{"id":461109,"date":"2026-02-11T00:11:10","date_gmt":"2026-02-11T00:11:10","guid":{"rendered":"https:\/\/www.newsbeep.com\/us\/461109\/"},"modified":"2026-02-11T00:11:10","modified_gmt":"2026-02-11T00:11:10","slug":"hospitals-and-insurers-are-fighting-over-money-leaving-patients-in-the-lurch","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/us\/461109\/","title":{"rendered":"Hospitals and insurers are fighting over money, leaving patients in the lurch"},"content":{"rendered":"<p id=\"anchor-966cad\" class=\"body-graf\">Natalie Reichel is due for her next cancer therapy in March.<\/p>\n<p id=\"anchor-8aa47b\" class=\"body-graf\">But a contract fight between Mount Sinai Health System in New York City, where she gets care, and Anthem Blue Cross Blue Shield, whose network her insurance uses, has cast doubt on whether she\u2019ll be able to get it on time.<\/p>\n<p id=\"anchor-1999ac\" class=\"body-graf\">\u201cI am feeling dubious,\u201d said Reichel, 40. <\/p>\n<p id=\"anchor-2f9dbb\" class=\"body-graf\">The dispute is about money: Mount Sinai says Anthem <a href=\"https:\/\/keepmountsinai.org\/\" rel=\"nofollow noopener\" target=\"_blank\">owes it more than $450 million<\/a> in unpaid claims, while Anthem says Mount Sinai is <a href=\"https:\/\/www.anthem.com\/update\/mountsinai#\" rel=\"nofollow noopener\" target=\"_blank\">demanding a 50% rate increase<\/a>. Mount Sinai\u2019s physicians went out of network for most Anthem plans on Jan. 1 after the two sides failed to reach a deal, though its hospitals and facilities remain in network until March 1. Roughly 200,000 Mount Sinai patients have Anthem coverage, according to the hospital system.<\/p>\n<p id=\"anchor-604661\" class=\"body-graf\">For Reichel, it means she\u2019ll need <a href=\"https:\/\/ag.ny.gov\/resources\/individuals\/health-care-insurance\/continuity-of-care\" rel=\"nofollow noopener\" target=\"_blank\">a special exception approved<\/a> by her insurance \u2014 something she says she\u2019s had trouble getting \u2014 or may need an entirely new team of specialists just to stay on her treatment schedule.<\/p>\n<p><img decoding=\"async\" loading=\"lazy\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2026\/02\/260210-Natalie-Reichel-and-wife-gk-b2ca47.jpg\" alt=\"Natalie Reichel, right, of New York City, pictured with her wife, Red Faolan.\" height=\"2500\" width=\"2500\"\/>Natalie Reichel, right, with her wife, Red Faolan.Courtesy Natalie Reichel<\/p>\n<p id=\"anchor-80656c\" class=\"body-graf\">Reichel was diagnosed with breast cancer when she was 33 years old. She\u2019s been in remission for about six years and regularly gets hormone-suppressing shots to reduce the risk of the cancer coming back. <\/p>\n<p id=\"anchor-dae7d5\" class=\"body-graf\">\u201cMy wife worries a lot about me getting a recurrence, because I was really young and it was really aggressive,\u201d Reichel said. <\/p>\n<p id=\"anchor-6a4eed\" class=\"body-graf\">Hospitals and insurers regularly renegotiate rates, but those discussions are usually handled behind closed doors and are typically resolved before patients feel an impact. <\/p>\n<p id=\"anchor-f99da4\" class=\"body-graf\">But in recent years, many negotiations have become more fraught, often spilling out into public view as both sides put out statements defending their position. When that happens, it\u2019s usually a bad sign for patients.  <\/p>\n<p id=\"anchor-25e2b1\" class=\"body-graf\">Jason Buxbaum, a researcher at Brown University, has been tracking disputes between hospitals and insurers since 2021. From June of that year through May 2025, about 1 in 5 hospitals had at least one public dispute with an insurance company, according to Buxbaum\u2019s preliminary findings. About 8% of the hospitals went out of network for the insurance company they were negotiating with, at least for a time. In total, roughly 500 to 600 public disputes between hospitals and insurers occur each year.