The department added 21 cases since its Oct. 21 update and now counts 255 hospitalizations and five measles-associated deaths this year.

The Crawford case matters less for its size than for its location. Warren County has reported 13 cases, Mercer County 24 and Venango County one, while Ashtabula County, Ohio, just across the state line, has reported 44, the Erie Times-News reported. For families in Meadville, Erie and nearby towns, the outbreak is no longer only a story about Lancaster County. It has reached their corner of the state.

No details about the Crawford patient have been released, which leaves parents without a clear picture of where exposure may have occurred. That makes checking vaccine records now especially important.

Northwest Pennsylvania Moves Onto the Measles Map

Erie County had not reported a 2026 case as of Oct. 20, but local officials have been bracing for one. “We are surrounded by counties with reported cases,” Erin Mrenak, director of the Erie County Department of Health, said before she learned of the Crawford case, according to the Erie Times-News. Erie County reported eight cases in 2025, its first since 1991.

Mrenak attributed Erie County’s lack of cases so far to a mix of luck and relatively high vaccination rates.

The outbreak’s center remains in the southern and central parts of the state. Lancaster County has reported more than 450 cases, Mifflin County nearly 200 and Chester County more than 90, according to mid-October state figures.

MedicalDaily previously reported on Pennsylvania’s immunoglobulin alert for exposed pregnant women, when the count stood at 1,286 cases in 42 counties. Since then, the state has added 52 cases and two more counties, and the outbreak has moved closer to the Ohio line.

Vaccine Demand Climbs as Cases Spread

Residents are responding. Pennsylvania providers typically give about 25,000 MMR doses a month, but they gave more than 56,000 in September and more than 39,000 in the first three weeks of October, the health department said. State health center staff have given more than 6,500 shots at 226 pop-up clinics since the outbreak began in late April.

The department reports only four cases among vaccinated people, less than 1% of the total. Two MMR doses are about 97% effective. Infants 6 to 11 months old who live in or travel to counties flagged for sustained community transmission on the state’s measles dashboard can get an early extra dose, which does not replace the two routine doses.

The death count remains in dispute. Pennsylvania reports five measles-associated deaths, while the CDC lists three nationwide, and Health Secretary Debra Bogen has accused the agency of obscuring the seriousness of the disease by not acknowledging the state’s deaths, The Philadelphia Inquirer reported. A possible sixth death, reported by the Franklin County coroner, is still under review.

Symptoms and Exposure Steps for Northwest Families

Symptoms usually begin 7 to 21 days after exposure with fever, cough, runny nose and red, watery eyes. A rash follows a few days later, starting on the head and spreading downward. People are typically contagious from four days before to four days after the rash appears, and the virus can remain infectious in the air and on surfaces for up to two hours.

Those at highest risk of serious illness include babies younger than 12 months, pregnant people without immunity, people with weakened immune systems, and unvaccinated adults. Nearly 20% of Pennsylvania patients have been hospitalized. Trouble breathing, confusion, seizures or signs of dehydration need urgent care.

Anyone who suspects measles should call a health care provider or the state hotline at 877-PA-HEALTH before arriving, so staff can prevent exposures in waiting rooms. Eligible children can get free shots through the Vaccines for Children program, and state health centers and federally qualified health centers also offer MMR.

The department updates its count several times a week. Upcoming figures will show whether Crawford’s case leads to others and whether Erie County records its first 2026 case. For northwest families, the most useful step now is a vaccine-records check.

Oct. 23, 2026: Pennsylvania reports 1,338 cases in 44 counties, with Crawford County added to the list.

Oct. 21, 2026: Crawford County’s first measles case since 2019 is reported as the state count reaches 1,317.

Oct. 19, 2026: Pennsylvania reports 1,286 cases in 42 counties.

Late April 2026: The current outbreak begins, according to the state health department.

Key Questions Answered

How many measles cases does Pennsylvania have?
