of potentially unauthorized access to certain systems on its network. Upon discovery, Erie took immediate action to investigate, remediate, and respond to the incident, which included the assistance of outside specialists. The investigation determined that Erie’s network was subject to unauthorized access between December 10, 2025 and January 27, 2026. The information contained within the affected data varies by individual but may have included a patient name, address, phone number, email address, Social Security number, driver’s license/state ID number, taxpayer ID number, financial account information, payment card information, date of birth, medical treatment or diagnosis information, prescription information, date of service, patient ID number, encounter ID number, provider name, patient account number, medical record number, Medicare/Medicaid number, health insurance information, and/or treatment cost information. Erie continued to provide patient care throughout the duration of the incident, and access to Erie’s cloud-based electronic health record system was not impacted.

Erie takes its obligations with respect to the privacy and security of information seriously. Erie is undertaking a thorough review of the relevant information to identify potentially affected individuals. Erie is also mailing letters to potentially affected individuals for whom it has address information, and Erie posted notice to its website. Erie has also implemented additional security measures to help prevent a similar occurrence in the future.

Erie has established a toll-free call center to answer questions about the incident, provide access to complimentary credit monitoring services, and address related concerns. The call center is available between the hours of 7:00 AM and 7:00 PM Central Time, Monday through Friday, excluding major U.S. holidays, and can be reached at 833-297-4842. Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports, account statements, and explanation of benefits forms for suspicious activity and to detect errors. Individuals may also place a fraud alert or credit freeze by contacting the credit reporting agencies: TransUnion at 1-800-680-7289; Experian at 1-888-397-3742; Equifax at 1-888-298-0045. Instances of known or suspected identity theft should also be reported to law enforcement, the applicable state Attorney General, and Federal Trade Commission.
