Three Rhode Island nursing assistants have been reprimanded – and a fourth was placed on probation – by the\u00a0Rhode Island Department of Health (RIDOH) over the past month, according to a review of state documents.<\/p>\n
RIDOH says that it investigated complaints \u201calleging unprofessional conduct\u201d by licensed nursing assistants Morena Mayorga, Sarah King, Odithe Rudio, and Sharease Smith, before taking action.\u00a0<\/p>\n

GET THE LATEST BREAKING NEWS HERE — SIGN UP FOR GOLOCAL FREE DAILY EBLAST<\/a><\/p>\n Mayorga was employed as a nursing assistant at an assisted living facility when, on October 1, 2025, Mayorga \u201cstruck a resident of the facility,\u201d according to RIDOH.\u00a0<\/p>\n Court records do not show that Mayorga was arrested for the incident.\u00a0<\/p>\n RIDOH says King was employed as a nursing assistant at a facility when on January 26, 2025, King \u201cspoke to a patient in a disrespectful manner.\u201d<\/p>\n Rudio was employed as a medication aide at an assisted living facility when on October 10, 2025, Rudio misidentified a resident and made a medication error, according to RIDOH.\u00a0<\/p>\n All three nursing assistants\u2019 behavior \u201cconstitutes unprofessional conduct pursuant to [Rhode Island] General Laws,\u201d says RIDOH, in issuing the reprimands, which are signed by the health professionals in addition to RIDOH.\u00a0<\/p>\n Probation\u00a0<\/p>\n According to RIDOH, Smith was employed as a nursing assistant by a staffing agency \u201cassigned to provide care to patients at a nursing facility.\u201d\u00a0<\/p>\n \u201cOn July 27, 2025, [Smith] was observed utilizing improper feeding techniques while feeding a patient,\u201d wrote RIDOH. \u201cOn several occasions, [Smith] spoon fed the patient, turned her back to the patient to continue speaking on her personal cell phone, and ignored the patient when the patient began coughing after being fed.\u201d<\/p>\n RIDOH said it \u201cconstitutes conduct subject to disciplinary proceedings\u201d pursuant to Rhode Island General Law – and placed Smith on \u201cactive probation.\u201d\u00a0<\/p>\n Among the requirements of the probation, Smith must provide all \u201chospitals and nursing facilities where she is employed as a nursing assistant with a copy of [the consent order] and a \u201cjob performance progress report form\u201d provided by RIDOH. \u00a0\u2028Smith can apply to have her license status changed to \u201cactive\u201d after a year.
