Amid <a href=\"https:\/\/www.marketplace.org\/story\/2025\/07\/21\/ag-sector-economic-activity-described-as-weak\" rel=\"nofollow noopener\" target=\"_blank\">falling prices for commodity crops<\/a> such as corn and soybeans, <a href=\"https:\/\/www.pbs.org\/newshour\/show\/trump-proposes-12-billion-in-aid-to-farmers-after-exceptionally-difficult-year\" rel=\"nofollow noopener\" target=\"_blank\">rising input costs<\/a> for supplies like <a href=\"https:\/\/farmpolicynews.illinois.edu\/2025\/09\/doj-usda-to-examine-rising-farm-input-costs\/\" rel=\"nofollow noopener\" target=\"_blank\">fertilizer and seeds<\/a>, as well as the Trump tariffs and the <a href=\"https:\/\/www.npr.org\/2025\/02\/27\/nx-s1-5311663\/farmers-will-be-hit-hard-by-the-dismantling-of-usaid\" rel=\"nofollow noopener\" target=\"_blank\">dismantling of USAID<\/a>, many farms weren\u2019t profitable last year.<\/p>\n<p>And now, the enhanced Affordable Care Act subsidies that many Americans, including farmers, relied on to purchase health insurance are gone, having <a href=\"https:\/\/www.npr.org\/sections\/shots-health-news\/2025\/08\/22\/nx-s1-5511182\/aca-tax-credits-health-insurance-open-enrollment\" rel=\"nofollow noopener\" target=\"_blank\">expired at the end of December<\/a>.<\/p>\n<p>James Davis, 55, who grows cotton, soybeans, and corn in northern Louisiana, said he didn\u2019t know how he and his wife would afford coverage. Their share of their insurance premium quadrupled for 2026, jumping to about $2,700 a month.<\/p>\n<p>\u201cYou can\u2019t afford it,\u201d Davis said. \u201cBottom line. There\u2019s nothing to discuss. You can\u2019t afford it without the subsidies.\u201d<\/p>\n<p>More than a quarter of the agricultural workforce purchases health insurance through the individual marketplace, according to <a href=\"https:\/\/www.kff.org\/affordable-care-act\/occupations-with-large-shares-of-workers-who-rely-on-individual-market-coverage\/\" rel=\"nofollow noopener\" target=\"_blank\">an analysis from KFF<\/a>, a health information nonprofit that includes KFF Health News.<\/p>\n<p>That 27% rate is much higher than the overall population\u2019s \u2014 only <a href=\"https:\/\/www.kff.org\/uninsured\/health-policy-101-the-uninsured-population-and-health-coverage\/?entry=table-of-contents-what-is-the-landscape-of-health-care-coverage-in-the-united-states\" rel=\"nofollow noopener\" target=\"_blank\">6% of U.S. adults have non-group coverage<\/a>.<\/p>\n<p>Farmers are used to facing challenges such as unpredictable weather and fluctuating commodity prices. But the loss of the enhanced subsidies, coupled with challenging economic conditions, will make coverage unaffordable for many.<\/p>\n<p>Without major intervention from Washington, farmers say they\u2019ll have to choose between being uninsured or leaving the farm work behind for a job that offers health insurance.<\/p>\n<\/p>\n<p>A Gamble for Farmers<\/p>\n<p>Farming is dangerous work. Agricultural workers spend much of their time outside and exposed to the elements. Many of their duties can lead to injury or illness. They drive and operate heavy machinery, work with toxic chemicals, and handle large animals.<\/p>\n<p>The rate of work-related deaths for farmers is <a href=\"https:\/\/extension.missouri.edu\/news\/farming-the-most-dangerous-job-in-the-us\" rel=\"nofollow noopener\" target=\"_blank\">seven times the national average<\/a>.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" width=\"1776\" height=\"1184\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2026\/01\/Cotton.jpg\" alt=\"A close-up of a cotton plant growing on a farm.\" class=\"wp-image-2145426\"  \/>Cotton grows on a farm in Richland Parish in northern Louisiana. Like many farmers, he is seeing his health insurance costs soar.