But a closer look shows that the lion&#8217;s share of this growth came <a href=\"https:\/\/www.nbcnews.com\/business\/economy\/january-jobs-revisions-trump-rcna258398\" target=\"_blank\" rel=\"nofollow noopener\">from one specific task:<\/a> caring for older Americans. <\/p>\n<p id=\"anchor-e0e028\" class=\"body-graf\">Grouped under the formal categories of &#8220;social assistance&#8221; and &#8220;healthcare,&#8221; at-home care services, hospitals and long-term care facilities added 124,000 positions.<\/p>\n<p id=\"anchor-eb36e9\" class=\"body-graf\">Much of this expansion was among the tens of thousands of aides and assistants who help elderly and disabled people bathe, dress, eat and manage their daily lives. Their work is essential \u2014 and booming \u2014 but it is also physically demanding and often modestly paid.<\/p>\n<p id=\"anchor-6b4c82\" class=\"body-graf\">The trend underscores a broader shift underway in the American labor market. While <a href=\"https:\/\/www.reuters.com\/business\/us-job-growth-accelerates-january-unemployment-rate-falls-43-2026-02-11\/\" target=\"_blank\" rel=\"nofollow noopener\">recent headlines<\/a> suggest that employment in the U.S. is resilient, much of today\u2019s job growth is not in the corporate offices or on the factory floors that have traditionally powered economic expansion. It is concentrated instead in labor-intensive care and service-based roles.<\/p>\n<p id=\"anchor-d20fa1\" class=\"body-graf\">As the country ages, demand for long-term care is projected to keep rising. At the same time, immigration restrictions, falling birth rates and sweeping Medicaid funding cuts are narrowing the pipeline of workers willing, or able, to fill these roles.<\/p>\n<p>Low pay, high demand<\/p>\n<p id=\"anchor-ae7818\" class=\"body-graf\">Health care spans some of the highest-paid professions in the country. Surgeons can earn <a href=\"https:\/\/data.bls.gov\/oes\/#\/industry\/000000\" target=\"_blank\" rel=\"nofollow noopener\">more than $450,000 annually<\/a>, data from the federal Bureau of Labor Statistics shows. But physicians are not the people driving the surge in health care work that amounted to more than 700,000 new jobs last year.<\/p>\n<p id=\"anchor-d4b2c6\" class=\"body-graf\">Nationwide, nearly 4 million people, most of them women, work as home health or personal care aides, per the BLS. Another roughly 1.5 million work as nursing assistants.<\/p>\n<p id=\"anchor-6d5cad\" class=\"body-graf\">Pay for many of these roles remains modest. Home health and personal care aides earn a median of about $16.82 an hour, or roughly $35,000 a year. By comparison, the federal poverty level for a family of four is $32,150.<\/p>\n<p id=\"anchor-99f1d0\" class=\"body-graf\">Nursing assistants earn a median of about $19.84 an hour, or roughly $41,000 annually. Both figures sit well below the national annual median wage of $49,500.<\/p>\n<p id=\"anchor-aa7bd3\" class=\"body-graf\">\u201cWhen we talk about the direct care workforce, we\u2019re talking about people who are providing typically long-term care services to older adults and younger people with disabilities,\u201d Priya Chidambaram, senior policy manager at <a href=\"https:\/\/www.kff.org\/\" target=\"_blank\" rel=\"nofollow noopener\">KFF<\/a>, a nonpartisan health policy research organization, told NBC News in an interview.<\/p>\n<p id=\"anchor-191501\" class=\"body-graf\">The direct care workforce is overwhelmingly female, about 87%, and more than a quarter are immigrants. Roughly 11% are noncitizen immigrants and 17% are naturalized citizens, according to KFF\u2019s analysis of federal data. About 40% of workers are at least 50 years old. Care at home is even more dependent upon immigrants, with around 1 in 3 home health workers foreign-born.<\/p>\n<p id=\"anchor-48a406\" class=\"body-graf\">Many enter the field after caring for a family member at home.<\/p>\n<p id=\"anchor-219df7\" class=\"body-graf\">\u201cWe see some people entering the workforce just from their own personal experience,\u201d Chidambaram said. Others want to work in health care, but encounter financial or educational hurdles that make a shorter certification program for a direct care role more attainable than a nursing degree.<\/p>\n<p id=\"anchor-8ea106\" class=\"body-graf\">Training requirements vary by state. Nursing assistants typically complete certified nurse aide training, while home health and personal care aides often take short certification courses. Roughly half of direct care workers have a high school education or less.<\/p>\n<p id=\"anchor-df42ee\" class=\"body-graf\">Despite relatively low educational barriers, the work itself is anything but easy.<\/p>\n<p id=\"anchor-ffe25e\" class=\"body-graf\">\u201cIt\u2019s emotionally and mentally and physically demanding work,\u201d Chidambaram said. \u201cThe financial benefits associated with the work don\u2019t meet up with the demands of the work itself.\u201d<\/p>\n<p id=\"anchor-03cd25\" class=\"body-graf\">Alyssa Crockett, a licensed practical nurse (LPN) currently working in long-term care, described the reality on the ground in <a href=\"https:\/\/www.tiktok.com\/@lyssacrockett14\/video\/7198933584638463275?_r=1&amp;_t=ZP-93s0cJs2fXD\" target=\"_blank\" rel=\"nofollow noopener\">a social media post: <\/a>\u201cWe cannot do everything. It\u2019s so messed up. My 12-hour shift? I get 25 minutes per person. \u2026 How is this possible?