{"id":165647,"date":"2026-04-14T18:30:15","date_gmt":"2026-04-14T18:30:15","guid":{"rendered":"https:\/\/www.newsbeep.com\/us-pa\/165647\/"},"modified":"2026-04-14T18:30:15","modified_gmt":"2026-04-14T18:30:15","slug":"why-rural-hospitals-in-pennsylvania-and-across-the-country-are-closing-in-increasing-numbers-5-myths-about-rural-health-care","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/us-pa\/165647\/","title":{"rendered":"Why rural hospitals in Pennsylvania and across the country are closing in increasing numbers \u2013 5 myths about rural health care"},"content":{"rendered":"<p>Some Pennsylvania hospitals are being pushed beyond the brink of closure.<\/p>\n<p>Taylor Hospital in Ridley Park <a href=\"https:\/\/6abc.com\/post\/shuttered-taylor-hospital-delaware-county-may-soon-reopen\/17775379\" rel=\"nofollow noopener\" target=\"_blank\">closed in April 2025<\/a>, Crozer-Chester Medical Center in Upland <a href=\"https:\/\/6abc.com\/post\/crozer-chester-medical-center-officially-closing-friday\/16301001\/\" rel=\"nofollow noopener\" target=\"_blank\">closed in May 2025<\/a>, and Heritage Valley Kennedy Hospital, formerly the Ohio Valley Hospital, in Kennedy Township <a href=\"https:\/\/www.post-gazette.com\/business\/healthcare-business\/2025\/04\/03\/hospital-closing-losses-pittsburgh-heritage-valley\/stories\/202504030066\" rel=\"nofollow noopener\" target=\"_blank\">closed in June 2025<\/a>.<\/p>\n<p>Rural hospitals are particularly vulnerable.<\/p>\n<p>The UNC Sheps Center, which tracks rural hospital infrastructure in the United States, has documented <a href=\"https:\/\/www.shepscenter.unc.edu\/programs-projects\/rural-health\/rural-hospital-closures\/\" rel=\"nofollow noopener\" target=\"_blank\">195 rural hospitals<\/a> that have closed or converted to outpatient facilities since January 2005. Six are in Pennsylvania. Closures have far <a href=\"https:\/\/www.kff.org\/health-costs\/key-facts-about-hospitals\/?entry=rural-hospitals-rural-openings-and-closings\" rel=\"nofollow noopener\" target=\"_blank\">outpaced the opening of new rural hospitals<\/a> during this period. <\/p>\n<p>As a <a href=\"https:\/\/scholar.google.com\/citations?user=T8OoOyYAAAAJ&amp;hl=en&amp;oi=ao\" rel=\"nofollow noopener\" target=\"_blank\">physician who has trained in rural communities<\/a> and a <a href=\"https:\/\/scholar.google.com\/citations?user=nCCYwEoAAAAJ&amp;hl=en&amp;oi=sra\" rel=\"nofollow noopener\" target=\"_blank\">researcher who studies community well-being and public health<\/a>, we see that every rural hospital closure exerts a domino effect on surrounding communities and residents. This can be difficult to quantify but manifests as lost jobs and economic decline, poorer health and a pervasive sense of fraying community fabric.<\/p>\n<p>Our 2022 study found that when a rural hospital closes, hospitals nearby see a measurable spike in inpatient admissions and emergency room visits that can cause significant financial strain. It\u2019s a phenomenon we called \u201c<a href=\"http:\/\/www.doi.org\/10.1002\/jhm.12961\" rel=\"nofollow noopener\" target=\"_blank\">the bystander effect<\/a>\u201d of hospital closures.<\/p>\n<p>Closures can sometimes feel random, but they are predictable consequences of the combination of health policy and market forces.<\/p>\n<p>Spending cuts and Pennsylvania budget<\/p>\n<p>Recent federal legislation has made the financial outlook for rural hospitals more precarious. The <a href=\"https:\/\/www.irs.gov\/newsroom\/one-big-beautiful-bill-act-tax-deductions-for-working-americans-and-seniors\" rel=\"nofollow noopener\" target=\"_blank\">package of tax breaks and spending cuts<\/a> that President Donald Trump signed into law on July 4, 2025, reduced Medicaid eligibility for Americans and capped federal reimbursements to hospitals and other health care providers.