The following results are based on these 103 responses. Table <a data-track=\"click\" data-track-label=\"link\" data-track-action=\"table anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Tab1\" rel=\"nofollow noopener\" target=\"_blank\">1<\/a> provides the aggregated result to the survey questions and interactive visualizations of the results are also provided on the Bone2Gene website.<\/p>\n<p>Demography<\/p>\n<p>Out of a total of 103 complete survey responses 23 individuals (22%) describe themselves as involved in the diagnostic process, 6 (6%) in pre- and post-diagnostic care, and 69 (67%) are involved in both. The remaining 5 (5%) are only involved in research activities in this field. Ninety-two (89%) of the participants are physicians. The majority of participants (84, 81%) primarily work at academic medical centers (Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"figure anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Fig1\" rel=\"nofollow noopener\" target=\"_blank\">1<\/a>A) and have more than 10\u00a0years of experience (also 84, 81%, Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"figure anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Fig1\" rel=\"nofollow noopener\" target=\"_blank\">1<\/a>B). Ninety-six (93%) of the respondents are involved in teaching or training other healthcare professionals (Table\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"table anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Tab1\" rel=\"nofollow noopener\" target=\"_blank\">1<\/a> and Supplementary Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"supplementary material anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#MOESM1\" rel=\"nofollow noopener\" target=\"_blank\">1<\/a>).<\/p>\n<p>Fig.\u00a01<a class=\"c-article-section__figure-link\" data-test=\"img-link\" data-track=\"click\" data-track-label=\"image\" data-track-action=\"view figure\" href=\"https:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1\/figures\/1\" rel=\"nofollow noopener\" target=\"_blank\"><img decoding=\"async\" aria-describedby=\"Fig1\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2025\/07\/13023_2025_3875_Fig1_HTML.png\" alt=\"figure 1\" loading=\"lazy\" width=\"685\" height=\"602\"\/><\/a><\/p>\n<p>Demographics of the 103 participants who completed the survey. C The distribution of participants on the world map; the circle sizes reflect the number of participants (see the Supplementary information for the exact number per each of the 27 countries)<\/p>\n<p>Survey participants represented most global regions (Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"figure anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Fig1\" rel=\"nofollow noopener\" target=\"_blank\">1<\/a>C) and a total of 27 different countries (Supplementary Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"supplementary material anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#MOESM1\" rel=\"nofollow noopener\" target=\"_blank\">2<\/a>). Fifty-eight (56%) of the respondents are located in Europe and 30 (29%) are in the United States and Canada.<\/p>\n<p>The diagram in Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"figure anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Fig2\" rel=\"nofollow noopener\" target=\"_blank\">2<\/a> shows the distribution of the responses from the 92 physician participants to the question \u201cWhich title best describes your position?\u201d. Participants could indicate multiple specialties. The most frequent groups were medical geneticists, pediatricians, and endocrinologists, together accounting for 71 (69%) of the respondents.<\/p>\n<p>Fig.\u00a02<a class=\"c-article-section__figure-link\" data-test=\"img-link\" data-track=\"click\" data-track-label=\"image\" data-track-action=\"view figure\" href=\"https:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1\/figures\/2\" rel=\"nofollow noopener\" target=\"_blank\"><img decoding=\"async\" aria-describedby=\"Fig2\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2025\/07\/13023_2025_3875_Fig2_HTML.png\" alt=\"figure 2\" loading=\"lazy\" width=\"685\" height=\"395\"\/><\/a><\/p>\n<p>Specialties of the physician participants (n\u2009=\u200992). The most common specialty represented amongst participants was medical genetics. Some participants selected multiple specialties (e.g., endocrinologists who also practice pediatrics, n\u2009=\u200910). Other specialties represented include nephrologist, internist, rheumatologist, pediatric physiatrist, maternal\u2013fetal medicine, general surgeon, pathologist, and geriatrician. We note that countries may have variable requirements regarding initial (general) training versus allowing clinicians to enter a subspecialty immediately, and some clinicians may have responded regarding their current practice rather than their entire training<\/p>\n<p>Patients statistics<\/p>\n<p>In this section, we report the responses to the questions related to the RBD patients seen by the participants. Fifty respondents (48%) see patients from all four age groups described in our questionnaire, namely 0\u20131,1\u201310,10\u201318, and greater than 18\u00a0years old. Further, most of the respondents see patients below 18\u00a0years old but 63 (61%) of them also see patients 18\u00a0years old or above (Table\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"table anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Tab1\" rel=\"nofollow noopener\" target=\"_blank\">1<\/a> and Supplementary Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"supplementary material anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#MOESM1\" rel=\"nofollow noopener\" target=\"_blank\">3<\/a>).