Here&#8217;s why it&#8217;s happening and what you can do about it"},"content":{"rendered":"\n<p class=\"yf-vbsvxt\">Many Americans assume they\u2019ll only get a hospital bill for medical care that takes place in a hospital.<\/p>\n<p class=\"yf-vbsvxt\">But a growing number of Americans are surprised to see hospital charges show up after visits to their regular physician or family practice \u2014 and even telehealth appointments taking place from their own homes.<\/p>\n<p class=\"yf-vbsvxt\">This is happening to patients whose regular physician\u2019s office or clinic is owned by a hospital, even if they\u2019re not located in one. As WFTV Channel 9 reports, the charges show up as \u201chospital facility fees\u201d (1).<\/p>\n<p class=\"yf-vbsvxt\">\u201cThey\u2019re going to the same doctor in the same building, same boring waiting room, but now unbeknownst to them, that physician practice is owned by a hospital system,\u201d Patricia Kelmar, with U.S. Public Interest Research Group (PIRG), told the media outlet.<\/p>\n<p class=\"yf-vbsvxt\">These facility fees can add anywhere from tens to hundreds of dollars to the cost of routine medical care.<\/p>\n<p class=\"yf-vbsvxt\">PIRG\u2019s recent report, Outpatient Outrage 2026, found that these fees generate hundreds of millions of dollars in additional revenue while offering no clear added benefit to patients (2).<\/p>\n<p class=\"yf-vbsvxt\">Rebecca Smith, a breast cancer survivor in Mooresville, North Carolina, was shocked the first time she was hit with a $400 bill for an office visit and blood draw that normally cost $75. This happened after a hospital system bought her local oncology practice.<\/p>\n<p class=\"yf-vbsvxt\">\u201cIt was like a bait-and-switch,\u201d Smith told WFAE 90.7 (3). \u201cI\u2019m having the same follow-up appointment I always have \u2014 and then, all of a sudden, they changed the billing. I felt like what they did was really dirty.\u201d<\/p>\n<p class=\"yf-vbsvxt\">Here\u2019s what\u2019s behind the fees and how to protect yourself.<\/p>\n<p class=\"yf-vbsvxt\">Facility fees were originally designed to help hospitals cover operating costs of emergency rooms, intensive care units and other around-the-clock services running out of hospitals themselves.<\/p>\n<p class=\"yf-vbsvxt\">But over time, hospitals started applying these charges to facilities beyond hospital walls.<\/p>\n<p class=\"yf-vbsvxt\">Today, when hospitals acquire physician practices outside the hospital they can bill insurers and patients at higher \u201chospital\u201d rates, even when the care hasn\u2019t changed.<\/p>\n<p class=\"yf-vbsvxt\">Read More: The average net worth of Americans is a surprising $620,654. But it almost means nothing. <a data-i13n=\"cpos:4;pos:1\" href=\"https:\/\/moneywise.com\/managing-money\/retirement-planning\/the-average-americans-net-worth-is-620654-but-that-number-means-little-heres-the-figure-that-counts?throw=HALF_yahoo&amp;placement_syn=placement_2&amp;utm_source=syn_oath_mon&amp;utm_medium=BL&amp;utm_campaign=160649&amp;utm_content=syn_c63c673d-e247-4a46-96c3-02db9e8c49ef\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:Here\u2019s the number that counts (and how to make it skyrocket);cpos:4;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">Here\u2019s the number that counts (and how to make it skyrocket)<\/a><\/p>\n<p>    Story Continues  <\/p>\n<p class=\"yf-vbsvxt\">Mary Mayhew, president and CEO of the Florida Hospital Association, argues that facility fees help fund broader access to care, noting that proximity to care matters, particularly in fast-growing regions like Central Florida.<\/p>\n<p class=\"yf-vbsvxt\">\u201cOur hospitals are really not just hospitals anymore,\u201d she told WFTV. \u201cThey are health systems, and these health systems are trying to preserve access to an array of community-based services.\u201d<\/p>\n<p class=\"yf-vbsvxt\">But consumer advocates counter that patients shouldn\u2019t shoulder higher costs for routine care simply because of consolidation, especially when prices aren\u2019t transparent upfront.<\/p>\n<p class=\"yf-vbsvxt\">They\u2019re asking lawmakers to prohibit facility fees for routine outpatient care, require clearer public reporting and adopt site-neutral payment policies (patients pay the same price for the same service regardless of location).<\/p>\n<p class=\"yf-vbsvxt\">To date, 21 states have passed some form of legislation around the fees.<\/p>\n<p class=\"yf-vbsvxt\">The federal <a data-i13n=\"cpos:5;pos:1\" href=\"https:\/\/www.dol.gov\/agencies\/ebsa\/about-ebsa\/our-activities\/resource-center\/publications\/avoid-surprise-healthcare-expenses\" rel=\"nofollow noopener\" target=\"_blank\" data-ylk=\"slk:No Surprises Act;cpos:5;pos:1;elm:context_link;itc:0;sec:content-canvas\" class=\"link \">No Surprises Act<\/a>, which took effect in 2022, limits certain unexpected out-of-network bills, particularly for emergency care.<\/p>\n<p class=\"yf-vbsvxt\">Unfortunately, it doesn&#8217;t prohibit facility fees for outpatient services at hospital-owned clinics.<\/p>\n<p class=\"yf-vbsvxt\">That means for now, outpatients face higher out-of-pocket costs, particularly for those with high deductibles or co-insurance.<\/p>\n<p class=\"yf-vbsvxt\">While policymakers debate reforms, patients aren\u2019t powerless.<\/p>\n<p class=\"yf-vbsvxt\">There are several things you can do to avoid or minimize surprise healthcare fees:<\/p>\n<p class=\"yf-vbsvxt\">Ask upfront whether a provider charges a facility fee and whether the office is owned by a hospital system.<\/p>\n<p class=\"yf-vbsvxt\">Request a good-faith estimate before non-emergency care.
