{"id":585802,"date":"2026-04-15T14:05:10","date_gmt":"2026-04-15T14:05:10","guid":{"rendered":"https:\/\/www.newsbeep.com\/us\/585802\/"},"modified":"2026-04-15T14:05:10","modified_gmt":"2026-04-15T14:05:10","slug":"scientists-may-now-know-why-glp-1s-dont-work-for-10-of-people-sciencealert","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/us\/585802\/","title":{"rendered":"Scientists May Now Know Why GLP-1s Don&#8217;t Work For 10% of People : ScienceAlert"},"content":{"rendered":"<p>Certain genetic variants, carried by around 10 percent of the population, could be making <a href=\"https:\/\/www.sciencealert.com\/diabetes\" rel=\"nofollow noopener\" target=\"_blank\">type 2 diabetes<\/a> GLP-1 treatments (such as Ozempic and Wegovy)  less effective, according to new research.<\/p>\n<p>These treatments are named after the glucagon-like peptide-1 (GLP-1) hormone that they simulate. In the case of  <a href=\"https:\/\/www.sciencealert.com\/diabetes\" class=\"lar_link lar_link_outgoing\" data-linkid=\"73018\" data-postid=\"198040\" rel=\"nofollow noopener\" target=\"_self\">diabetes<\/a>, that means <a href=\"https:\/\/www.sciencealert.com\/time-restricted-eating-helps-control-blood-sugar-in-type-2-diabetes\" rel=\"nofollow noopener\" target=\"_blank\">regulating blood sugar<\/a> by boosting insulin and slowing down digestion.<\/p>\n<p>However, the impact <a href=\"https:\/\/www.sciencealert.com\/the-biggest-mistake-people-make-with-glp-1-drugs-and-how-to-avoid-it\" rel=\"nofollow noopener\" target=\"_blank\">of GLP-1 drugs<\/a> on people with type 2 diabetes can vary considerably. The international team of researchers behind the new study wanted to see whether genetics may be playing a part.<\/p>\n<p><img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2026\/04\/GLPChart.jpg\" alt=\"GLP-1 chart\" width=\"642\" height=\"515\" class=\"wp-image-198043 size-full\"   loading=\"lazy\"\/>The study found people with certain gene variations were less sensitive and therefore more resistant to GLP-1 drugs (right). (Umapathysivam et al., Genome Med., 2026)<\/p>\n<p>To investigate, the team focused on an enzyme called PAM (short for <a href=\"https:\/\/en.wikipedia.org\/wiki\/Peptidylglycine_alpha-amidating_monooxygenase\" rel=\"nofollow noopener\" target=\"_blank\">peptidyl-glycine alpha-amidating monooxygenase<\/a>), and the gene that encodes it.<\/p>\n<p>Around 1 in 10 people carry variants in the PAM gene that can interfere with <a href=\"https:\/\/www.sciencealert.com\/study-uncovers-trove-of-hormones-that-could-freeze-time-on-aging-skin\" rel=\"nofollow noopener\" target=\"_blank\">the activation of hormones<\/a>, including GLP-1. PAM variants are also more common in people with diabetes.<\/p>\n<p>&#8220;When I treat patients in the diabetes clinic, I see a huge variation in response to these GLP-1-based medications and it is difficult to predict this response clinically,&#8221; <a href=\"https:\/\/med.stanford.edu\/news\/all-news\/2026\/04\/glp-1-diabetes.html\" rel=\"nofollow noopener\" target=\"_blank\">says<\/a> endocrinologist Mahesh Umapathysivam, from Adelaide University in Australia.<\/p>\n<p>&#8220;This is the first step in being able to use someone&#8217;s genetic makeup to help us improve that decision-making process.&#8221;<\/p>\n<p>In their analysis, the researchers compared 19 people with the PAM gene variant p.S539W to 19 matched controls, studying how their bodies tolerated consuming <a href=\"https:\/\/www.sciencealert.com\/scientists-quantified-the-harm-of-sugary-drinks-and-its-devastating\" rel=\"nofollow noopener\" target=\"_blank\">a sugary drink<\/a>.<\/p>\n<p>Surprisingly, those with the variant showed an abundance of GLP-1 in their system. It turned out p.S539W was creating GLP-1 resistance, stopping the hormone from being properly processed by the body.<\/p>\n<p>&#8220;Despite people with the PAM variant having higher circulating levels of GLP-1, we saw no evidence of higher biological activity,&#8221; <a href=\"https:\/\/med.stanford.edu\/news\/all-news\/2026\/04\/glp-1-diabetes.html\" rel=\"nofollow noopener\" target=\"_blank\">says<\/a> endocrinologist Anna Gloyn, from Stanford University.<\/p>\n<p>&#8220;They were not reducing their blood sugar levels more quickly. More GLP-1 was needed to have the same biological effect, meaning they were resistant to GLP-1.&#8221;<\/p>\n<p>This switch around was so unexpected, the researchers ran further tests on mouse models with the PAM gene deactivated. This meant no PAM enzyme, and the mice developed signs of GLP-1 resistance too, with blood sugar <a href=\"https:\/\/www.sciencealert.com\/blood-sugar-spikes-linked-with-69-higher-risk-of-alzheimers\" rel=\"nofollow noopener\" target=\"_blank\">left unregulated<\/a> despite elevated levels of GLP-1.<\/p>\n<p>The team didn&#8217;t stop there, either. The next step was to analyze  <a href=\"https:\/\/www.sciencealert.com\/clinical-trials\" class=\"lar_link lar_link_outgoing\" data-linkid=\"73032\" data-postid=\"198040\" rel=\"nofollow noopener\" target=\"_self\">clinical trials<\/a> covering a total of 1,119 participants, to see if people with specific PAM variations were reacting less favorably to GLP-1 drugs.<\/p>\n<p><a href=\"https:\/\/www.sciencealert.com\/newsletter?utm_source=promo_generic_health\" rel=\"nofollow noopener\" target=\"_blank\"><img decoding=\"async\" src=\"https:\/\/www.newsbeep.com\/us\/wp-content\/uploads\/2026\/03\/Generic-Health-Promo-Final-642x273.jpg\" alt=\"Subscribe to ScienceAlert's free fact-checked newsletter\" width=\"642\" height=\"273\" class=\"alignnone wp-image-182810 size-medium\"   loading=\"lazy\"\/><\/a><\/p>\n<p>Again, the findings backed up the idea that these genetic variants lead to GLP-1 resistance: Those who had the standard PAM gene variants reacted better to GLP-1 medication than those who didn&#8217;t. This connection wasn&#8217;t seen with the <a href=\"https:\/\/www.sciencealert.com\/breakthrough-diabetes-treatment-may-deliver-insulin-through-a-skin-cream\" rel=\"nofollow noopener\" target=\"_blank\">other diabetes drugs<\/a> trialed.<\/p>\n<p>There&#8217;s plenty of scope for future research here.