<\/p>\n<p id=\"anchor-dd17b0\" class=\"body-graf\">\u201cThis has accelerated in recent years,\u201d he said. \u201cThere\u2019s definitely a potential for folks to get screwed over.\u201d<\/p>\n<p id=\"anchor-9db553\" class=\"body-graf\">The disputes have played out with some of the biggest names in health care. Insurance giant UnitedHealthcare <a href=\"https:\/\/www.uhc.com\/nyp\" rel=\"nofollow noopener\" target=\"_blank\">warned members<\/a> could lose in-network access at NewYork-Presbyterian unless the two sides reached an agreement. The insurer also missed a June 30 deadline with Memorial Sloan Kettering Cancer Center, though a deal <a href=\"https:\/\/www.fiercehealthcare.com\/payers\/memorial-sloan-kettering-set-go-out-unitedhealthcares-network-july-1\" rel=\"nofollow noopener\" target=\"_blank\">was reached the next day<\/a>. In Southeast Texas, Memorial Hermann Health System and Blue Cross Blue Shield of Texas are currently in <a href=\"https:\/\/memorialhermann.org\/about-us\/bcbstx-negotiations\" rel=\"nofollow noopener\" target=\"_blank\">contract talks<\/a> for commercial plans, with an April 1 deadline; Memorial Hermann ended its relationship with the insurer for Medicare Advantage plans on Jan. 1. Last year, Duke Health said it would leave Aetna\u2019s network if it didn\u2019t reach a deal \u2014 which <a href=\"https:\/\/www.dukehealth.org\/aetna\" rel=\"nofollow noopener\" target=\"_blank\">it ultimately did<\/a> weeks later.<\/p>\n<p id=\"anchor-dd7c6a\" class=\"body-graf\">Leemore Dafny, a professor of public policy at the Harvard Kennedy School, said <a href=\"https:\/\/www.nbcnews.com\/health\/health-news\/aca-medicaid-medicare-health-care-expensive-some-cheaper-others-rcna249526\" target=\"_blank\" rel=\"nofollow noopener\">rising health care costs<\/a> are a major reason these disputes are becoming more common.<\/p>\n<p id=\"anchor-4d035d\" class=\"body-graf\">According to <a href=\"https:\/\/www.aha.org\/costsofcaring#:~:text=Hospital%20Expenses%20are%20Growing%20Faster,from%20Strata%20Decision%20Technology%2C%20LLC.\" rel=\"nofollow noopener\" target=\"_blank\">the American Hospital Association<\/a>, an industry trade group, total hospital expenses grew 5.1% in 2024, significantly outpacing the overall inflation rate of 2.9%. It slowed in 2025, although it remains elevated, the group said.<\/p>\n<p id=\"anchor-d0f813\" class=\"body-graf\">\u201cHospital prices have grown tremendously,\u201d Dafny said. \u201cSo you can imagine that the pushback from insurers against requests or demands for payment increases is going to be pretty fixed.\u201d<\/p>\n<p id=\"anchor-519031\" class=\"body-graf\">Insurers have tried to use tools like <a href=\"https:\/\/www.nbcnews.com\/health\/health-news\/kennedy-says-health-insurers-promise-change-prior-authorization-proces-rcna214515\" rel=\"nofollow noopener\" target=\"_blank\">prior authorization<\/a> and <a href=\"https:\/\/www.nbcnews.com\/specials\/cost-of-denial\/index.html\" rel=\"nofollow noopener\" target=\"_blank\">claim denials<\/a> to tamp down on health care costs \u2014 but have faced growing public blowback over it, Dafny said. Now, she said, many insurers see contract negotiations as one of the few levers they have left.<\/p>\n<p id=\"anchor-1a8f45\" class=\"body-graf\">\u201cSo what\u2019s left is let\u2019s try to pay less for things or let\u2019s actually try to potentially exclude high cost providers altogether,\u201d she said. \u201c It\u2019s [the] job as an insurer to provide access to high quality care and try to keep it affordable. And that might mean not rolling over every time a provider demands a price increase, even if the provider is dealing with rising prices itself.