(Drew Hawkins\/Gulf States Newsroom)<\/p>\n<p>The financial toll of non-fatal farm injuries is also significant. <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/38922747\/\" rel=\"nofollow noopener\" target=\"_blank\">A study<\/a> from the University of Nebraska Medical Center found that the average cost of a farming injury was $10,878 in medical care and $4,735 in lost work.<\/p>\n<p>It\u2019s essential that farmers can purchase comprehensive insurance, said <a href=\"https:\/\/abe.psu.edu\/directory\/ffb5072\" rel=\"nofollow noopener\" target=\"_blank\">Florence Becot<\/a>, a rural sociologist and associate professor of agricultural health and safety at Pennsylvania State University, where she studies the social and economic needs of farm households.<\/p>\n<p>In a <a href=\"https:\/\/link.springer.com\/article\/10.1007\/s10460-022-10307-4\" rel=\"nofollow noopener\" target=\"_blank\">2022 study<\/a>, Becot found that more than 20% of U.S. farm households had medical debt exceeding $1,000 and that more than half were not confident they could cover the costs of a major illness or injury.<\/p>\n<p>\u201cThat shows you the level of vulnerability and concerns that farmers are facing,\u201d she said.<\/p>\n<p>Mental health is also a concern. <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/37449301\/\" rel=\"nofollow noopener\" target=\"_blank\">Farmers are roughly twice<\/a> as likely to die by suicide compared with the general population. Mental health hotlines that serve rural communities have <a href=\"https:\/\/pbswisconsin.org\/news-item\/how-farmers-at-risk-for-suicide-can-find-mental-health-help\/\" rel=\"nofollow noopener\" target=\"_blank\">seen<\/a> an <a href=\"https:\/\/www.minnpost.com\/mental-health-addiction\/2025\/12\/as-farmers-face-trade-wars-and-inflation-calls-to-mental-health-hotlines-rise\/\" rel=\"nofollow noopener\" target=\"_blank\">uptick<\/a> in calls.<\/p>\n<p>These concerns around farmers\u2019 <a href=\"https:\/\/investigatemidwest.org\/2020\/03\/05\/seeds-of-despair-isolated-and-with-limited-access-to-mental-health-care-hundreds-are-dying-by-suicide\/\" rel=\"nofollow noopener\" target=\"_blank\">increased emotional distress<\/a>, coupled with a <a href=\"https:\/\/farmpolicynews.illinois.edu\/2025\/07\/farm-bankruptcies-this-year-already-exceed-2024-levels\/\" rel=\"nofollow noopener\" target=\"_blank\">rise in bankruptcies<\/a>, conjures memories of the farm crisis of the 1980s, said <a href=\"https:\/\/www.usarice.com\/about-us\/usa-rice-staff\" rel=\"nofollow noopener\" target=\"_blank\">Michael Klein<\/a>, a vice president at the industry group <a href=\"https:\/\/www.usarice.com\/\" rel=\"nofollow noopener\" target=\"_blank\">USA Rice<\/a>. During that decade, there was a raft of foreclosures, and <a href=\"https:\/\/www.nytimes.com\/1991\/10\/14\/us\/farmer-suicide-rate-swells-in-1980-s-study-says.html\" rel=\"nofollow noopener\" target=\"_blank\">hundreds of farmers took their own lives<\/a>.<\/p>\n<p>\u201cWe\u2019re really afraid of what\u2019s going to happen,\u201d Klein said.<\/p>\n<p>\t\t<a href=\"https:\/\/kffhealthnews.org\/email\/\" rel=\"nofollow noopener\" target=\"_blank\"><br \/>\n\t\t\tEmail Sign-Up\t\t<\/a><\/p>\n<p class=\"newsletter__description\">\n\t\tSubscribe to KFF Health News&#8217; free Morning Briefing.\t<\/p>\n<p>Farmers can be reluctant to acknowledge that they rely on government-subsidized insurance, said Meghan Palmer, 43, who runs a dairy farm in northeastern Iowa with her husband, John, 45.<\/p>\n<p>\u201cWe\u2019re not handout-takers,\u201d Palmer said.<\/p>\n<p>More than 40% of dairy farmers lack health insurance \u2014 <a href=\"https:\/\/farms.extension.wisc.edu\/safeguarding-your-well-being-and-your-bottom-line-with-health-insurance-2\/\" rel=\"nofollow noopener\" target=\"_blank\">one of the highest rates<\/a> among all agricultural sectors.