{"id":337267,"date":"2025-12-08T16:49:20","date_gmt":"2025-12-08T16:49:20","guid":{"rendered":"https:\/\/www.newsbeep.com\/us\/337267\/"},"modified":"2025-12-08T16:49:20","modified_gmt":"2025-12-08T16:49:20","slug":"life-and-death-stakes-of-the-debate-over-aca-subsidies","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/us\/337267\/","title":{"rendered":"Life-and-death stakes of the debate over ACA subsidies"},"content":{"rendered":"<p>The government shutdown may be over, but Congress still hasn\u2019t solved the biggest problem left on its plate: Extend the expiring Affordable Care Act subsidies to avoid a\u00a0<a href=\"https:\/\/www.kff.org\/quick-take\/aca-insurers-are-raising-premiums-by-an-estimated-26-but-most-enrollees-could-see-sharper-increases-in-what-they-pay\/\" target=\"_blank\" rel=\"noopener nofollow\">doubling<\/a> of insurance premiums or replace them with something new altogether. Lawmakers have committed to a vote <a href=\"https:\/\/apnews.com\/article\/shutdown-health-tax-credits-affordable-care-act-2b5ae3651ff16783a00e00dc1ce264bf\" rel=\"nofollow noopener\" target=\"_blank\">in December<\/a>.<\/p>\n<p>While<a href=\"https:\/\/www.nytimes.com\/2025\/10\/07\/us\/politics\/shutdown-trump-health-care-subsidies.html\" target=\"_blank\" rel=\"noopener nofollow\"> public debate<\/a> about the issue has centered on dollars and deficits, the stakes are actually far greater: Access to comprehensive health coverage can determine whether people live or die.<\/p>\n<p>A new idea is now gaining traction among some lawmakers: Let the subsidies expire and replace them with <a href=\"https:\/\/www.cnbc.com\/2025\/11\/17\/aca-tax-credits-hsa-cassidy-obamacare-congress.html\" target=\"_blank\" rel=\"noopener nofollow\">federal deposits into health savings accounts<\/a>. These deposits wouldn\u2019t lower the cost of buying a plan. Instead, the proposal assumes that people would purchase cheaper, limited coverage \u2014 such as high-deductible or <a href=\"https:\/\/www.youtube.com\/watch?v=q7UyJ51vbZk\" rel=\"nofollow noopener\" target=\"_blank\">catastrophic<\/a> plans, which often come with deductibles around $6,000 \u2014 and then use the HSA funds to help pay the medical bills those plans don\u2019t cover. <\/p>\n<p>But HSA deposits do nothing to prevent the real harm. The health consequences of losing insurance \u2014 or of having insurance that is difficult to affordably use \u2014 are<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/39792985\/\" target=\"_blank\" rel=\"noopener nofollow\"> well documented<\/a>. A substantial body of\u00a0<a href=\"https:\/\/www.acpjournals.org\/doi\/10.7326\/M17-1403\" target=\"_blank\" rel=\"noopener nofollow\">research<\/a> shows a clear relationship between lack of comprehensive coverage and higher death rates.<\/p>\n<p>The U.S. Supreme Court once <a href=\"https:\/\/supreme.justia.com\/cases\/federal\/us\/17\/316\/\" target=\"_blank\" rel=\"noopener nofollow\">observed<\/a> that \u201cthe power to tax involves the power to destroy.\u201d In the context of health insurance, the converse is true: Withdrawing support for comprehensive coverage can be destructive as well. It affects not just how people pay for care but also whether they receive care at all.<\/p>\n<p>One of the clearest demonstrations of this comes from a randomized study in which researchers partnered with the Internal Revenue Service to send informational letters about health insurance to millions of households. Only some households received a letter, and because the letters significantly increased enrollment, the researchers could reliably measure the effect of gaining coverage.\u00a0<\/p>\n<p>The findings were striking:\u00a0<a href=\"https:\/\/academic.oup.com\/qje\/article\/136\/1\/1\/5911132\" target=\"_blank\" rel=\"noopener nofollow\">For every 52 people<\/a> who gained health insurance, one life was saved. Scaled to the 3.8 million people projected to lose insurance by the expiring subsidies, the potential death toll reaches into the tens of thousands \u2014 even if the real-world effect is only a fraction of the study\u2019s estimate.