Dell Children’s at Home, the pediatric hospice program at Dell Children’s Medical Center<\/a>, is transitioning from home-based care to an outpatient clinic setting, a spokesperson told Community Impact on Oct. 30.<\/p>\n

What’s happening<\/p>\n

Care will be centralized at Dell Children’s Comprehensive Care Clinic<\/a>, located on the second floor of Austin ISD’s Rosedale School<\/a> campus. It serves children 17 and younger with complex medical needs, according to its website. The clinic setting offers specialized symptom management, coordinated clinic appointments and daily telephone check-ins.<\/p>\n

“Centralizing care within our Comprehensive Care Clinic seamlessly connects families to our pediatric palliative care physicians, case managers, and social workers all in one location,” the spokesperson said.<\/p>\n

Complex care referrals are already underway, and individualized transition plans have been created for each child.<\/p>\n
