LucyRx has already acquired two smaller competitors in 2024 and 2025.\n<\/p>\n<p>Many smaller PBMs still rely on the Big Three PBMs for critical infrastructure, including claims processing, pharmacy networks, rebate administration, and other core services.\n<\/p>\n<p>At the same time, the growing complexity of state and federal regulation is creating additional operational and compliance burdens. Thanks, CAA! \ud83d\ude43\n<\/p>\n<p>Those costs will fall most heavily on smaller PBMs that lack the scale, capital resources, and integrated capabilities of the industry&#8217;s largest players.\n<\/p>\n<p>Meanwhile, transparency is becoming less of a differentiator. Or, as Syndrome put it in The Incredibles: \u201cWhen everyone\u2019s super, no one will be.\u201d\n<\/p>\n<p>Even today, many plan sponsors are skeptical about smaller PBMs\u2019 transparency claims. For smaller PBMs, <a href=\"https:\/\/www.drugchannels.net\/2025\/10\/cignas-rebate-free-pharmacy-model-three.html#:~:text=Results%20from%20Pharmaceutical%20Strategies%20Group%E2%80%99s\" target=\"_blank\" rel=\"nofollow noopener\">fewer than half of plan sponsors (44%) believe they had full revenue transparency, and only 51% said they understood rebate handling<\/a>. Smaller PBMs have an edge\u2014but it\u2019s narrower than you might think.\n<\/p>\n<p>The result? More pressure on smaller PBMs and, likely, more consolidation across the sector.\n<\/p>\n<p>ICYMI, here\u2019s my PBM shakeout video:\n<\/p>\n<\/p>\n<p><a href=\"https:\/\/drugch.nl\/4aEnGnJ\" target=\"_blank\" rel=\"nofollow noopener\">Drug Channels\u2019 Adam Fein: 340B Entering New \u2018Era of Transparency\u2019<\/a>, 340B Report<br \/>\n<br \/>William Newton at 340B Report wrote an excellent summary of DCI&#8217;s recent webinar on the evolving 340B landscape: <a href=\"https:\/\/drugch.nl\/june2026\" target=\"_blank\" rel=\"nofollow noopener\">340B in 2026: Market Shifts, Policy Battles, and What They Mean for Stakeholders<\/a>.<\/p>\n<p>As we discussed, manufacturers increasingly face the risk of triplicate discounts on the same prescription: an upfront 340B discount, an IRA Maximum Fair Price (MFP) refund, and a Medicaid rebate paid to states.\n<\/p>\n<p>During the webinar, I shared several predictions:\n<\/p>\n<p>The Inflation Reduction Act (IRA) will accelerate efforts to increase visibility into 340B claims and transactions.<br \/>\nCMS and the courts\u2014not Congress\u2014will continue to drive many of the most significant changes to the 340B program. <\/p>\n<p>Plan sponsors will intensify efforts to stop rebate losses from 340B contract pharmacy claims.<br \/>\n<a href=\"https:\/\/drugch.nl\/june2026\" target=\"_blank\" rel=\"nofollow noopener\">Click here to watch the webinar replay.<\/a><br \/>\n<br \/><a href=\"https:\/\/drugch.nl\/4vM8zRw\" target=\"_blank\" rel=\"nofollow noopener\"> 2026 Trends in Specialty Drug Benefits Report<\/a>, Pharmaceutical Strategies Group<br \/>\n<br \/>A persistent puzzle in the pharmacy benefit world surfaced again in Pharmaceutical Strategies Group\u2019s (PSG) latest benefit design reports.<\/p>\n<p>PBMs frequently state that they pass through 95%+ of manufacturer rebates to their employer clients. Check out Section 9.1.1. of our <a href=\"https:\/\/drugch.nl\/pharmacy\" target=\"_blank\" rel=\"nofollow noopener\">2026 Economic Report on U.S. Pharmacies and Pharmacy Benefit Managers<\/a>.<\/p>\n<p>So what&#8217;s behind the disconnect?\n<\/p>\n<p>Are employers defining &#8220;100% of rebates&#8221; differently than PBMs?<br \/>\nDo guaranteed rebate arrangements create confusion about what is actually being passed through?<br \/>\nWill the Consolidated Appropriations Act&#8217;s transparency requirements narrow this perception gap?<br \/>\nOr is something else going on?<\/p>\n<p><a href=\"https:\/\/www.linkedin.com\/posts\/adamjfein_pbm-drugchannels-drugbenefits-activity-7471913153560403968-ZdIK\" target=\"_blank\" rel=\"nofollow noopener\">Click here to check out the DCI community\u2019s lively LinkedIn debate on my question.<\/a><\/p>\n<p><a href=\"https:\/\/drugch.nl\/3PFwiDs\" target=\"_blank\" rel=\"nofollow noopener\">Identifying Meaningful Patient Savings on Generics: Direct-to-Consumer Prices Versus Commercial Insurance Cost Sharing<\/a>, Annals of Internal Medicine<br \/>\n<br \/>For a surprising number of generic drugs, paying cash through Mark Cuban Cost Plus Drug Company (MCCPDC) can be dramatically cheaper than using commercial insurance.<\/p>\n<p>A new study from Annals of Internal Medicine found:\n<\/p>\n<p>For generic prescriptions with out-of-pocket costs above $15, nearly 80% would have been cheaper through Cost Plus Drugs.<br \/>\nFor prescriptions with cost sharing above $100, the median patient cost dropped from $140 through insurance to just $25 through Cost Plus.<br \/>\nAffordability directly impacts adherence and outcomes. Unfortunately, the warped incentives of the U.S. drug channel mean that many insured patients pay more than they would under transparent cash pricing.<\/p>\n<p>Small caveat: Fewer than 15% of generic prescriptions had out-of-pocket costs above $15.\n<\/p>\n<p>Nonetheless, this is another reminder from Mark Cuban that complexity is the enemy of low costs and efficiency.<\/p>\n<p><a href=\"https:\/\/drugch.nl\/4dTBUC4\" target=\"_blank\" rel=\"nofollow noopener\">Nominations and Withdrawal Sent to the Senate<\/a>, The White House<br \/>\n<br \/>Congratulations to Ge Bai on her nomination to serve as Assistant Secretary of U.S. Department of Health and Human Services (HHS). <\/p>\n<p>My $0.02: Ge is exceptionally knowledgeable, thoughtful, and dedicated to improving healthcare.
