DAYTON, Ohio and NEW YORK, Sept. 3, 2025 /PRNewswire/ — The New York Department of Health (NY DOH) has approved the affiliation between CareSource and ElderServe Health effective Sept. 1, 2025.
In March 2025, the two organizations announced plans to seek an affiliation, subject to state regulatory approvals. With the NY DOH’s approval, the agreement can now move forward. As part of this transition, RiverSpring Living, the organization’s former parent company, will conclude its affiliation with ElderServe Health.

With approximately 3.5 million New Yorkers aged 65 and over, this demographic represents the fastest-growing segment of the state’s population, and more than half will require long-term care at some point in their lives. Additionally, over a million New Yorkers with disabilities, chronic illnesses or other functional challenges are expected to need long-term care services. The CareSource and ElderServe Health affiliation is strategically designed to address these critical long-term care needs for New Yorkers.

ElderServe Health provides long-term services and supports to over 20,000 older adults and individuals with chronic conditions across New York City, as well as Westchester, Nassau, and Suffolk Counties. The organization primarily serves a frail and elderly membership who are predominantly dual-eligible and enrolled in one of several programs, including the New York State Medicaid Managed Long Term Care (MLTC) Program, Medicare Advantage Institutional Special Needs Plan (I-SNP), or Medicaid Advantage Plus / Medicare Advantage Dual Eligible Special Needs Program (D-SNP MAP).
