{"id":60392,"date":"2025-08-05T15:05:10","date_gmt":"2025-08-05T15:05:10","guid":{"rendered":"https:\/\/www.newsbeep.com\/us\/60392\/"},"modified":"2025-08-05T15:05:10","modified_gmt":"2025-08-05T15:05:10","slug":"titan-submersible-disaster-was-preventable-coast-guard-says","status":"publish","type":"post","link":"https:\/\/www.newsbeep.com\/us\/60392\/","title":{"rendered":"Titan submersible disaster was preventable, Coast Guard says"},"content":{"rendered":"<p>PORTLAND, Maine (AP) \u2014 The deadly <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/titan-submersible-titanic-implosion-stockton-rush-b2294353a00fa88688ddb8fba3aefcde\" rel=\"nofollow noopener\" target=\"_blank\">Titan submersible disaster<\/a> that raised global alarms about private deep-sea tourism was the result of preventable safety failures and deliberate efforts to avoid oversight, according to a U.S. Coast Guard report released Tuesday.<\/p>\n<p>All five people inside the Titan <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/titanic-shipwreck-titan-submersible-search-deepsea-atlantic-implosion-90b9c54c3887c99099170a5afded15bc\" rel=\"nofollow noopener\" target=\"_blank\">died in a catastrophic implosion<\/a> as it descended to the <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/titanic-submersible-lost-james-cameron-5e6eeb0b66c2d669c89c6eba278b00d1\" rel=\"nofollow noopener\" target=\"_blank\">wreck of the Titanic<\/a> off Canada, and the dayslong search for the missing vessel grabbed international headlines. The Coast Guard convened its highest level of investigation in the aftermath.<\/p>\n<p>The Titan was owned by <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/video\/u-s-coast-guard-atlantic-ocean-charleston-robotics-national-0b71476c31ed45ed96a10ee3e2a861cb\" rel=\"nofollow noopener\" target=\"_blank\">OceanGate<\/a>, a private company based in Washington state. The operator of the submersible, OceanGate head Stockton Rush, died in the implosion. <\/p>\n<p>The report found the company\u2019s safety procedures were \u201ccritically flawed,\u201d citing \u201cglaring disparities\u201d between their safety protocols and actual practices. The disaster has <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/titanic-submersible-disaster-implosion-lawsuit-ba3efaf4c4dcb766698761206676b655\" rel=\"nofollow noopener\" target=\"_blank\">led to lawsuits<\/a> and calls for tighter regulation of the developing private deep sea expedition industry. <\/p>\n<p>Preventing the next Titan disaster<\/p>\n<p>Jason Neubauer, with the Marine Board of Investigation, said that the findings will help prevent future tragedies.<\/p>\n<p>\u201cThere is a need for stronger oversight and clear options for operators who are exploring new concepts outside of the existing regulatory framework,\u201d he said in a statement.<\/p>\n<p>OceanGate suspended operations in July 2023. A spokesperson for the company said it has been wound down and was fully cooperating with the investigation.<\/p>\n<p>\u201cWe again offer our deepest condolences to the families of those who died on June 18, 2023, and to all those impacted by the tragedy,\u201d said the spokesperson, Christian Hammond.<\/p>\n<p>Coast Guard report details \u2018red flags\u2019 at OceanGate<\/p>\n<p>Throughout the report, which spans more than 300 pages, investigators repeatedly point to OceanGate\u2019s culture of downplaying, ignoring and even falsifying key safety information to improve its reputation and evade scrutiny from regulators. OceanGate ignored \u201cred flags\u201d and had a \u201ctoxic workplace culture,\u201d while its mission was hindered by lack of domestic and international framework for submersible operations, the report says.<\/p>\n<p>Numerous OceanGate employees have come forward in the two years since the implosion to support those claims. The report says firings of senior staff members and the looming threat of being fired were used to dissuade employees and contractors from expressing safety concerns.<\/p>\n<p>\u201cBy strategically creating and exploiting regulatory confusion and oversight challenges, OceanGate was ultimately able to operate TITAN completely outside of the established deep-sea protocols,\u201d the report found. <\/p>\n<p>The Titan\u2019s inadequacies <\/p>\n<p>Investigators found that the submersible\u2019s design, certification, maintenance and inspection process were all inadequate. Coast Guard officials noted at the start of last year\u2019s hearing that the submersible had not been independently reviewed, as is standard practice.<\/p>\n<p>Mounting financial pressures in 2023 led to a decision by OceanGate to store the Titan submersible outdoors over the Canadian winter, where its hull was exposed to temperature fluctuations that compromised the integrity of the vessel, the report said. <\/p>\n<p>The Marine Board concluded that Rush, OceanGate\u2019s CEO, \u201cexhibited negligence\u201d that contributed to the deaths of four people. If Rush had survived, the case would have been handed off to the U.S. Department of Justice and he may have been subject to criminal charges, the board said.<\/p>\n<p>The Marine Board said one challenge of the investigation was that \u201csignificant amounts\u201d of video footage evidence that had been captured by witnesses was not subject to its subpoena authority because the witnesses weren\u2019t U.S. citizens. <\/p>\n<p>The victims of the Titan disaster<\/p>\n<p>In addition to Rush, the implosion killed French explorer Paul-Henri Nargeolet, British adventurer Hamish Harding and two members of a prominent Pakistani family, Shahzada Dawood and his son Suleman Dawood.<\/p>\n<p>The family of Nargeolet, a veteran French undersea explorer known as \u201cMr. Titanic,\u201d filed a more than $50 million <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/titanic-submersible-disaster-implosion-lawsuit-ba3efaf4c4dcb766698761206676b655\" rel=\"nofollow noopener\" target=\"_blank\">lawsuit<\/a> last year that said the crew experienced \u201cterror and mental anguish\u201d before the disaster. The lawsuit accused OceanGate of gross negligence.<\/p>\n<p>The Titan\u2019s final dive<\/p>\n<p>Titan had been making voyages to the Titanic site since 2021. The Titan\u2019s <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/titan-implosion-submersible-titanic-25a496d4e80c51009d43b8e220bb92d7\" rel=\"nofollow noopener\" target=\"_blank\">final<\/a> dive came on June 18, 2023, a Sunday morning when the submersible would lose contact with its support vessel about two hours later. The submersible was reported overdue that afternoon, and ships, planes and equipment were rushed to the scene about 435 miles (700 kilometers) south of St. John\u2019s, Newfoundland. <\/p>\n<p>Wreckage of the Titan would subsequently be found on the ocean floor about 330 yards (300 meters) off the bow of the Titanic, Coast Guard officials said.<\/p>\n<p>The Marine Board of Investigation held several days of <a class=\"Link AnClick-LinkEnhancement\" data-gtm-enhancement-style=\"LinkEnhancementA\" href=\"https:\/\/apnews.com\/article\/titan-submersible-implosion-hearing-2ae3c044e50a69647bd42cffe96c7f98\" rel=\"nofollow noopener\" target=\"_blank\">hearings<\/a> about the implosion in October 2024.
