{"id":43701,"date":"2025-11-25T01:01:06","date_gmt":"2025-11-25T01:01:06","guid":{"rendered":"https:\/\/www.newsbeep.com\/us-pa\/43701\/"},"modified":"2025-11-25T01:01:06","modified_gmt":"2025-11-25T01:01:06","slug":"what-pa-s-50-1-billion-budget-does-for-rural-hospitals-communities-2","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/us-pa\/43701\/","title":{"rendered":"What Pa.\u2019s $50.1 billion budget does for rural hospitals, communities"},"content":{"rendered":"<p>BELLEFONTE \u2014 As federal Medicaid cuts loomed over Pennsylvania\u2019s rural hospitals, health advocates focused on safeguarding funding for these already strained facilities and programs that help keep their doors open during this year\u2019s state budget talks.<\/p>\n<p>The $50.1 billion state budget \u2014 passed by the legislature and signed by Democratic Gov. Josh Shapiro <a href=\"https:\/\/www.spotlightpa.org\/news\/2025\/11\/pennsylvania-budget-education-funding-rggi-climate-cyber-charter-capitol\/\" target=\"_blank\" rel=\"noopener nofollow\">after a four-month impasse<\/a> \u2014 maintains support for rural and critical access hospitals. It preserves spending on programs that have faced <a href=\"https:\/\/www.spotlightpa.org\/statecollege\/2024\/05\/pennsylvania-elk-county-penn-highlands-dubois-st-marys-rural-labor-delivery-maternity-care\/\" target=\"_blank\" rel=\"noopener nofollow\">cuts and closures<\/a> in rural areas. And it increases state Medicaid spending as the feds prepare to cut roughly $1 trillion in funding over the next decade because of the so-called \u201cbig, beautiful\u201d budget bill.<\/p>\n<p>While stakeholders are pleased with the state\u2019s <a href=\"https:\/\/www.pa.gov\/content\/dam\/copapwp-pagov\/en\/budget\/documents\/publications-and-reports\/commonwealthbudget\/2025-26-budget-documents\/2025-26%20enacted%20budget%20-%20general%20fund%20state%20appropriations%20v2.pdf\" target=\"_blank\" rel=\"noopener nofollow\">final deal<\/a>, it only offers some relief for rural Pennsylvania, where hospitals operate on thin margins and Medicaid helps keep the lights on. Advocates \u2014 now focused on next year\u2019s budget \u2014 are hoping Pennsylvania can secure a piece of a new $50 billion federal program to address longstanding challenges to accessing care in rural communities.<\/p>\n<p>The Hospital and Healthsystem Association of Pennsylvania (HAP), which represents more than 235 hospitals statewide, welcomed the preservation of past funding \u2014 such as $7.9 million for critical access hospitals and $10.6 million for obstetric and neonatal services \u2014 president and CEO Nicole Stallings said.<\/p>\n<p>The budget includes $10 million in one-time payments to rural hospitals, which will draw a $25.1 million federal match. Financially distressed hospitals will also receive $13.5 million, and while those dollars aren\u2019t exclusively earmarked for rural facilities, Stallings expects they\u2019ll still benefit.<\/p>\n<p>Lisa Davis, director of the Pennsylvania Office of Rural Health, said the boost for Medical Assistance, or Medicaid programs \u2014 a more than $775 million increase collectively \u2014 is \u201creally needed\u201d amid an expected <a href=\"https:\/\/www.spotlightpa.org\/statecollege\/2025\/07\/medicaid-pennsylvania-trump-federal-budget-rural-hospitals\/\" target=\"_blank\" rel=\"noopener nofollow\">increase in uninsured patients and uncompensated care<\/a> due to the federal cuts.<\/p>\n<p>\u201cNo state in the country can really adequately fill those losses, but I think being able to have this increased funding for Medicaid is good,\u201d Davis said.<\/p>\n<p>About 3 million people in Pennsylvania \u2014 23% of the population \u2014 are covered by Medicaid, according to <a href=\"https:\/\/www.pa.gov\/agencies\/dhs\/resources\/data-reports.html\" target=\"_blank\" rel=\"noopener nofollow\">state data<\/a>. More than 728,000 recipients live in rural counties, where residents are typically older and more reliant on government-funded insurance programs. Those programs reimburse at lower rates compared to private insurance companies and haven\u2019t kept pace with inflation.<\/p>\n<p>Medicaid currently reimburses hospitals 71 cents on the dollar. Once the federal changes <a href=\"https:\/\/www.kff.org\/medicaid\/implementation-dates-for-2025-budget-reconciliation-law\/\" target=\"_blank\" rel=\"noopener nofollow\">take full effect<\/a>, HAP estimates that number will drop to 64 cents, Stallings said.<\/p>\n<p>\u201cWe know that\u2019s not going to be sustainable,\u201d she said. \u201cAbout 39% of our rural hospitals are currently experiencing year-over-year negative operating margins. They are financially fragile and distressed, and these cuts will really accelerate a lot of the challenges that they\u2019re facing.\u201d<\/p>\n<p>A <a href=\"https:\/\/www.markey.senate.gov\/imo\/media\/doc\/letter_on_rural_hospitals.pdf\" target=\"_blank\" rel=\"noopener nofollow\">report<\/a> commissioned by Congressional Democrats over the summer estimated that five rural hospitals in Pennsylvania were at risk of closing or reducing services. The list included Penn Highlands Connellsville in Fayette County, which <a href=\"https:\/\/www.monvalleyindependent.com\/2025\/06\/20\/penn-highlands-healthcare-announces-layoffs\/\" target=\"_blank\" rel=\"noopener nofollow\">announced layoffs in June<\/a>.<\/p>\n<p>Congressional Republicans did secure a $50 billion allocation for the Rural Health Transformation Program as part of the federal spending package. Lawmakers see it as an opportunity to address workforce gaps, upgrade technology, and bolster access to care.<\/p>\n<p>\u201cAnything that comes down from the federal government to help ease up on the burden of health care in the state is always welcome,\u201d state Rep. Kathy Rapp, R-Warren, said.<\/p>\n<p>A spokesperson for the state Department of Human Services <a href=\"https:\/\/www.spotlightpa.org\/statecollege\/2025\/07\/medicaid-pennsylvania-trump-federal-budget-rural-hospitals\/\" target=\"_blank\" rel=\"noopener nofollow\">previously<\/a> said the federal grant dollars won\u2019t be enough to fill the \u201cmassive gap\u201d created by the federal Medicaid cuts. Allocations from the program will be decided by Dec. 31 and begin during fiscal year 2026.<\/p>\n<p>Pennsylvania\u2019s <a href=\"https:\/\/www.pa.gov\/content\/dam\/copapwp-pagov\/en\/dhs\/documents\/rural-health\/pa-rural-health-transformation-plan.pdf\" target=\"_blank\" rel=\"noopener nofollow\">67-page application<\/a> asks for $200 million annually, totaling $1 billion over five years. The money cannot be used to replace federal funds lost from other cuts and changes.<\/p>\n<p>The state pulled from years of feedback from rural communities to draft its request, which focuses on regionalized care, expanded mental health resources, and modernizing emergency services infrastructure.