<\/p>\n<p>These reimbursements are a critical revenue source for rural hospitals, which tend to serve large numbers of Medicaid patients. <\/p>\n<p>State officials estimate Pennsylvania could lose about <a href=\"https:\/\/www.pa.gov\/governor\/topics\/federal-budget#:%7E:text=What%20happened?,family%20planning%20clinics%20at%20risk\" rel=\"nofollow noopener\" target=\"_blank\">US$20 billion in federal Medicaid funding<\/a> over the next decade, beginning in 2028, due to the 2025 law. <a href=\"https:\/\/www.ifo.state.pa.us\/releases\/846\/Pennsylvania-Medicaid-Update\/\" rel=\"nofollow noopener\" target=\"_blank\">Pennsylvania currently receives $32.6 billion<\/a> in annual federal Medicaid funding. <\/p>\n<p>Within this complicated and shifting policy landscape, federal and state policymakers are also trying to stabilize rural health systems. The 2025 tax and spending law also authorized a <a href=\"https:\/\/www.cms.gov\/priorities\/rural-health-transformation-rht-program\/overview\" rel=\"nofollow noopener\" target=\"_blank\">$50 billion Rural Health Transformation Program<\/a> in 2025 aimed at stabilizing rural care infrastructure.<\/p>\n<p>Meanwhile, Pennsylvania Gov. Josh Shapiro\u2019s proposed $53.3 billion state budget for 2026-27 would maintain current hospital funding levels while adding $1 billion in Medicaid spending. <\/p>\n<p>Why rural hospitals are in crisis<\/p>\n<p>Rural hospitals face a financially difficult task: serve an older, working-class community while staying solvent and being prepared for emergencies at any hour.<\/p>\n<p>The core problem is what economists term \u201chigh fixed costs\u201d \u2013 for staff, equipment, facilities and administration \u2013 that stay <a href=\"https:\/\/www.kff.org\/health-costs\/10-things-to-know-about-rural-hospitals\/\" rel=\"nofollow noopener\" target=\"_blank\">relatively stable even when fewer patients walk through the door<\/a>. <\/p>\n<p>Rural hospitals also tend to <a href=\"https:\/\/www.kff.org\/health-costs\/10-things-to-know-about-rural-hospitals\/\" rel=\"nofollow noopener\" target=\"_blank\">rely heavily on Medicaid and Medicare reimbursements<\/a>, which typically pay less than private insurance companies. This leaves a smaller cushion when costs rise.<\/p>\n<p>Policy choices can change the odds. A 2018 study found that states that <a href=\"https:\/\/www.kff.org\/affordable-care-act\/states-getting-a-jump-start-on-health\/\" rel=\"nofollow noopener\" target=\"_blank\">expanded Medicaid<\/a> under the Affordable Care Act saw rural hospitals <a href=\"https:\/\/www.doi.org\/10.1377\/hlthaff.2017.0976\" rel=\"nofollow noopener\" target=\"_blank\">perform better financially and close less often<\/a>, especially those in areas with large uninsured populations. <\/p>\n<p>When hospitals begin to struggle financially, they often seek a larger entity to buy them or try to merge into a robustly financed health system. But consolidation has trade-offs. One national study found commercial insurance prices for patients at nearby hospitals <a href=\"https:\/\/www.doi.org\/10.1377\/hlthaff.2024.00700\" rel=\"nofollow noopener\" target=\"_blank\">rose 3.6%<\/a> after a rural hospital closed. <\/p>\n<p>In 2021 the Centers for Medicare &amp; Medicaid Services began granting a \u201c<a href=\"https:\/\/www.cms.gov\/medicare\/health-safety-standards\/certification-compliance\/rural-emergency-hospitals\" rel=\"nofollow noopener\" target=\"_blank\">rural emergency hospital\u201d designation<\/a>. This designation allows struggling rural hospitals to keep their emergency departments and outpatient services while eliminating inpatient beds. <\/p>\n<p>For some communities, having a rural emergency hospital preserves a critical point of access to care. Yet residents still <a href=\"https:\/\/www.doi.org\/10.1111\/jrh.70112\" rel=\"nofollow noopener\" target=\"_blank\">feel the loss of other hospital services<\/a> that are no longer available locally.<\/p>\n<p>            <img decoding=\"async\" alt=\"Exterior of an emergency department of a hospital\" src=\"https:\/\/www.newsbeep.com\/us-pa\/wp-content\/uploads\/2026\/04\/file-20260323-57-tpn6h.jpg\" class=\"native-lazy\" loading=\"lazy\"  \/><\/p>\n<p>              A hospital that receives a \u2018rural emergency hospital\u2019 designation has an emergency department and outpatient services but no inpatient beds.