The committee is considering a resolution reaffirming the city’s support for BAMC’s Level I trauma center and military medical readiness mission.

The draft resolution states Methodist Healthcare has announced plans to develop or seek designation as a Level II trauma facility on Oct. 1, 2026. It says without safeguards, the additional trauma capacity could reduce the civilian trauma volume needed for BAMC’s training mission.

The resolution also asks the Southwest Texas Regional Advisory Council, or STRAC, to weigh in. STRAC coordinates the trauma and emergency health care system for 22 counties in its region. STRAC does not designate trauma centers — that authority belongs to the Texas Department of State Health Services (DSHS).

BAMC is the Department of Defense’s only Level I trauma center. It also serves as a training and research center for military physicians, nurses, medics and other health care professionals preparing to treat combat injuries.

University Hospital also has a Level I trauma center.

Level I vs. Level II trauma centers

According to the American College of Surgeons, a Level I trauma center provides the broadest range of services and sees more patients. It must have immediate access to specialists, surgery, critical care and other advanced services. Level I centers also have significant teaching, research and injury-prevention responsibilities.

A Level II trauma center also provides comprehensive emergency trauma care and must have round-the-clock surgical and specialty capabilities. But it generally has fewer research, teaching and volume requirements than a Level I center, according to ACS. Some of the most complex patients may still need to be transferred to receive a higher level of care, potentially to a Level I facility.

The city resolution says BAMC needs a sufficient volume of patients, as well as a mix of injuries with different levels of complexity, to support the military’s mission. The resolution says if another hospital receives patients who might otherwise go to BAMC, the military facility could see fewer opportunities to treat the most complex injuries.

This is not the first time Methodist has proposed a trauma center

Methodist proposed adding a Level II trauma center in San Antonio in 2017.

In January 2018, KSAT reported Army officials opposed the idea. Brig. Gen. Jeffrey Johnson, then the top commander at BAMC, said an additional trauma center could threaten military medical training and Defense Department readiness.

At the time, city and county officials publicly voiced concerns yet acknowledged they did not have the authority to stop a state trauma designation.

KSAT has reached out to officials at BAMC, Methodist Healthcare, University Hospital, DSHS and STRAC and will update this article as we get responses.

Read also:

Copyright 2026 by KSAT – All rights reserved.

SAN ANTONIO – While leaders at Methodist Healthcare pitch a Level II trauma center, city officials say it…

[8
154893,584,111399,82,84,83
post-450329","post","type-post","status-publish","format-standard","has-post-thumbnail","category-san-antonio","tag-bamc","tag-health","tag-methodist-hospital","tag-san-antonio","tag-san-antonio-headlines","tag-san-antonio-news
["GET
