my doctor gave a shot of socialism with my annual exam"},"content":{"rendered":"<p>Why is my doctor worried about my mortgage payments?<\/p>\n<p>So I found myself wondering as I filled out a questionnaire before a recent primary-care visit.<\/p>\n<p>Along with the usual prompts about my health, I encountered a series of questions related to my financial situation.<\/p>\n<p>The form asked whether in the last 12 months I was unable to pay my mortgage or rent on time, and if I was \u201cworried that my food would run out before I got the money to buy more.\u201d<\/p>\n<p>These questions weren\u2019t an attempt to gauge whether I would pay my medical bill; they were part of a campaign driven by political activists out to justify socialist policies <a href=\"https:\/\/nypost.com\/2024\/04\/11\/opinion\/government-programs-are-widening-black-white-health-disparities\/\" rel=\"nofollow noopener\" target=\"_blank\">in the name of improving health care<\/a>.\u00a0<\/p>\n<p>Federal rules\u00a0issued by the Centers for Medicare and Medicaid Services actually give doctors a financial incentive to ask these questions.\u00a0<\/p>\n<p>In other words, medicine is one more institution that has been co-opted to <a href=\"https:\/\/nypost.com\/2024\/09\/15\/opinion\/new-study-finds-medical-schools-value-activism-over-science\/\" rel=\"nofollow noopener\" target=\"_blank\">advance a leftist party line<\/a>.<\/p>\n<p>The questions I was asked reflect the activist obsession with what\u2019s known as the \u201cSocial Determinants of Health.\u201d<\/p>\n<p>The way the argument goes, factors like income, education, neighborhood conditions, racism, access to transportation and so on heavily determine a patient\u2019s health.<\/p>\n<p>The\u00a0World Health Organization\u00a0and the\u00a0Centers for Disease Control\u00a0repeatedly assert that these factors are major drivers of disparities in\u00a0life expectancy,\u00a0infant mortality,\u00a0diabetes rates and\u00a0cardiovascular disease\u00a0across racial and socioeconomic groups.\u00a0<\/p>\n<p>But it\u2019s not true: The framework is based on critical logical and empirical fallacies.<\/p>\n<p>It confuses correlation with causation, while systematically downplaying or ignoring biological, behavioral and genetic variables that often have far more to do with differences in patient health outcomes.<\/p>\n<p>But that\u2019s what you\u2019d expect from a notion that has nothing to do with medicine \u2014 and <a href=\"https:\/\/nypost.com\/2024\/02\/17\/opinion\/americas-medical-system-has-become-totally-woke\/\" rel=\"nofollow noopener\" target=\"_blank\">everything to do with politics<\/a>.<\/p>\n<p>A few examples show the absurdity of the activist enterprise.<\/p>\n<p>Under this leftist framework, people with lower incomes must necessarily have worse health outcomes.<\/p>\n<p>Yet Hispanic Americans have a higher life expectancy than whites despite their lower average incomes,\u00a0according to federal data.<\/p>\n<p>That\u2019s a fact of life that activists ignore, because it doesn\u2019t support their plan to divvy up money along racial or socioeconomic lines.<\/p>\n<p>Or consider that black Americans have higher rates of hypertension and stroke.<\/p>\n<p>Those who believe that social determinants control for health say <a href=\"https:\/\/nypost.com\/2019\/11\/05\/non-white-cancer-survivors-less-likely-to-get-doctors-they-need\/\" rel=\"nofollow noopener\" target=\"_blank\">discrimination must be the cause<\/a> \u2014 that racism leads to higher poverty rates in black communities, hurting health in turn \u2014 yet when researchers investigate hypertension among black adults, they find lifestyle choices like diet\u00a0are the more reality-based factors.<\/p>\n<p>Activists don\u2019t acknowledge that truth, because it doesn\u2019t justify their demand for redistribution.<\/p>\n<p>The willful ignorance of evidence is part and parcel of the activist worldview.<\/p>\n<p>\t\t\t\t\t\t\tGet opinions and commentary from our up!\n\t\t\t\t<\/p>\n<p>Behavioral factors \u2014 diet, physical activity, tobacco use, alcohol consumption and so on \u2014 consistently explain\u00a0much of the variance in chronic disease outcomes, dwarfing the effects of income or education.<\/p>\n<p>The\u00a0Framingham Heart Study\u00a0and numerous cohort studies repeatedly find that modifiable behaviors dominate risk, even in impoverished populations.<\/p>\n<p>People who choose not to smoke, who eat vegetables, and who exercise regularly have better cardiovascular outcomes than those who do none of those things.<\/p>\n<p>Social conditions can make healthy behavior harder, but they don\u2019t make it impossible \u2014 and they certainly don\u2019t deposit plaque into arteries.<\/p>\n<p>And the belief that health is socially determined does real damage.<\/p>\n<p>Insisting that health disparities are largely if not entirely structural removes agency from individuals, while absolving personal responsibility.<\/p>\n<p>This not only misdiagnoses the real causes of health disparities, it also justifies expensive, marginally e\ufb00ective socialist policies.<\/p>\n<p>Case in point: The CDC\u2019s \u201cHealth Impact in 5 Years\u201d initiative\u00a0explicitly lists\u00a0income supplements and public transportation as health interventions.\u00a0<\/p>\n<p>Yet\u00a0evidence from major health providers shows\u00a0that financial support has no meaningful effect on health outcomes.<\/p>\n<p>That includes universal basic income programs, a major leftist priority \u2014 which, evidence shows, <a href=\"https:\/\/nypost.com\/2024\/07\/24\/opinion\/no-strings-cash-leaves-the-poor-worse-off-study\/\" rel=\"nofollow noopener\" target=\"_blank\">have no impact on health<\/a>.<\/p>\n<p>But this isn\u2019t about medicine.<\/p>\n<p>It\u2019s about redistribution \u2014 and even racial reparations \u2014 masquerading as health care.<\/p>\n<p>The activist endgame is obvious: Once health outcomes are attributed primarily to income gaps, any disparity justifies \u2014 even demands \u2014 government intervention.<\/p>\n<p>Taxation, seizure, and compelled transfer of wealth then cease to be economic questions; they become public-health emergencies on par with an epidemic.<\/p>\n<p>But they\u2019re not.<\/p>\n<p>President Donald Trump should repeal the federal rules that encourage this abuse of medicine.<\/p>\n<p>The social-determinants framework supplies the moral and scientific veneer for a permanent machinery of wealth distribution.<\/p>\n<p>Remember that the next time your doctor asks if you can afford your mortgage.<\/p>\n<p>It\u2019s not a matter of medicine, but a matter of leftist politics \u2014 and you\u2019re the lab rat.<\/p>\n<p>Dr. Stanley Goldfarb is chairman