<br \/>\n              <a class=\"source\" href=\"https:\/\/newsroom.ap.org\/detail\/RuralEmergencyHospitals\/33af38cb2ffe4cf499adbb61a8615852\/photo\" rel=\"nofollow noopener\" target=\"_blank\">AP Photo\/Rogelio V. Solis<\/a><\/p>\n<p>Cascade of consequences<\/p>\n<p>We\u2019ve observed that the communities hit hardest by rural hospital closures share some common traits: <a href=\"https:\/\/www.kff.org\/health-costs\/10-things-to-know-about-rural-hospitals\/\" rel=\"nofollow noopener\" target=\"_blank\">shrinking and aging populations<\/a>, <a href=\"http:\/\/www.doi.org\/10.2139\/ssrn.6290348\" rel=\"nofollow noopener\" target=\"_blank\">loss of manufacturing or mining jobs<\/a>, increasing rates of \u201c<a href=\"https:\/\/doi.org\/10.1001\/jamanetworkopen.2021.18134\" rel=\"nofollow noopener\" target=\"_blank\">diseases of despair<\/a>\u201d \u2013 suicidality, drug abuse, alcoholism \u2013 difficulty recruiting health care workers, and fewer local medical services when something goes wrong.<\/p>\n<p>The most immediate consequence of a closure is the distance former patients must travel for care. Patients in urban areas often have many health care options at their disposal, but the Government Accountability Office has found that after closures, rural residents traveled about <a href=\"https:\/\/www.gao.gov\/products\/gao-21-93\" rel=\"nofollow noopener\" target=\"_blank\">20 miles farther for routine care<\/a> and up to 40 miles farther for specialized services such as addiction treatment. <\/p>\n<p>This can have consequences: a longer and costlier ambulance ride, a missed follow-up because an appointment interferes with work obligations, a caregiver who loses half a day of work to drive a loved one to a specialist, or someone struggling with depression and painkiller addiction who doesn\u2019t feel mentally able to face a road trip.<\/p>\n<p>Emergency response is affected too. A national analysis found closures <a href=\"https:\/\/www.doi.org\/10.1111\/1475-6773.13254\" rel=\"nofollow noopener\" target=\"_blank\">increased average ambulance transport time<\/a> and total time from calling 911 to receiving care. <\/p>\n<p>Maternity care is especially concerning. A national study found 537 hospitals stopped delivering babies between 2010 and 2022, with <a href=\"https:\/\/www.doi.org\/10.1001\/jama.2024.23010\" rel=\"nofollow noopener\" target=\"_blank\">238 of them located in rural areas<\/a>. <\/p>\n<p>An estimated <a href=\"https:\/\/www.rural.pa.gov\/download.cfm?file=Resources\/PDFs\/Access%20to%20Maternity%20Care%20in%20Pennsylvania%20Labor%20and%20Delivery%20Services%20Fact%20Sheet%20Web.pdf\" rel=\"nofollow noopener\" target=\"_blank\">22 of Pennsylvania\u2019s 67 counties<\/a> \u2013 all of them rural \u2013 do not have a hospital that provides labor and delivery services. Research shows fewer people gave birth in their own county after closures, with <a href=\"https:\/\/www.doi.org\/10.1111\/1475-6773.14248\" rel=\"nofollow noopener\" target=\"_blank\">worse maternal and fetal outcomes<\/a> for individuals coming from moderately rural areas. <\/p>\n<p>            <img decoding=\"async\" alt=\"View of people sitting in blue chairs in hospital waiting room\" src=\"https:\/\/www.newsbeep.com\/us-pa\/wp-content\/uploads\/2026\/04\/file-20260323-57-ep63fk.jpg\" class=\"native-lazy\" loading=\"lazy\"  \/><\/p>\n<p>              Many rural hospital patients rely on Medicaid, which tends to reimburse health care providers at lower rates than private insurance companies.