UNDATED (WKRC) – A large health care system that operates in two states mistakenly sent condolence letters to 531 patients about their own deaths.

ABC News reported that MaineHealth said it “sincerely regrets” the error. The letters included information on how the next of kin could resolve the patients’ estates.

A company spokesperson told ABC News that an error in the computer system caused the mistake, generating estate vendor letters that were mistakenly sent to living patients.

The patients were not listed as deceased in the company’s medical records, and apology letters were sent to all those affected, according to the spokesperson.