<\/p>\n<p>But going uninsured is not an option for the Palmers.<\/p>\n<p>During their first year of marriage, the couple recalled, they were uninsured and had to pay out-of-pocket for two unexpected health crises: Palmer had an appendectomy, and her husband needed stitches after getting kicked in the face by one of his cows.<\/p>\n<p>\u201cIt was stupid of us,\u201d Palmer said of the decision to forgo coverage.<\/p>\n<p>But this year, the combined out-of-pocket monthly cost of their plans is increasing by more than 90%, to $368.18. Their total 2026 deductible is $7,200.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" width=\"1075\" height=\"804\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2026\/01\/Palmer-Cows.jpg\" alt=\"A row of cows standing in a grassy field.\" class=\"wp-image-2145427\"  \/>Prairie Star Farm has been in Palmer\u2019s family for three generations. She hopes that someday one of her kids will want to take over operations, but it\u2019s getting harder for a farm to be profitable.(Meghan Palmer)<\/p>\n<p>Palmer is a registered nurse who picks up shifts on an as-needed basis, allowing her the flexibility to prioritize her work on the farm. She\u2019s now searching for a job with health benefits. But she worries a job that doesn\u2019t allow her to keep up with the farm work will create a greater burden for her husband.<\/p>\n<p>\u201cJohn is working exhausted most of the time,\u201d she said. \u201cThat\u2019s when mistakes get made and you end up in the ER.\u201d<\/p>\n<p>Political Consequences<\/p>\n<p>Even after the enhanced subsidies expired at the end of 2025, the Palmers estimate their income will still be low enough that they\u2019ll qualify for some tax credits to purchase coverage.<\/p>\n<p>However, under the GOP\u2019s One Big Beautiful Bill Act, <a href=\"https:\/\/www.kff.org\/affordable-care-act\/8-things-to-watch-for-the-2026-aca-open-enrollment-period\/#:~:text=Starting%20in%20the%202026%20plan,minimize%20repayments%20at%20tax%20time.\" rel=\"nofollow noopener\" target=\"_blank\">repayment limits are being eliminated<\/a>, so if the Palmers have a surprisingly profitable 2026, they\u2019ll be forced to pay some, or even all, of that subsidy back at tax time.<\/p>\n<p>A farmer\u2019s income can vary drastically year to year, Becot said, partly because commodity prices can fluctuate rapidly.<\/p>\n<p>Some farmers might deliberately choose to not expand their businesses, because too much profit might mean they lose access to health care subsidies.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" width=\"3596\" height=\"2397\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2026\/01\/Tractor.jpg\" alt=\"A tractor parked in a field.\" class=\"wp-image-2145423\"  \/>A tractor in Richland Parish. But prioritizing health care affordability by suppressing operational growth can have long-term consequences for a farm\u2019s success.<\/p>\n<p>Palmer, in Iowa, and Davis, in Louisiana, are both upset that lawmakers aren\u2019t more sensitive to the economic demands of farming and how those have coincided with rising health costs.<\/p>\n<p>President Donald Trump recently pledged $12 billion in one-time <a href=\"https:\/\/www.usda.gov\/about-usda\/news\/press-releases\/2025\/12\/08\/trump-administration-announces-12-billion-farmer-bridge-payments-american-farmers-impacted-unfair\" rel=\"nofollow noopener\" target=\"_blank\">bridge payments<\/a> to row crop farmers, but that\u2019s not going to stop health care costs from ballooning.<\/p>\n<p>Republicans are aware that health care affordability is a problem and have put forth proposals, said <a href=\"https:\/\/csbs.uni.edu\/polisci\/directory\/donna-hoffman\" rel=\"nofollow noopener\" target=\"_blank\">Donna Hoffman<\/a>, a political scientist at the University of Northern Iowa.