<br \/>\n              <a class=\"source\" href=\"https:\/\/www.gettyimages.com\/detail\/news-photo\/patients-wait-to-be-seen-in-the-emergency-room-at-valley-news-photo\/2220669620\" rel=\"nofollow noopener\" target=\"_blank\">Ricky Carioti\/The Washington Post via Getty Images<\/a><\/p>\n<p>Debunking common misconceptions<\/p>\n<p>White papers written by advisory groups and consulting firms tend to have differing takes compared with academic literature on this topic. This contrast may reflect the distinct perspectives and incentive structures guiding their recommendations for rural hospital and health care reform. As a result, we feel it is important to highlight misconceptions and revisit some fundamental concepts regarding rural health care. <\/p>\n<p>1. Hospitals that close are not necessarily poorly managed<\/p>\n<p>Leadership matters, but the same pattern repeats across states and decades: <a href=\"https:\/\/www.kff.org\/health-costs\/10-things-to-know-about-rural-hospitals\/\" rel=\"nofollow noopener\" target=\"_blank\">Fewer patients, high fixed costs and payment structures<\/a> <a href=\"https:\/\/bipartisanpolicy.org\/report\/sustaining-rural-hospital-access-adjustments-to-medicare-rural-hospital-designations\/\" rel=\"nofollow noopener\" target=\"_blank\">disadvantage rural hospitals<\/a>. <\/p>\n<p>2: Telehealth cannot replace a hospital<\/p>\n<p>Telehealth fills some gaps, but it can\u2019t substitute for an emergency response system <a href=\"https:\/\/www.doi.org\/10.1111\/1475-6773.13254\" rel=\"nofollow noopener\" target=\"_blank\">when minutes matter<\/a>, or replace care that <a href=\"https:\/\/www.gao.gov\/products\/gao-21-93\" rel=\"nofollow noopener\" target=\"_blank\">requires patients to be physically present<\/a>.<\/p>\n<p>3. Effects ripple beyond rural areas<\/p>\n<p>When hospitals close, <a href=\"https:\/\/www.doi.org\/10.1002\/jhm.12961\" rel=\"nofollow noopener\" target=\"_blank\">nearby facilities absorb the overflow<\/a> and often face less competition. Research shows <a href=\"https:\/\/www.doi.org\/10.1377\/hlthaff.2024.00700\" rel=\"nofollow noopener\" target=\"_blank\">their prices tend to rise<\/a>. This is due to lower reimbursements from patients\u2019 insurers, an increase in care complexity and more treatment plans that are non-procedural. Elective surgical and nonsurgical procedures, which can be scheduled in advance, <a href=\"https:\/\/www.doi.org\/10.1186\/s12913-020-05975-z\" rel=\"nofollow noopener\" target=\"_blank\">drive much of the profits hospitals need to survive<\/a>.<\/p>\n<p>Because <a href=\"https:\/\/doi.org\/10.1093\/haschl\/qxae078\" rel=\"nofollow noopener\" target=\"_blank\">hospital prices are a major driver of insurance premiums<\/a>, those increases may ultimately affect patients and employers <a href=\"https:\/\/doi.org\/10.1377\/hlthaff.2024.00700\" rel=\"nofollow noopener\" target=\"_blank\">beyond the communities where closures occur<\/a>.<\/p>\n<p>4. Conversion to outpatient-only facilities comes with costs<\/p>\n<p>Rural emergency hospital status preserves 24\/7 emergency and outpatient care but <a href=\"https:\/\/www.cms.gov\/medicare\/health-safety-standards\/certification-compliance\/rural-emergency-hospitals\" rel=\"nofollow noopener\" target=\"_blank\">eliminates inpatient beds<\/a>. That means patients who need further hospital care \u2013 even for a short recovery \u2013 must be transferred, which can add cost, risk and expense. While this model can retain emergency services, <a href=\"https:\/\/www.doi.org\/10.1111\/jrh.70112\" rel=\"nofollow noopener\" target=\"_blank\">communities still feel the loss<\/a>  of local inpatient care. <\/p>\n<p>5. Mergers don\u2019t always save the day<\/p>\n<p>Affiliations with larger health systems or private equity groups <a href=\"https:\/\/www.doi.org\/10.1177\/10775587251355671\" rel=\"nofollow noopener\" target=\"_blank\">have mixed outcomes<\/a>. Even when a hospital stays open, services can change significantly due to organizational restructuring. Some researchers and policymakers have expressed concern about the <a href=\"https:\/\/hsph.harvard.edu\/news\/private-equitys-appetite-for-hospitals-may-put-patients-at-risk\/\" rel=\"nofollow noopener\" target=\"_blank\">aggressive cost-cutting strategies of private equity groups<\/a>, which can increase financial pressure on already vulnerable hospitals.<\/p>\n<p>We believe rural hospitals are vital to the economic and social stability of Pennsylvania\u2019s rural communities, and that funding them adequately requires an approach that takes into account their high fixed costs but low patient volume.
