A new randomized clinical trial led by researchers at the University of Pittsburgh and collaborating institutions found that adults recovering from mild traumatic brain injury (mild TBI or concussion) experienced similar overall symptom improvement whether they received targeted, individualized therapies or standard behavioral management care.\u00a0<\/p>\n

The study followed 162 adults, ages 18 to 49, treated at specialty concussion clinics and randomly assigned to either a targeted multidomain (T\u2011MD) treatment program or a standard behavioral management approach. Participants were followed for four weeks, with\u00a0additional\u00a0evaluation at three months.\u00a0<\/p>\n

\u201cWhat this study shows is that evidence\u2011based behavioral management remains a very effective foundation for concussion care,\u201d said Anthony P. Kontos, Ph.D., research director of the UPMC Sports Medicine Concussion Program and corresponding author of the study. \u201cAt the same time, targeted therapies\u00a0provided\u00a0added benefit for specific symptoms such as,\u00a0vestibular (movement, balance),\u00a0vision, and cognitive\u00a0problems.\u201d\u00a0<\/p>\n

Why the Findings Matter\u00a0<\/p>\n

Concussion\u00a0affects more than 3 million Americans each year, yet many patients do not receive follow\u2011up care beyond their\u00a0initial\u00a0diagnosis. Clinicians have increasingly moved toward earlier and more active treatments, both of which are supported by research, but questions\u00a0remain\u00a0about\u00a0when and\u00a0how\u00a0individualized\u00a0concussion\u00a0care needs to be.\u00a0<\/p>\n

\u201cThese results\u00a0can\u00a0help clinicians make\u00a0informed\u00a0decisions\u00a0about\u00a0concussion\u00a0treatment,\u201d Kontos said. \u201cThese\u00a0new findings\u00a0show\u00a0that\u00a0behavioral management works well for\u00a0all\u00a0patients,\u00a0but\u00a0also highlight\u00a0the benefits of\u00a0targeted therapies\u00a0for patients with specific symptoms or clinical profiles.\u201d\u00a0<\/p>\n

Implications for Patient Care\u00a0<\/p>\n

The findings support a flexible, evidence\u2011based approach to concussion\u00a0care.\u00a0Behavioral management\u2014including\u00a0guided physical activity, sleep regulation, hydration,\u00a0nutrition\u00a0and stress management\u2014remain effective for improving overall symptoms.\u00a0However, targeted interventions may offer\u00a0additional\u00a0benefits\u00a0for patients with\u00a0vision,\u00a0vestibular\u00a0and\u00a0cognitive\u00a0profiles.\u00a0<\/p>\n

\u201cOur goal\u00a0as clinicians\u00a0is to match the right treatments\u00a0to the right patient\u00a0and to do\u00a0so\u00a0in a timely fashion,\u201d\u00a0said\u00a0Micky Collins, Ph.D., executive and clinical director of\u00a0the UPMC Sports Medicine Concussion Program. \u201cThis study provides important data to\u00a0help\u00a0guide that process\u00a0to ensure that our\u00a0patients receive the most effective\u00a0concussion\u00a0care possible.\u201d<\/p>\n
