{"id":27863,"date":"2025-11-07T16:42:10","date_gmt":"2025-11-07T16:42:10","guid":{"rendered":"https:\/\/www.newsbeep.com\/us-pa\/27863\/"},"modified":"2025-11-07T16:42:10","modified_gmt":"2025-11-07T16:42:10","slug":"pennsylvania-counties-face-tough-choices-on-spending-2b-opioid-settlement-funds-2","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/us-pa\/27863\/","title":{"rendered":"Pennsylvania counties face tough choices on spending $2B opioid settlement funds"},"content":{"rendered":"<p class=\"mb-4 text-lg md:leading-8 break-words\">In communities across Pennsylvania, local officials are deciding how to spend over US$2 billion dollars from the state\u2019s <a href=\"https:\/\/www.opioidsettlementguides.com\/pennsylvania\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:opioid settlement agreements;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">opioid settlement agreements<\/a>.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">For many, the task is proving promising yet challenging \u2013 and raises questions about how to best navigate complex local needs.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Pennsylvania will receive the money over 18 years from lawsuits filed by state attorneys general against opioid manufacturers and distributors. About 70% of these funds will be <a href=\"https:\/\/www.opioidsettlementguides.com\/pennsylvania\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:distributed to county governments;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">distributed to county governments<\/a>, with the remaining funds going to the state legislature and the groups that leveraged the lawsuits.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">The <a href=\"https:\/\/doi.org\/10.1111\/1468-0009.12589\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:amount provided to each county;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">amount provided to each county<\/a> is proportional to the opioid-related harms experienced by the county. Each county government is responsible for developing its own funding strategy for substance use programs, which can focus on things such as prevention, treatment, recovery or harm reduction.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Our <a href=\"https:\/\/csua.ssri.psu.edu\/elevate-pennsylvania-initiative\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:research team;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">research team<\/a> at Penn State interviewed 72 county officials, health professionals and service providers across six counties in Pennsylvania to understand their early experiences with these funds.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">We <a href=\"https:\/\/doi.org\/10.1186\/s13011-025-00665-7\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:summarized our findings;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">summarized our findings<\/a> in a recent article for the peer-reviewed Substance Abuse Treatment, Prevention, and Policy journal. We found that stakeholders view the settlement funds not simply as extra money but as an opportunity to heal \u2013 and to test how well local communities can make their own choices about spending.<\/p>\n<p>\u2018Bags of money\u2019 but limited guidance<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Pennsylvania\u2019s distribution strategy was designed to give local governments flexibility. A document called <a href=\"https:\/\/www.paopioidtrust.org\/getmedia\/4877e10e-cb3f-44b7-acbc-465affc880e4\/Exhibit-E-List-of-Opioid-Remediation-Uses.pdf\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:Exhibit E;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">Exhibit E<\/a> lists the ways that counties can spend the settlement money.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">This collaborative document was written as part of the settlement to outline shared guidelines that apply to all the states receiving funds. It lists everything from the types of approved substance use treatments to what qualifies as prevention. In practice, Exhibit E provides diverse opportunities for spending but has also created widespread uncertainty among recipients about which strategies to prioritize.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Some interviewees felt overwhelmed by the logistics of their funding decisions. They understood that the general purpose of the money is to support communities harmed by opioid overprescription. But they lacked clarity on how much time they had to spend it, what the reporting requirements are, and what counts as an eligible activity. For example, some wanted to use the funds to pay administrators for new prevention programs, but administration isn\u2019t included in Exhibit E.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">As one local elected official in southeastern Pennsylvania put it, \u201cThere\u2019s been a whole lot of stuff that we don\u2019t know \u2013 more than we do know. And now we\u2019re running with bags of money through the community and (we\u2019re) not sure how we can spend it, or if we can spend it.