\u201d<\/p>\n<p id=\"anchor-cacf93\" class=\"body-graf\">\u201cI\u2019m truly at the point of realizing that I\u2019m going to lose my nursing license working in long-term care,\u201d she continued, \u201cwhich sucks, because it\u2019s my entire heart. We need to do better.\u201d<\/p>\n<p id=\"anchor-cbb544\" class=\"body-graf\">Not surprisingly, turnover within the profession is high. Studies show annual turnover among nursing assistants <a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7992115\/\" target=\"_blank\" rel=\"nofollow noopener\">can approach 100%,<\/a> and home care roles see turnover rates<a href=\"https:\/\/homehealthcarenews.com\/2025\/07\/inside-the-data-revenue-staffing-wins-and-challenges-shape-home-based-care-strategy\/\" target=\"_blank\" rel=\"nofollow noopener\"> of around 75%<\/a>, reflecting both burnout and unstable hours.<\/p>\n<p>An aging population<\/p>\n<p id=\"anchor-5c5ea1\" class=\"body-graf\">The strain on this workforce is building at the very moment the country needs it most.<\/p>\n<p id=\"anchor-3a8413\" class=\"body-graf\">The share of Americans ages 65 and older<a href=\"https:\/\/www.census.gov\/library\/stories\/2018\/03\/graying-america.html\" target=\"_blank\" rel=\"nofollow noopener\"> is projected to reach over 20% by 2035<\/a>, meaning roughly 1 in 5 Americans may require some form of care. Baby boomers are moving <a href=\"https:\/\/www.pewresearch.org\/short-reads\/2026\/01\/09\/the-oldest-baby-boomers-turn-80-in-2026\/\" target=\"_blank\" rel=\"nofollow noopener\">deeper into their 70s and 80s<\/a>, and more older adults say they want to age at home rather than in institutional settings.<\/p>\n<p id=\"anchor-c33ba2\" class=\"body-graf\">\u201cAll these things are combining to create this inevitability that there will not be enough of a workforce to meet the demands of an aging population,\u201d Chidambaram said.<\/p>\n<p id=\"anchor-c261eb\" class=\"body-graf\">Immigration policy adds another layer of uncertainty. Visa categories typically used by highly specialized workers, such as H-1B, are less common for these roles. Instead, employment-based immigrant visas and temporary protected status (TPS) holders are more likely to be represented, though detailed visa breakdowns are not publicly available.<\/p>\n<p id=\"anchor-ad780f\" class=\"body-graf\">The Trump administration has <a href=\"https:\/\/travel.state.gov\/content\/travel\/en\/News\/visas-news\/immigrant-visa-processing-updates-for-nationalities-at-high-risk-of-public-benefits-usage.html\" target=\"_blank\" rel=\"nofollow noopener\">tightened pathways for legal immigration<\/a>, including pausing or restricting some employment-based immigrant visas and<a href=\"https:\/\/forumtogether.org\/article\/temporary-protected-status-fact-sheet\/\" target=\"_blank\" rel=\"nofollow noopener\"> ending TPS designations for several countries<\/a> that have historically been the source of large numbers of care workers in American facilities.<\/p>\n<p id=\"anchor-bbce9b\" class=\"body-graf\">This <a href=\"https:\/\/www.kff.org\/racial-equity-and-health-policy\/what-role-do-immigrants-play-in-the-hospital-workforce\/\" target=\"_blank\" rel=\"nofollow noopener\">raises questions <\/a>about how future staffing needs will be met: \u201cThis workforce will be heavily impacted by federal policy as a result of immigration changes,\u201d Chidambaram said.<\/p>\n<p id=\"anchor-376244\" class=\"body-graf\">Medicaid, which pays for the majority of long-term care services in the United States, also plays a central role. Cuts to the program \u2014<a href=\"https:\/\/www.medicarerights.org\/medicare-watch\/2025\/10\/02\/states-already-cutting-medicaid-massive-federal-cuts-yet-to-come\" target=\"_blank\" rel=\"nofollow noopener\"> including approximately $1 trillion in reductions passed in recent legislation <\/a>\u2014 could ripple through agencies that employ aides and nursing assistants.<\/p>\n<p id=\"anchor-0ffa45\" class=\"body-graf\">Reduced funding would likely squeeze the workers themselves. The most probable outcome is that \u201cdirect care workers are going to be paid less somehow,\u201d said Chidambaram.<\/p>\n<p id=\"anchor-1c74b4\" class=\"body-graf\">While some economists have questioned whether an economy so dependent on elder care could eventually face a demographic drop-off once the baby boom generation passes, researchers say the more immediate risk is the opposite: not having enough workers to meet the coming waves of demand.<\/p>\n<p id=\"anchor-a15f63\" class=\"body-graf\">\u201cWe have this growing demand, and it sounds like the supply is going to reduce pretty significantly over the next few years,\u201d Chidambaram said, noting that the workforce serves not only aging Americans but also younger people with disabilities who rely on long-term care.<\/p>\n<p id=\"anchor-d4442f\" class=\"endmark body-graf\">It\u2019s a strain that\u2019s already building \u2014 and one that will not fade quickly. As Chidambaram put it: \u201cIt\u2019s going to impact us for the next 30 years.\u201d<\/p>\n<p><script async src=\"\/\/www.tiktok.com\/embed.js\"><\/script><\/p>\n","protected":false},"excerpt":{"rendered":"The U.S. added 130,000 jobs in January, according to federal data released this week.