\u201d<\/p>\n<p id=\"anchor-5eaafb\" class=\"body-graf\">Buxbaum said that another factor in the growing number of disputes is <a href=\"https:\/\/www.nbcnews.com\/health\/health-news\/hospitals-are-required-post-prices-common-procedures-rcna26984\" rel=\"nofollow noopener\" target=\"_blank\">recent federal transparency rules<\/a> that force hospitals and insurers to reveal more about what they charge \u2014 and what they\u2019re paid. In the past, hospitals often had a rough sense of what rate a nearby health system was getting from the insurer, but didn\u2019t know for sure, he said.<\/p>\n<p id=\"anchor-d562e1\" class=\"body-graf\">\u201cNow nobody wants to be the worst paid on the block,\u201d he said.<\/p>\n<p id=\"anchor-1e66b9\" class=\"body-graf\">Dafny said the problem is likely to grow, especially for Medicare Advantage, which now covers well over half of people on Medicare. Medicare Advantage is run by private insurers, rather than the government.<\/p>\n<p id=\"anchor-ed9e19\" class=\"body-graf\">Unlike traditional Medicare \u2014 where patients can generally see any provider who accepts it \u2014 Medicare Advantage plans typically rely on a smaller network of doctors and hospitals.<\/p>\n<p id=\"anchor-258a8f\" class=\"body-graf\">\u201cThere\u2019s no question that these processes slow things down and people suffer,\u201d she said.<\/p>\n<p id=\"anchor-4af913\" class=\"body-graf\">Brent Estes, the chief managed care officer at Mount Sinai and the lead negotiator in the Anthem dispute, said the hospital system is standing its ground. Mount Sinai is open to hammering out a new contract with Anthem \u2014 if the insurer is willing to meet its terms.<\/p>\n<p id=\"anchor-e67e56\" class=\"body-graf\">\u201cIt\u2019s not like we can get paid the same forever,\u201d Estes said. \u201cWe feel like we\u2019re substantially lower in terms of our payment rates than our academic peers, which puts us in a difficult position to compete in the market in the long run.\u201d<\/p>\n<p id=\"anchor-5a3b2d\" class=\"body-graf\">In an emailed statement, an Anthem spokesperson said the insurer\u2019s sticking point is cost.<\/p>\n<p id=\"anchor-c53079\" class=\"body-graf\">\u201cMount Sinai is seeking price increases and the removal of patient overbilling protections that would drive healthcare costs up by as much as 50 percent by 2028 \u2014 far outpacing inflation,\u201d the statement read. \u201cThose costs would ultimately be paid by patients, unions, employers, and governments.\u201d<\/p>\n<p id=\"anchor-d8b325\" class=\"body-graf\">The spokesperson said Anthem doesn\u2019t have the authority to grant Reichel so-called continuity of care approval. (Reichel\u2019s insurance is administered by International Benefits Administrators, a third party group that uses Anthem\u2019s provider network.) The exception can allow patients to keep seeing their doctors temporarily even after they go out of network. <\/p>\n<p id=\"anchor-e7b1ab\" class=\"body-graf\">Reichel said she is working on getting that approval from her insurer. <\/p>\n<p id=\"anchor-ec65f2\" class=\"body-graf\">In the meantime, Reichel said her providers gave her a three-month injection of her treatment at the end of last year because they worried she could face delays once they went out of network.<\/p>\n<p id=\"anchor-f19cdb\" class=\"body-graf\">She doesn\u2019t think a deal will be reached by the time she needs her next one.<\/p>\n<p id=\"anchor-eaf434\" class=\"endmark body-graf\">\u201cI\u2019m feeling like I\u2019ll be jumping through a whole bunch of hoops just to have to jump through a bunch of hoops again,\u201d she said.<\/p>\n","protected":false},"excerpt":{"rendered":"Natalie Reichel is due for her next cancer therapy in March.