\u201d<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Many county officials worried about spending the funds too slowly, or on activities that could end up being ineligible or ineffective. Service providers sometimes didn\u2019t know who in their county had the authority to decide where the money went. While they may have wanted to provide recommendations or input, they were unsure how.<\/p>\n<p><img alt=\"\" loading=\"lazy\" width=\"960\" height=\"752\" decoding=\"async\" data-nimg=\"1\" class=\"rounded-lg\" style=\"color:transparent\" src=\"https:\/\/s.yimg.com\/ny\/api\/res\/1.2\/q3VZ0FbmkR1FarB0c9nQzQ--\/YXBwaWQ9aGlnaGxhbmRlcjt3PTk2MDtoPTc1MjtjZj13ZWJw\/https:\/\/media.zenfs.com\/en\/the_conversation_us_articles_815\/4598db961b3664e0610c867dfd393363\"\/>A chance to experiment and innovate<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Even amid confusion, most of the people we interviewed saw the settlement funding as a unique opportunity.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Exhibit E\u2019s broad guidelines allow for experimentation, and many expressed interest in supporting local needs and implementing projects that they had wanted for a long time. This included things like expanding peer recovery support programs or establishing family support services.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">\u201cThe guidelines are so varied that it gives those local communities opportunities to look at the menu and find out from community members, \u2018How can we help resolve this problem together?\u2019\u201d one local drug and alcohol department employee told us. \u201cIt\u2019s a collaborative that really helps the community as a whole get well as a whole. I am a real believer in \u2018It takes a village.\u2019\u201d<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Several participants emphasized that the flexibility in Exhibit E creates room to revise plans as needs evolve or change. Counties can change their funding priorities each year to adapt.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Several counties have already started issuing small grants to grassroots organizations, recognizing that those closest to people harmed by the opioid crisis often know best what kinds of interventions might work.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">One county employee involved in distributing funds in her county shared that her team was \u201cwilling to try anything, really, within the bounds.\u201d<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">\u201cAnd if it doesn\u2019t work, we can back off,\u201d she added. \u201cBut I feel like you don\u2019t know until you try it.\u201d<\/p>\n<p>A moral responsibility to get it right<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Although our study focused on policy implementation, participants often framed their responsibilities in moral terms.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Many said they felt a strong obligation to use the funds wisely, given the scale of loss their communities have endured. The Pennsylvania Department of Health reported <a href=\"https:\/\/www.pa.gov\/agencies\/health\/programs\/opioids\/individuals-families\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:4,719 overdose deaths in the state;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">4,719 overdose deaths in the state<\/a> in 2023, and 83% were opioid-related. That number <a href=\"https:\/\/public.tableau.com\/app\/profile\/pennsylvania.pdmp\/viz\/PennsylvaniaODSMPDrugOverdoseSurveillanceInteractiveDataReport\/Contents\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:dropped to 3,336 in 2024;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">dropped to 3,336 in 2024<\/a>, mirroring <a href=\"https:\/\/www.cdc.gov\/nchs\/nvss\/vsrr\/drug-overdose-data.htm\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:national trends;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">national trends<\/a>.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">One elected official described the funds as \u201cthe only hope we can provide families that have lost loved ones to this crisis,\u201d emphasizing that he felt a \u201creal obligation\u201d to make the funds count.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Others echoed that careful, transparent decision-making is part of a broader recovery effort. Beyond abiding by funding guidelines, they felt it was also important to be honest and transparent to community members.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">\u201cWe don\u2019t want to come out with \u2018Pennsylvania wasted its money, or (this) county wasted its money,\u2019\u201d said an addictions researcher.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Still others cautioned that the settlement funds alone cannot repair the full scope of harms caused by the opioid crisis, warning against viewing the settlements as a cure-all.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">\u201cThere\u2019s not really a monetary value that you can put on these things,\u201d a person who works in the substance use sector told us. \u201cI\u2019m glad that this money\u2019s available, but ultimately for me \u2026 it\u2019s a little too late. You know?
