An investigation found that Advanced Pain and Regenerative Medical Solutions Corporation, or APRM Solutions, submitted 4,009 fraudulent claims to the Connecticut Medicaid Program for allergy immunotherapy services, the DCJ said. The agency noted that the services were either not performed, or performed by an unsupervised, unqualified person.
The scheme resulted in a profit of $659,627.66, according to the DCJ.
The Office of the Chief State’s Attorney’s Medicaid Fraud Control Unit charged David Waltzer, 62, of East Haven; Eid J. Atallah, 61, of Middlebury; Terry L. Choszczyk, 65, of Seymour; and Kimberly Choszczyk, 38, of Ansonia, the DCJ said.
The DCJ noted that the four people either owned or worked at the clinic.
The DCJ said APRM Solutions filed the fraudulent billings between February 2024 and October 2025. The claims were filed using medical providers’ personal identification numbers without their permission, according to the DCJ.
Additionally, Kimberly Choszczyk, also known as Kimberly Castillo, performed allergy testing, mixed allergy medicine and administered allergy shots without a medical license, the DCJ said.
Waltzer, Atallah, Terry Choszczyk and Kimberly Choszczyk each are charged with first-degree larceny by defrauding a public community, health insurance fraud, violation of license requirements, conspiracy to commit first-degree larceny by defrauding a public community, conspiracy to commit health insurance fraud and conspiracy to commit violation of license requirements.
They were each released on $100,000 bonds and are scheduled to appear at state Superior Court in Derby May 29, the DCJ said.
