DAKOTA DUNES, S.D. (KTIV) – A major healthcare shift is coming to Siouxland, as Avera is acquiring the CNOS practice based in Dakota Dunes, South Dakota.
CNOS and Avera announced the new partnership in a joint press release on Wednesday, July 15. In it, Avera and CNOS said this partnership includes over 100 CNOS physicians, advanced practice providers, physical therapists, occupational therapists and athletic trainers across nine specialties and subspecialties with over 350 staff.
The acquisition is effective on Jan. 1, 2027, with CNOS being named Avera CNOS going forward.
“We had a long and thoughtful process, which included input from our stakeholders, on what CNOS was looking for in a partner,” said Nolan Lubarski, Chief Executive Officer of CNOS. “It became very clear early in our conversations that our visions for the future of healthcare in this region were aligned. Avera and CNOS share similar values, including a strong commitment to clinical excellence, patient-centered care and community service. We are excited to be part of this growing organization.”
CNOS formed in the 1990s and offers comprehensive care to the Siouxland region in Northwest Iowa. The group has grown through strategic alignments and mergers with other specialty practices, expanding clinical capabilities and providers. Today, CNOS says it provides outreach services to 29 satellite locations

Avera currently has two owned and seven partner hospitals in Iowa, as well as 15 clinics and several outreach locations for specialty care.
According to Avera, this acquisition expands its specialty care offerings and better coordinates services to provide a more integrated care experience that better supports the growing Northwest Iowa region.
“Avera has built a great network of clinics and hospitals across the Siouxland region. These locations already collaborate closely with the physicians at CNOS,” said David Flicek, Chief Operating Officer with Avera. “This group is well known for excellence in quality outcomes and patient care. We are excited to welcome them to Avera and look forward to providing even greater continuity of care for the patients and communities we serve.”
CNOS said it will start contacting patients on Wednesday, July 15, notifying them of this change.
“Our culture is defined by the dedication and compassion of our providers and employees,” said Lubarski. “Because our organizations share the same commitment, this relationship is a natural fit. Together, we’ll strengthen our mission and expand our ability to deliver exceptional care to more people.”
