The Dallas Count Health and Human Services Diabetes Prevention Program starts next month. Here’s how residents can join this supportive community dedicated to building better habits and increasing physical activity.

The free, 12-month program supports healthy lifestyle change to prevent or reduce the risk of Type 2 diabetes.

The program will feature incentives and prizes to help participants reach weight loss goals, become more physically active, and build healthier habits. Participants will receive lifestyle coaching and group support as well as stress management tools.

Residents can enroll in the program by visiting this webpage. Participants must be 18 years or older, have a BMI of greater than or equal to 25 kg/m2 (or greater than or equal to 23 kg/m2 if they are Asian American), and have a previous diagnosis of type 1 or type 2 diabetes.

Additionally, participants must take a blood test and meet certain metrics, be clinically diagnosed with gestational diabetes mellitus (GDM) during pregnancy, or get a score of 5 or above on the prediabetes screening test.

The blood test must be from within the past year. A health care professional may refer potential participants.

In addition to obesity and diabetes prevention, the Disease Prevention Division works to promote food and nutrition, asthma control, tobacco use control and prevention, and hypertension prevention.
