A Plano restaurant remained closed after a second health inspection found a roach infestation and other serious violations, according to the city’s inspection reports.

The reports compiled by the Star-Telegram contain data on health violations from 232 inspections between Aug. 2 and Aug. 15.

Plano’s health inspections function on a 100-point scale, where a score of 100 is considered perfect and a score of 70 is considered extremely poor. Each establishment receives between one and four routine inspections per year.

Nizam’s Biryaniwala at 909 W. Spring Creek Parkway, Suite 230 scored 61 and was temporarily closed after health inspectors observed a roach infestation throughout the establishment. In the followup inspection on Aug. 14, the restaurant was ordered to remain closed until a written approval was obtained from the health department.

The restaurant was initially closed during an inspection July 30, when more than 20 live and 100 dead roaches were seen at the eatery. Inspectors also noted that some spices at the restaurant were contaminated with live roaches and considered them adulterated and not safe for human consumption, the Star-Telegram previously reported.

Additional restaurants with violations related to roaches and rodent droppings:

Thai Jasmine Thai Cuisine at 2050 W. Spring Creek Parkway, Suite 206. Health inspectors found multiple pests-related violations including dried spices contaminated with rodent droppings, and more than 20 dead roaches were seen throughout the kitchen area. The inspectors also noted that food handlers were using liquid hand soap at a kitchen handwashing sink that had dead roaches floating in it. The restaurant scored 83.

Rajula’s Kitchen at 820 W. Spring Creek Parkway, Suite 304. Health inspectors found one small roach-like insect on the floor in the back kitchen prep area. The restaurant scored 87.

The Gyro Shop at 6909 Coit Road, Suite 201, had a score of 91. Health inspectors observed five dead roaches at the restaurant.

Fang’s Wok at 3309 Coit Road, Suite 100. Score: 71.

Café Mozart Bakery at 2070 W. Spring Creek Parkway, Suite 306. Score: 77.

Indopak Supermarket at 2060 W. Spring Creek Parkway, Suite 400. Score: 79.

Kabab Darbar at 3825 W. Spring Creek Parkway, Suite 209. Score: 79.

New Orleans Crab Shack at 901 W. Spring Creek Parkway, Suite 125. Score: 79.