<\/p>\n<p>Why does comprehensive health insurance matter so much? There are several proven ways that having meaningful coverage improves health.\u00a0<\/p>\n<p>One is the ability to seek timely care when something feels seriously wrong. Studies\u00a0<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/15111915\/\" target=\"_blank\" rel=\"noopener nofollow\">show<\/a> that people without coverage often delay or avoid going to the hospital when they have serious symptoms such as chest pain or shortness of breath because of cost. These delays can be deadly: Timely treatment for heart attacks, strokes and other emergencies is one of the most consistent ways insurance reduces mortality.<\/p>\n<p>Even when emergencies are covered after the deductible, the fact that patients must pay thousands of dollars out of pocket first leads many to avoid seeking care altogether \u2014 and catastrophic plans would amplify this effect. Studies of high-deductible health plans show enrollees <a href=\"https:\/\/newsroom.heart.org\/news\/people-with-high-deductible-health-plans-less-likely-to-seek-er-treatment-for-chest-pain\" target=\"_blank\" rel=\"noopener nofollow\">delay evaluation of chest pain<\/a>,<a href=\"https:\/\/www.ahajournals.org\/doi\/10.1161\/CIRCULATIONAHA.120.052501\" rel=\"nofollow noopener\" target=\"_blank\"> avoid emergency departments<\/a> for concerning symptoms, and experience<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/37750565\/\" target=\"_blank\" rel=\"noopener nofollow\"> worse outcomes in heart attacks<\/a> and<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/29382660\/\" target=\"_blank\" rel=\"noopener nofollow\"> diabetic crises<\/a>.\u00a0<\/p>\n<p>Insurance also plays a crucial role in catching serious diseases while they are still treatable. People are<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10984378\" target=\"_blank\" rel=\"noopener nofollow\"> far more likely to seek preventive care<\/a> when insurance makes it affordable (or as our own research has found,<a href=\"https:\/\/papers.ssrn.com\/abstract=5316312\" target=\"_blank\" rel=\"noopener nofollow\"> when insurance makes it free<\/a>). A JAMA Oncology<a href=\"https:\/\/jamanetwork.com\/journals\/jamaoncology\/fullarticle\/2758266\" target=\"_blank\" rel=\"noopener nofollow\"> study<\/a> of 177,075 women found those without private insurance were much more likely to be diagnosed with late-stage cancer, resulting in survival rates far lower than women with comprehensive coverage.<\/p>\n<p>Just as important is what happens in the day-to-day management of chronic conditions. A large body of <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/30628493\/\" target=\"_blank\" rel=\"noopener nofollow\">research<\/a> \u2014 including our <a href=\"https:\/\/onlinelibrary.wiley.com\/doi\/full\/10.1111\/jels.12368\" target=\"_blank\" rel=\"noopener nofollow\">own<\/a> \u2014 shows uninsured and underinsured people often delay or skip this routine care because of cost, even when they know it will worsen their health. High deductibles re-create this dynamic: Patients routinely forgo essential medications, diabetes management or blood-pressure checks until they meet thousands of dollars in out-of-pocket expenses. <\/p>\n<p>Federal HSA deposits help only at the margins; a modest contribution cannot meaningfully offset a $6,000 deductible. Cost-sharing continues to shape behavior, leading many, and especially those with limited means, to postpone care until it is too late.<\/p>\n<p>As Congress approaches its December vote, it is essential to consider not only the budgetary implications but also the extensive evidence linking insurance to health and survival. The research record is clear: The affordability of real, comprehensive insurance is not an abstract policy question.