<\/p>\n<p>The number of patients with known or suspected rare bone diseases seen by the facilities where the respondents work follows a roughly bell-shaped distribution (Supplementary Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"supplementary material anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#MOESM1\" rel=\"nofollow noopener\" target=\"_blank\">3<\/a>). Sixty-four (62%) of the facilities see 10\u2013100, seventeen (16%) see less than 10, and twenty-one (20%) see more than 100 patients per month.<\/p>\n<p>According to the most recent revision of the nosology of RBDs [<a data-track=\"click\" data-track-action=\"reference anchor\" data-track-label=\"link\" data-test=\"citation-ref\" aria-label=\"Reference 1\" title=\"Unger S, Ferreira CR, Mortier GR, Ali H, Bertola DR, Calder A, et al. Nosology of genetic skeletal disorders: 2023 revision. Am J Med Genet A. 2023;191:1164\u2013209. &#010;                  https:\/\/doi.org\/10.1002\/ajmg.a.63132&#010;                  &#010;                .\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#ref-CR1\" id=\"ref-link-section-d9796931e2159\" rel=\"nofollow noopener\" target=\"_blank\">1<\/a>] there are 41 different groups of skeletal disorders. In one of the questions in our survey, we asked the participants to mark all the groups that they or their healthcare facility have dealt with. The results are visualized in Supplementary Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"supplementary material anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#MOESM1\" rel=\"nofollow noopener\" target=\"_blank\">4<\/a>. The most frequently observed disorder was the Osteogenesis imperfecta and bone fragility group, with 89% (n\u2009=\u200992) of participants reporting care of patients with this group of conditions at their institution. This was followed by Disorders of bone mineralization (n\u2009=\u200989, 86%) and FGFR3 chondrodysplasias (n\u2009=\u200988, 85%).<\/p>\n<p>Role of imaging in diagnosing RBDs<\/p>\n<p>Ninety-four (91%) of the respondents find imaging to be very or extremely important, and the majority (n\u2009=\u200984, 81%) consider X-rays to be the most important imaging modality (Table\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"table anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Tab1\" rel=\"nofollow noopener\" target=\"_blank\">1<\/a> and Supplementary Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"supplementary material anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#MOESM1\" rel=\"nofollow noopener\" target=\"_blank\">5<\/a>).<\/p>\n<p>Further, our result (Table\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"table anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Tab1\" rel=\"nofollow noopener\" target=\"_blank\">1<\/a> and Supplementary Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"supplementary material anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#MOESM1\" rel=\"nofollow noopener\" target=\"_blank\">6<\/a>) shows that most participants, 83% (n\u2009=\u200986) regardless of years of experience, find the interpretation of patients\u2019 radiographs and delineation of different RBDs based on visual inspection to be somewhat (n\u2009=\u200959) or extremely difficult (n\u2009=\u200927).<\/p>\n<p>Participants\u2019 perception of the use of AI<\/p>\n<p>Forty-four percent of the participants (45\/103) are somewhat (n\u2009=\u200931) or very (n\u2009=\u200914) concerned about AI-related errors, while 24% (n\u2009=\u200925) are not very concerned (Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"figure anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Fig3\" rel=\"nofollow noopener\" target=\"_blank\">3<\/a>A). Around half of the participants (n\u2009=\u200951) consider the explainability of AI algorithms to be very (42\/103) or extremely (9\/103) important (Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"figure anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Fig3\" rel=\"nofollow noopener\" target=\"_blank\">3<\/a>B). There was no relation between the years of experience of the participants and their answers to these questions.<\/p>\n<p>Fig.\u00a03<a class=\"c-article-section__figure-link\" data-test=\"img-link\" data-track=\"click\" data-track-label=\"image\" data-track-action=\"view figure\" href=\"https:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1\/figures\/3\" rel=\"nofollow noopener\" target=\"_blank\"><img decoding=\"async\" aria-describedby=\"Fig3\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2025\/07\/13023_2025_3875_Fig3_HTML.png\" alt=\"figure 3\" loading=\"lazy\" width=\"685\" height=\"546\"\/><\/a><\/p>\n<p>Responses of the participants regarding A concerns about potential AI errors, B importance of explainability, C regulatory considerations, and D willingness to undergo further training for using AI<\/p>\n<p>On the subject of regulatory considerations or ethical concerns (Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"figure anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Fig3\" rel=\"nofollow noopener\" target=\"_blank\">3<\/a>C), the interpretability of AI algorithms received the most attention (n\u2009=\u200969) followed by data privacy and security (n\u2009=\u200964). Ninety-three (90%) of the participants\u00a0indicated they would be willing to do additional training for utilizing AI (Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"figure anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Fig3\" rel=\"nofollow noopener\" target=\"_blank\">3<\/a>D), and when asked how likely they are to consider integrating image recognition AI into their current diagnostic workflow, 81% responded they were somewhat (n\u2009=\u200939) or extremely (n\u2009=\u200945) likely (Fig.\u00a0<a data-track=\"click\" data-track-label=\"link\" data-track-action=\"figure anchor\" href=\"http:\/\/ojrd.biomedcentral.com\/articles\/10.1186\/s13023-025-03875-1#Fig4\" rel=\"nofollow noopener\" target=\"_blank\">4<\/a>).
