SHELTON \u2014 A woman visited a Shelton <\/a>medical clinic just one time for\u00a0immunotherapy testing, but the facility billed Medicaid<\/a> for 92 appointments totaling about $14,670, according to arrest\u00a0warrants.<\/p>\n Another patient of that same clinic, Advanced Pain and Regenerative Medical Solutions Corporation, told investigators that a tall, blonde woman, whom the patient assumed was a doctor, performed a prick test and told her she had mild allergies to cats, mosquitoes and grass, the warrants say. She received one shot, but the clinic billed Medicaid for 66 appointments totaling about $10,220, investigators said.<\/p>\n Article continues below this ad<\/p>\n From Feb. 26, 2024 through Oct. 1, 2025, a forensic investigator found the clinic charged the government program meant to insure low-income people for about 4,000 fraudulent claims totaling approximately $660,000, the warrants say. The state Division of Criminal Justice found that services were either not performed, or done by an unsupervised, unqualified person, according to an agency news release.<\/p>\n Last Wednesday, the Office of the Chief State\u2019s Attorney\u2019s Medicaid Fraud Control Unit arrested clinic owner David\u00a0Waltzer, 62, of East Haven; treasurer Eid J. Atallah, 61, of Middlebury; human resources and office manager Terry\u00a0L. Choszczyk, 65, of Seymour; and Kimberly Choszczyk, 38, of Ansonia, who treated patients without a medical license, according to arrest warrants.<\/p>\n The four face charges of first-degree larceny by defrauding a public community, health insurance fraud, violation of license requirements, conspiracy to commit first-degree larceny by defrauding a public community, conspiracy to commit health insurance fraud and conspiracy to commit violation of license requirements.
